Persistent Depressive Disorder: the Clinical Approach of the Patient Associating Depression and Dental Pathology - Case Report and Clinical Considerations

Growing research literature has documented an increased association between depression and dental problems. Patients with severe dental problems suffer from psychosocial consequences, distress and psychiatric problems.AIM: To emphasize the relationship between resistant depression and poor dental health.METHOD:  This case report demonstrates the association between resistant depression and poor dental health in a 46-year-old female patient. Scores on clinical assessment measures suggested clinically severe levels of anxiety, worry, stigmatization, depression, sleeping and eating disorders and decreased satisfaction in quality of life at the beginning of the intervention. The theoretical rationale and treatment implications are presented.RESULTS: The scores on all these measures improved at the end of the dental interventions and no intense remaining depressive symptoms were reported afterward. Increased scores on life satisfaction and quality of life were documented as well. This case illustrates the potential benefit of dental treatment associating psychiatric treatment. A definitively positive association exists between poor dental health and depression. Once simultaneous treatment is initiated, there are chances for patients to have a positive evolution and social reinsertion.CONCLUSION: The high occurrence of depression, anxiety and stress among patients with dental problems highlights the importance of providing support programs and implementing preventive measures to anticipate and help persons with this type of pathology, particularly those who are most susceptible to higher levels of these psychological conditions.</p

    PERIODONTAL CONSIDERATIONS IN FIXED PROSTHESES

Restorative dentistry is by far the most important factor for plaque accumulation. Purpose The study aimed at assessing the periodontal condition relating to the existence of iatrogenic fixed prosthetic therapy. Material and methods. The studied sample consisted of 47 patients, including 23 men and 24 women aged between 18 and 65 years. We assessed the existence of periodontal damage related to local restorative iatrogenic factors. Results and discussion. In the examined patients were noted numerous discrepancies both in terms of margins placement relative to the gingival sulcus related to abutment teeth preparation. Vertical and horizontal discrepancies, occurring in marginal adaptation of restorations may be the reasons for periodontal irritation. Therefore, it is necessary to avoid the apical extension of the margins by unnecessary sinking of the margins of the preparation, which will lead to alteration of the epithelial insertion of at the level of the cemento-enamel junction Important in this respect is the thickness of the crowns margins. Conclusions. The restorative treatment objective is long-term maintenance of the masticatory system in good health condition. Dental restorations and periodontal health are closely linked-periodontal health is necessary for proper operation of all restorations, while functional stimulation provided by dental restorations is essential in periodontal health

Aim of study: this report describes a patient with severe anterior mandibular dental crowding determinated by mandibular retrusion and hypercontraction of mentalis muscle. Material and methods: A 32-year-old female patient requested an orthodontic clinical examination in November 2019 having concerns about the possible loss of mandibular incisors with periodontal recessions. Results: The patient refused orthognathic surgery being treated with fixed orthodontic appliances after dental extractions necessary for dental alignment and bite correction, under the supervision of the attending periodontist and dentist. Conclusions: Even if the retrognathic mandibular position was not corrected by orthognathic surgery, the patient was very satisfied with the results of the orthodontic treatment, which improved her smile, dental and facial appearance and gave self-confidence

The purpose of this paper is to identify and analyze the shortcomings of achieving gnatho prosthetic devices and their detrimental effect on periodontal health, and the periodontal protection methods employed in the technological algorithm applied for fixed gnatho prosthetic devices. Material and methods To achieve periodontal prophylaxis, we analyzed the compliance in clinical and technological process of a series of technical requirements for rebuilding morpho- functional integrity of the dental arch by bridges. Results To achieve periodontal prophylaxis should be pursued during clinical and technological procedures the following: respect of coronary morphology, restoring the contour and position of contact areas, inter proximal embrasures restoration, restoration of the physiological contours of the buccal and lingual surfaces, the correct reproduction of natural cemento-enamel junction on mesial and distal faces, respect of gingival sulcus area, achieving perfect adaptation of the margins of restorations, periodontal protection in sub-sulcular area, achieving the profile of emergence. Conclusions By respecting these the principles, are ensured both restoration longevity and periodontal health

Study on the Influence of Regular Physical Activity on Children’s Oral Health

The benefits of physical activities conducted systematically on the harmonious development, intellectual performance, and general health of children are unanimously accepted. This study’s aim is to determine whether differences in oral health between young athletes and children not engaged in competitive sports are present. A total of 173 children aged between 6 and 17 years, 58 hockey players, 55 football players, and 60 in the control group were divided into groups according to their activity, age, and biological sex and examined for oral hygiene and dental and periodontal health, using clinically determined indices. Statistical analysis showed significant differences between the groups, with lower (better) values for athletes, regardless of age, sex, or activity. Oral hygiene showed the most relevant differences for males aged 14 to 17, as did the index for dental health. Periodontal health, on the other hand, was significantly better for females aged 6 to 13. Based on this data, the beneficial influence of regular physical activity also has an impact on oral health. Identifying the mechanisms behind this needs to be explored in depth and may be a topic for further research
