Social-emotional factors underlying internalizing problems and peer relations in deaf or hard of hearing youth
The goal of this dissertation was to enhance our understanding about internalizing problems and peer relations in DHH youth (aged around 11 years). This was done along principles of the developmental psychopathology framework in which DHH children were compared to their hearing counterparts and in which multiple (underlying) social-emotional factors were examined. Results showed that DHH youth reported equal levels of somatic complaints, but more depressive symptoms, lower friendship quality and some forms of victimization more than their hearing peers. Throughout the dissertation a recurring DHH-related characteristic causing variability within the DHH sample was type of education; DHH children in special education reported poorer outcomes than their DHH peers in mainstream education. Concerning the underlying factors, a key finding was that emotional functioning underlies the development of internalizing problems and peer relations in both DHH and hearing youth, while the influence of social factors caused a discrepancy between the two groups. For example, poor social understanding is related to depressive symptoms in DHH youth, while this relation is absent in the hearing sample. Use of the Brief Shame and Guilt Questionnaire in Deaf and Hard of Hearing Children and Adolescents
No assessment tools are available to measure shame and guilt in children who are deaf or hard of hearing (DHH), while these self-conscious emotions might play a role in the frequently noted social and behavioral problems in this group. Therefore, the aim of this study was to validate the Brief Shame and Guilt Questionnaire (BSGQ) in DHH children. In addition, we examined associations of shame and guilt with social anxiety, self-esteem, delinquency, and psychopathic behaviors. A sum of 225 hearing (Mage= 11.62 years) and 108 DHH (Mage= 11.82 years) participants completed the self-report BSGQ. Multigroup confirmatory factor analysis confirmed the two-factor structure (i.e., shame and guilt) of the BSGQ in the DHH group. Measurement invariance was established across both groups. However, the DHH group reported lower levels of self-conscious emotions in comparison with the hearing group. The BSGQ showed good concurrent validity, where shame was associated with higher levels of social anxiety and lower levels of self-esteem, and guilt was associated with lower levels of delinquency and psychopathic behavior in both groups. Future research should investigate the potential behavioral consequences of lower reported levels of self-conscious emotions in DHH youth

Impact of ultraviolet radiation on marine crustacean zooplankton and ichthyoplankton: a synthesis of results from the estuary and Gulf of St. Lawrence, Canada
The objectives of the research program reported upon here were (1) to measure ambient levels of UV radiation and
determine whichvariables most strongly affected its attenuation in the waters of the estuary and Gulf of St. Lawrence, Canada; and
(2) to investigate the potential direct impacts of W radiation on species of crustacean zooplankton and fish whose early life stages
are planktonic. In this geographic region, productivity-determining biophysical interactions occur in the upper 0 to 30 m of the
water column. Measurements of the diffuse attenuation coefficients for ultraviolet-B radiation (W-B, 280 to 320 nm) at various
locations in this region indicated maximum 10% depths (the depth to which 10% of the surface energy penetrates at a given wavelength)
of 3 to 4 m at a wavelength of 310 nm. Organisms residing in this layer-including the eggs and larvae of Calanus finmarchicus
and Atlantic cod Gadus morhua-are exposed to biologically damaging levels of W radiation. As a result of these physical
and biological characteristics, this system offered a relevant opportunity to assess the impacts of UV on subarctic marine
ecosystems. Eggs of C. finmarchicus were incubated under the sun, with and without the W-B and/or UV-A (320 to 400 nm) wavebands.
W-exposed eggs exhibited low percent hatchmg compared to those protected from W : W radiation had a strong negative
impact on C. finmarchicus eggs. Further, percent hatching in W-B-exposed eggs was not significantly lower than that in eggs
exposed to UV-A only: under natural sunlight, UV-A radiation appeared to be more detrimental to C. finmarchicus embryos than
was UV-B. In analogous experiments with Atlantic cod eggs, exposure to UV-B produced a significant negative effect. However,
UV-A had no negative effect on cod eggs. Additional experiments using a solar simulator (SS) revealed high wavelength-dependent
mortality in both C. finmarchicus and cod embryos exposed to UV. The strongest effects occurred under exposures to wavelengths
below 312 nm. At the shorter wavelengths (<305 nm) UV-B-induced mortality was strongly dose-dependent, but (for both
C. finmarchicus and cod) not significantly influenced by dose-rate. Thus, at least within the limits of the exposures under which the
biological weighting functions (BWFs) were generated, reciprocity held. The BWFs derived for UV-B-induced mortality in C. finmarchicus
and cod eggs were similar in shape to the action spectrum for UV-B effects on naked DNA. Further, the wavelengthdependence
of DNA damage was similar to that for the mortality effect. These observations suggest that W-induced mortality in
C. finmarchicus and cod eggs is a direct result of DNA damage. There was no evidence of a detrimental effect of UV-A radiation in
these SS-derived results. A mathematical model that includes the BWFs, vertical mixing of eggs, meteorological and hydrographic
conditions, and ozone depletion, indicates that W-induced mortality in the C. finmarchicus egg population could be as high as
32.5 %, while the impact on the cod egg population was no more than 1.2%. Variability in cloud cover, water transparency (and the
variables that affect it), and vertical distribution and displacement of planktonic organisms within the mixed layer can all have a
greater effect on the flux of UV-B radiation to which they are exposed than will ozone layer depletion at these latitudes. Our observations
indicate that C, finmarchicus and cod eggs present in the first meter of the water column (likely only a small percentage of
the total egg populations) are susceptible to W radiation. However, although exposure to UV can negatively impact crustacean
zooplankton and ichthyoplankton populations, these direct effects are likely minimal within the context of all the other environmental
factors that produce the very high levels of mortality typically observed in their planktonic early life stages. The impact of
indnect effects-which may well be of much greater import-has yet to be evaluated
Friendship and emotion control in pre-adolescents with or without hearing loss
Emotional functioning plays a crucial role in the social development of children and adolescents. We examined the extent to which emotion control was related to the quality of friendships in pre-adolescents with and without hearing loss. We tested 350 pre-adolescents (75 deaf/hard of hearing in mainstream education (DHHm), 48 deaf/hard of hearing in special education (DHHs), and 227 hearing) through self-report. Outcomes confirmed a positive association between emotion control and positive friendships for all groups, with one notable exception: more approach strategies for emotion regulation were associated with more negative friendship features in the DHHs group. In addition, the DHHm group demonstrated high levels of emotion control, while their levels of positive friendship features were still lower compared to the hearing group
Congenital cardiovascular defects in children with intestinal malrotation
Intestinal malrotation (IM) and cardiovascular defects (CCVD) are both common congenital defects. We investigated the prevalence and types of CCVD in a 25-year IM population, and its association with post-IM-operative morbidity and mortality. Data on the type of CCVD, other congenital defects, syndromes, associations, post-IM-operative morbidity and mortality were retrospectively reviewed from the records of IM patients born between 1980 and 2005. Data were analyzed on (significant) differences between CCVD subgroups, and risk factors for both morbidity and mortality were calculated. Seventy-seven of 284 IM patients (27.1%) were diagnosed with a major or minor CCVD (37 and 40 patients, respectively). Syndromes and associations were more frequently diagnosed in patients with major than with a minor CCVD (67.6 vs. 40%, respectively). Post-IM-operative complications, although frequently observed (61%), did not differ between patients with major and minor CCVD. Physical CCVD signs before IM surgery increased post-IM-operative morbidity significantly (OR 4.0, 95% CI 1.4â11.0). Fifteen patients died (19.5%), seven due to cardiovascular cause. Mortality risk was increased by intestinal ischemia and post-IM-operative complications and by major CCVD after correction for age at weight at the time of IM operation. Congenital cardiovascular defects in children with intestinal malrotation are common, with high morbidity and mortality rates after IM operation. Elective IM surgery in young patients with CCVD should be performed in a centre with adequate paediatric cardiac care. Benefits of laparoscopic intervention need further study
Methotrexate therapy associated with a reduced rate of new-onset uveitis in patients with biological-naĂŻve juvenile idiopathic arthritis
OBJECTIVE: To study the effect of methotrexate (MTX) therapy on new-onset uveitis in patients with biological-naĂŻve juvenile idiopathic arthritis (JIA). METHODS: In this matched case-control study, we compared MTX exposure between cases with JIA-associated chronic uveitis (JIA-U) and patients with JIA and without JIA-U at the time of matching (controls). Data were collected from electronic health records of the University Medical Centre Utrecht, the Netherlands. Cases with JIA-U were matched 1:1 to JIA control patients based on JIA diagnosis date, age at JIA diagnosis, JIA subtype, antinuclear antibodies status and disease duration. The effect of MTX on JIA-U onset was analysed using a multivariable time-varying Cox regression analysis. RESULTS: Ninety-two patients with JIA were included and characteristics were similar between cases with JIA-U (n=46) and controls (n=46). Both ever-use of MTX and exposure years were lower in cases with JIA-U than in controls. Cases with JIA-U significantly more often discontinued MTX treatment (p=0.03) and out of those who did, 50% afterwards developed uveitis within 1âyear. On adjusted analysis, MTX was associated with a significantly reduced new-onset uveitis rate (HR: 0.35; 95% CI: 0.17 to 0.75). No different effect was observed between a low (<10âmg/m2/week) and standard MTX dose (â„10âmg/m2/week). CONCLUSION: This study demonstrates an independent protective effect of MTX on new-onset uveitis in patients with biological-naĂŻve JIA. Clinicians might consider early initiation of MTX in patients at high uveitis risk. We advocate more frequent ophthalmologic screening in the first 6-12 months after MTX discontinuation
