    Prevalence and significance of alterations in cardiac structure and function in patients with heart failure and a preserved ejection fraction

    <p><b>Background:</b> The purpose of this study was to examine the prevalence of abnormalities in cardiac structure and function present in patients with heart failure and a preserved ejection fraction (HFPEF) and to determine whether these alterations in structure and function were associated with cardiovascular morbidity and mortality.</p> <p><b>Methods and Results:</b> The Irbesartan in HFPEF trial (I-PRESERVE) enrolled 4128 patients; echocardiographic determination of left ventricular (LV) volume, mass, left atrial (LA) size, systolic function, and diastolic function were made at baseline in 745 patients. The primary end point was death or protocol-specific cardiovascular hospitalization. A secondary end point was the composite of heart failure death or heart failure hospitalization. Associations between baseline structure and function and patient outcomes were examined using univariate and multivariable Cox proportional hazard analyses. In this substudy, LV hypertrophy or concentric remodeling was present in 59%, LA enlargement was present in 66%, and diastolic dysfunction was present in 69% of the patients. Multivariable analyses controlling for 7 clinical variables (including log N-terminal pro-B–type natriuretic peptide) indicated that increased LV mass, mass/volume ratio, and LA size were independently associated with an increased risk of both primary and heart failure events (all P<0.05).</p> <p><b>Conclusions:</b> Left ventricular hypertrophy or concentric remodeling, LA enlargement, and diastolic dysfunction were present in the majority of patients with HFPEF. Left ventricular mass and LA size were independently associated with an increased risk of morbidity and mortality. The presence of structural remodeling and diastolic dysfunction may be useful additions to diagnostic criteria and provide important prognostic insights in patients with HFPEF.</p&gt

В связи с развитием газификации (увеличением протяженности газопроводов, ростом количества газифицированных квартир, объектов и т.д.), старением газопроводов и изношенностью оборудования нормированные потери газа в газораспределительных сетях немного повысятся и к 2030 году составят около 2% от объёмов потребления. Ожидается, что потери природного газа в газотранспортной системе в 2030 году достигнут уровня 0,3% от объёмов его транспортировки. В работе рассмотрены виды и источники технологических потерь природного газа, изучены способы их уменьшения при транспортировке по магистральным газопроводам.In gas-distributing network there can be normalized and above-standard production and technological losses of natural gas. Due to the development of gasification (the extension of gas pipelines, a growing number of gasified apartments, facilities, etc.), ageing of gas pipelines and the depreciation of equipment normalized-losses of gas in gas distribution networks will increase slightly in 2030 will be about 2% of the volume of consumption. It is expected that losses of natural gas in the transmission system in 2030 will reach the level of 0.3% of total transport. The article discusses the types and sources of technological losses of natural gas, studied ways to reduce them during transportation through trunk gas pipelines

    Superconductivity in striped and multi-Fermi-surface Hubbard models: From the cuprates to the pnictides

    Single- and multi-band Hubbard models have been found to describe many of the complex phenomena that are observed in the cuprate and iron-based high-temperature superconductors. Simulations of these models therefore provide an ideal framework to study and understand the superconducting properties of these systems and the mechanisms responsible for them. Here we review recent dynamic cluster quantum Monte Carlo simulations of these models, which provide an unbiased view of the leading correlations in the system. In particular, we discuss what these simulations tell us about superconductivity in the homogeneous 2D single-orbital Hubbard model, and how charge stripes affect this behavior. We then describe recent simulations of a bilayer Hubbard model, which provides a simple model to study the type and nature of pairing in systems with multiple Fermi surfaces such as the iron-based superconductors.Comment: Published as part of Superstripes 2011 (Rome) conference proceeding

    A de novo variant in the bovine ADAMTSL4 gene in an Original Braunvieh calf with congenital cataract

    Inherited forms of cataract are a heterogeneous group of eye disorders known in livestock species. Clinicopathological analysis of a single case of impaired vision in a newborn Original Braunvieh calf revealed nuclear cataract. Whole-genome sequencing of the parent-offspring trio revealed a de novo mutation of ADAMTSL4 in this case. The heterozygous p.Arg776His missense variant affects a conserved residue of the ADAMTSL4 gene that encodes a secreted glycoprotein expressed in the lens throughout embryonic development. In humans, ADAMTSL4 genetic variants cause recessively inherited forms of subluxation of the lens. Given that ADAMTSL4 is a functional candidate gene for inherited disorders of the lens, we suggest that heterozygosity for the identified missense variant may have caused the congenital cataract in the affected calf. Cattle populations should be monitored for unexplained cataract cases, with subsequent DNA sequencing a hypothesized pathogenic effect of heterozygous ADAMTSL4 variants could be confirmed

    A New Functional Method to Choose the Target Lobe for Lung Volume Reduction in Emphysema - Comparison with the Conventional Densitometric Method

    Lung volume reduction (LVR) improves breathing mechanics by reducing hyperinflation. Lobar selection usually focuses on choosing the most destroyed emphysematous lobes as seen on an inspiratory CT scan. However, it has never been shown to what extent these densitometric CT parameters predict the least deflation of an individual lobe during expiration. The addition of expiratory CT analysis allows measurement of the extent of lobar air trapping and could therefore provide additional functional information for choice of potential treatment targets.To determine lobar vital capacity/lobar total capacity (LVC/LTC) as a functional parameter for lobar air trapping using on an inspiratory and expiratory CT scan. To compare lobar selection by LVC/LTC with the established morphological CT density parameters.36 patients referred for endoscopic LVR were studied. LVC/LTC, defined as delta volume over maximum volume of a lobe, was calculated using inspiratory and expiratory CT scans. The CT morphological parameters of mean lung density (MLD), low attenuation volume (LAV), and 15th percentile of Hounsfield units (15%P) were determined on an inspiratory CT scan for each lobe. We compared and correlated LVC/LTC with MLD, LAV, and 15%P.There was a weak correlation between the functional parameter LVC/LTC and all inspiratory densitometric parameters. Target lobe selection using lowest lobar deflation (lowest LVC/LTC) correlated with target lobe selection based on lowest MLD in 18 patients (50.0%), with the highest LAV in 13 patients (36.1%), and with the lowest 15%P in 12 patients (33.3%).CT-based measurement of deflation (LVC/LTC) as a functional parameter correlates weakly with all densitometric CT parameters on a lobar level. Therefore, morphological criteria based on inspiratory CT densitometry partially reflect the deflation of particular lung lobes, and may be of limited value as a sole predictor for target lobe selection in LVR

    We analyze the stability of the vortex lattice in a rotating superfluid against thermal fluctuations associated with the long-wavelength Tkachenko modes of the lattice. Inclusion of only the two-dimensional modes leads formally to instability in infinite lattices; however, when the full three-dimensional spectrum of modes is taken into account, the thermally-induced lattice displacements are indeed finite.Comment: 16 page

    Magnetization Jump in a Model for Flux Lattice Melting at Low Magnetic Fields

    Using a frustrated XY model on a lattice with open boundary conditions, we numerically study the magnetization change near a flux lattice melting transition at low fields. In both two and three dimensions, we find that the melting transition is followed at a higher temperature by the onset of large dissipation associated with the zero-field XY transition. It is characterized by the proliferation of vortex-antivortex pairs (in 2D) or vortex loops (in 3D). At the upper transition, there is a sharp increase in magnetization, in qualitative agreement with recent local Hall probe experiments.Comment: updated figures and texts. new movies available at http://www.physics.ohio-state.edu:80/~ryu/jj.html.     Superfluid-insulator transition of the Josephson junction array model with commensurate frustration

    We have studied the rationally frustrated Josephson-junction array model in the square lattice through Monte Carlo simulations of (2+1)(2+1)D XY-model. For frustration f=1/4f=1/4, the model at zero temperature shows a continuous superfluid-insulator transition. From the measurement of the correlation function and the superfluid stiffness, we obtain the dynamical critical exponent z=1.0z=1.0 and the correlation length critical exponent ν=0.4±0.05\nu=0.4 \pm 0.05. While the dynamical critical exponent is the same as that for cases f=0f=0, 1/2, and 1/3, the correlation length critical exponent is surprisingly quite different.
