    Practical Recommendations of the Obesity Management Task Force of the European Association for the Study of Obesity for the Post-Bariatric Surgery Medical Management

    Bariatric surgery is today the most effective long-term therapy for the management of patients with severe obesity, and its use is recommended by the relevant guidelines of the management of obesity in adults. Bariatric surgery is in general safe and effective, but it can cause new clinical problems and is associated with specific diagnostic, preventive and therapeutic needs. For clinicians, the acquisition of special knowledge and skills is required in order to deliver appropriate and effective care to the post-bariatric patient. In the present recommendations, the basic notions needed to provide first-level adequate medical care to post-bariatric patients are summarised. Basic information about nutrition, management of co-morbidities, pregnancy, psychological issues as well as weight regain prevention and management is derived from current evidences and existing guidelines. A short list of clinical practical recommendations is included for each item. It remains clear that referral to a bariatric multidisciplinary centre, preferably the one performing the original procedure, should be considered in case of more complex clinical situations

    Engineering a Diverse Ligase Toolbox for Peptide Segment Condensation

    The substrate profile of peptiligase, a stable enzyme designed for peptide ligation in aqueous environments, was mapped using six different peptide libraries. The most discriminating substrate binding pocket proved to be the first nucleophile binding subsite (S1), which is crucial for the peptide ligation yield. Two important amino acids shaping the S1 pocket are M213 and L208. A site-saturation library of the M213 position yielded two variants with a significantly broadened substrate profile, i.e., M213G and M213P. Next, examination of two libraries with M213G+L208X and M213P+L208X (with X being any proteinogenic amino acid) resulted in a toolbox of enzymes which can accommodate any proteinogenic amino acid in the S1 pocket, except proline. The applicability of a particular enzyme variant in chemoenzymatic peptide synthesis was demonstrated by coupling at the gram scale of two peptide segments to yield exenatide, a 39-mer therapeutic peptide used in the treatment of diabetes type II. The overall yield of 43% is at least 2-fold higher than yields reported for conventional syntheses of exenatide by full solid-phase peptide synthesis; large-scale production costs are expected to be significantly reduced if the enzymatic coupling process is employed to manufacture this peptide.</p

    Performance comparison of aperture-less and confocal infrared microscopes

    We compared hyperspectral infrared raster maps and images for contrast, definition and resolution of the same samples recorded with a confocal microscope coupled with a synchrotron radiation source vs a Focal Plane Array (FPA) detector equipped microscope. Biological samples (hair and skin sections) and astrophysics samples (meteoritic grains) were used. The samples presented are a few microns in size, such as embedded particles, a single unique cell or thin layer. Our results show that the actual spatial resolution and contrast of FPA images were lower than spectral maps from the confocal microscope. The FPA microscope also produced measurements that lacked accuracy: size of sample features and peak intensity were inaccurately estimated. More surprisingly, the intensity of absorption peaks in the FPA images was lower than the intensity measured from the same sample with a confocal microscope. Our measurements underlined the complementarity of FPA and confocal microscopes. FPA can be used to quickly measure the overall composition of a sample and detect the distribution of its components, but may fail measuring the exact chemical composition of the small features and may not detect weak spectral differences between adjacent positions. The averaging effect of aperture-less systems not only affects image resolution but also lowers their spectral accuracy. Confocal microscopes are inherently slower but give a more accurate measurement of the local composition at the diffraction limit
