    Focussed antenatal care a possible opportunity towards safe motherhood

    Background: The Primary Health Care Setting gives a challenging opportunity for the clinicians to deal with pregnancy into favorable outcomes solely based on the clinical skills in view of innumerable socio-cultural-economic barriers. The Pregnant women make satisfactory progress-  to  full term, deliver  with  minimal morbidity, no loss of life and healthy baby-How to ensure? This is the objectives of our study. Suppose Obstetricians spare time, use checklist, Prioritize and provide care will it make any difference in saving mothers? Objectives: Describe in detail the process of Focused Antenatal Care as practiced in Primary Health Care setting and Minimize mortality and morbidity due to pregnancy by 25 percent from 169. (Maternal Mortality Rate (MMR 169).Methods: This is a community based descriptive, prospective, cohort study about a group of pregnant women till their delivery, using multiple cluster random sampling of 251 high risk pregnant women and subsequent follow up over 3 months with focused care. Compilation of data and analysis using SPSS Version 20.Results: Total Study participants 251 represented all the sections of target population with regard to socio-economic and cultural background. The participants attended FANC giving a response rate of 100%. These participants had one or more risk factor. All but 10 participants attended 4 or more FANC clinic visits 241 (96%). In these participants the commonest manageable morbid conditions are underweight (20%), anaemia (14%) preeclampsia (8%), eclampsia (2%) and gestational diabetes. In our finding 87% mothers completed full term pregnancy, 11.5% preterm (>28 but 2.5Kg, 13% Baby weight < 2.5 Kg. with an average of 2.9Kg.Conclusions: We are able to describe the Focused ANC and able to help improve the quality of life and to minimize morbidity and mortality in pregnant women

    Periodontal status among adult cigarette smokers using Miswak (Salvadora persica) for oral hygiene: A crosssectional study from Asir Region of Saudi Arabia

    Purpose: To compare the periodontal status of exclusive users of Miswak (Salvadora persica) with that of exclusive toothbrush users among adult smokers of cigarettes in Saudi Arabia.Methods: This educational-institutional study included one hundred and fifty adult patients (age and socioeconomic status matched, SES) between the ages of 18 and 75, and were listed as the group I, II, and III, with 50 participants each (participants with no oral hygiene, toothbrush users and miswak users respectively). Oral hygiene habits, the number of cigarettes smoked, and time since the habit began. Periodontal and radiographic parameters were reported.Results: All parameters related to periodontium (plaque index (PI), gingival index (GI), bleeding on probing (BOP), probing pocket depth (PPD), gingival recession (Rec), clinical attachment level (CAL) and bone loss (BL)) showed that the analysed groups exhibited differences (p &lt; 0.001). Least amount of BL (anterior and posterior) was found in toothbrush users and Miswak users, while least amount of attachment loss (both anterior and posterior) was recorded among Miswak users (p &gt; 0.05). Positive association was found for GI (r = 0.753) and recession (r = 0.436, p &lt; 0.001) in terms of the number of cigarettes smoked in group I, and anterior PPD (r = 0.388, p &lt; 0.001) in group III for the duration of smoking. A negative correlation was found for BOP in group III (r = - 0.339, p &lt; 0.05) in terms of frequency of tooth cleaning and GI (r = - 0.381) (p &lt; 0.001) in group II, in terms of time taken attain oral hygiene.Conclusion: This study indicates less severe periodontal damage among adult current cigarette smokers in miswak users than in toothbrush users.     Periodontal Treatment Practice and Referral Profile of General Dentists – A Cross-sectional Questionnaire Survey

    BACKGROUND: Because of medical advancement, the mean age of the general population is increasing gradually. This increase in the mean age of patients has led to its implications on oral health. Hence, it is logical to assume an increase in the figure of referrals to specialists and the complexity of treatments provided. AIM: The aim of the survey was to assess the current status of periodontal treatment provided, awareness of referral profile, and criteria taken into consideration for the referral to the periodontist by general dentists. MATERIALS AND METHODS: This cross-sectional questionnaire study was conducted among 318 general dentists. The relevant work related to the status of periodontal procedures provided by dentists and their referral relationships were studied. The questionnaire consisting of six questions was designed. Descriptive statistics, including frequency distributions and measures of central tendency and Chi-square test, were used. RESULTS: The study showed that majority of the general dentists do non-surgical procedures by their own. However, around 85% of respondents did not provide surgical periodontal treatment by themselves. About 25% of the responding general dentists thought that they had inadequate education and experience in the field of periodontics which was a major reason for the referral to specialists. CONCLUSION: It was concluded that the general dentists are comfortable in carrying out the non-surgical procedures, but they had some reservations about the periodontal surgical procedures. Inadequate experience and facilities were the major reasons for the referral to periodontists. More of this type of study should be conducted to know the basic level of periodontal practices carried out among the general dentists and about the referral to the specialists

    The utilization of hybrid nanofluids (HNs) to boost heat transfer is a promising area of study, and thus, numerous scientists, researchers, and academics have voiced their admiration and interest in this area. One of the main functions of nanofluids is their dynamic role in cooling small electrical devices such as microchips and associated gadgets. The major goal of this study is to perform an analysis of the buoyancy flow of a shrinking/stretching sheet, whilst considering the fascinating and practical uses of hybrid nanofluids. The influence of a nonlinear heat source/sink induced by a micropolar fluid is also inspected. Water-based alumina and copper nanoparticles are utilized to calculate the fine points of the fluid flow and the features of heat transfer. The governing equations are framed with acceptable assumptions and the required similarity transformations are used to turn the set of partial differential equations into ordinary differential equations. The bvp4c technique is used to solve the simplified equations. Dual solutions are presented for certain values of stretching/shrinking parameters as well as the mixed convective parameter.     Features of radiative mixed convective heat transfer on the slip flow of nanofluid past a stretching bended sheet with activation energy and binary reaction

    The current exploration aims to inspect the features of thermal radiation on the buoyancy or mixed convective fluid flow induced by nanofluid through a stretching permeable bended sheet. The impact of activation energy and binary reaction along with slip migration is taken into account to discuss the fine points of water-based alumina nanoparticle flow. The structure of the curved sheet is assumed to be stretchable and the bended texture is coiled within a circular section with radius (Formula presented.). The similarity technique is utilized to reduce the leading partial differential equations into ordinary differential equations. These reduced equations are then deciphered numerically by employing the bvp4c method. The outcomes of the model were constructed in the form of several figures and bar graphs for the case of opposing and assisting flows with varying distinct embedded control parameters. More precisely, the outcomes show that the temperature distribution profile increases with the increase in nanoparticle’s volume fraction as well as thermal radiation parameter. In addition, the heat and mass transfer rates were significantly improved for the higher value of the radiation and Schmidt number.     Role of endoscopy in evaluating upper gastrointestinal tract lesions in rural population

    Background and Objectives: Before the advent of endoscopy direct access to the lesion for the confirmation of the diagnosis was difficult, this posed difficulty in contemplating adequate and appropriate surgery. Endoscopy as a diagnostic and therapeutic tool has grown in recent years. Upper gastrointestinal (GI) endoscopy is one of the most fascinating branch which serves not only as a means of resolving or amplifying the diagnosis made clinically or by X-ray, but also a primary diagnostic procedure for conditions not otherwise diagnosable on unoperated case. Fiber optic upper GI endoscopy has already become firmly established as a reliable, quick and inexpensive tool. This study was done to detect the upper gastrointestinal lesions in rural population of Kolar District, the distribution pattern of various upper GI lesions in patients presenting with upper GI symptoms and to follow the endoscopic diagnosis for medical and surgical management. Materials and Methods: The study group includes patients reporting to outpatient department and also the inpatients in wards of General Surgery and other departments, who have upper GI symptoms, were advised endoscopy at R. L. JALAPPA Hospital and Research Centre, Kolar, from a period of December 2011 to August 2013. Results: Of the 600 cases, 370 were males, and 230 were females. Disease incidence was highest in 51–70 years age group, that is, 21.6%. Pain abdomen was the most common symptom. Epigastric tenderness was the most common sign among the patients clinically. Reflux esophagitis and diffuse gastritis formed most common cases (307 cases). The incidence of duodenitis - 7.83%, peptic ulcer -3.3%, esophageal varices - 1.5%, the incidence of carcinoma esophagus and carcinoma stomach was approximately same that is, 4.5% and 4.6% respectively. The incidence of esophageal candidiasis was 4.16%. The majority of the patients had a normal study that is, 14.5%. Conclusion: Upper GI lesions were more common in males. The incidence of diseases was highest among the elderly age group. Most of the benign mucosal lesions were mainly due to spicy food and habit of tobacco consumption. The incidence of malignancy was mostly among older age group above 50 years. The incidence of the normal study was high owing to increased medical care, easy availability of the procedure and increased medical awareness among patients. In all these cases, upper GI endoscopy not only helped in diagnosing the disease but also helped to get information about pathology, extent of disease and complications that have occurred. This study highlights the importance of diagnostic and therapeutic uses, recording of the various gastroenterological diseases we come across in rural population

    Efficacy of two commercially available dentifrices in reducing dentinal hypersensitivity

    <b>Objective:</b> A parallel design clinical study evaluated reduction in hypersensitivity after brushing for 12 weeks with Anchor toothpaste (containing potassium citrate, zinc citrate, triclosan and sodium monofluorophosphate) (test) and Colgate Total (sodium fluoride, silica, triclosan and copolymer) (control) dentifrices. <b>Materials and Methods:</b> Sixty adults with sensitivity to hot and cold stimulus in at least two tooth surfaces were stratified at the baseline examination by tactile, hot and cold stimuli scores in two balanced groups. Subjects were randomly allocated the test and control dentifrices and evaluated after 6 and 12 weeks of dentifrice use for hypersensitivity. <b>Results:</b> The two teeth that were selected in each patient were designated as two different sets. The 12 <sup>th</sup> -week scores as compared to baseline scores for tactile, heat and cold tests in the test group showed a reduction in tooth hypersensitivity by 36.67&#x0025; (<i>P</i> &lt; 0.01), 20.35&#x0025; (</i>P</i> &lt; 0.01) and 53.64 &#x0025; (<i>P</i> &lt; 0.01), respectively, in the first set of teeth and 43.75&#x0025; (<i>P</i> &lt; 0.01), 24.48&#x0025; (<i>P</i> &lt; 0.01) and 59.78&#x0025; (<i>P</i> &lt; 0.01), respectively, in the second set of teeth. The 12 <sup>th</sup> -week scores as compared to baseline scores for tactile, heat and cold tests in the control group showed a reduction in tooth hypersensitivity by 42.86&#x0025; (<i>P</i> &lt; 0.01), 13.02&#x0025; (<i>P</i> &lt; 0.01) and 45.14&#x0025; (<i>P</i> &lt; 0.01), respectively, in the first set of teeth and 40&#x0025; (<i>P</i> &lt; 0.01), 16.59&#x0025; (<i>P</i> &lt; 0.01) and 44.16&#x0025; (<i>P</i> &lt; 0.01), respectively, in the second set of teeth. <b>Conclusions:</b> Both the products reduced dentinal hypersensitivity in the study subjects at the end of the 12-week period. However, there was no statistically significant difference in reduction in hypersensitivity between the two products
