    Using Small-Area Analysis to Estimate County-Level Racial Disparities in Obesity Demonstrating the Necessity of Targeted Interventions

    Data on the national and state levels is often used to inform policy decisions and strategies designed to reduce racial disparities in obesity. Obesity-related health outcomes are realized on the individual level, and policies based on state and national-level data may be inappropriate due to the variations in health outcomes within and between states. To examine county-level variation of obesity within states, we use a small-area analysis technique to fill the void for county-level obesity data by race. Five years of Behavioral Risk Factor Surveillance System data are used to estimate the prevalence of obesity by county, both overall and race-stratified. A modified weighting system is used based on demographics at the county level using 2010 census data. We fit a multilevel reweighted regression model to obtain county-level prevalence estimates by race. We compare the distribution of prevalence estimates of non-Hispanic Blacks to non-Hispanic Whites. For 25 of the 26 states included in our analysis there is a statistically significant difference between within-state county-level average obesity prevalence rates for non-Hispanic Whites and non-Hispanic Blacks. This study provides information needed to target disparities interventions and resources to the local areas with greatest need; it also identifies the necessity of doing so

    Walnut Allergy Across Europe: Distribution of Allergen Sensitization Patterns and Prediction of Severity

    Background: Walnut allergy is common across the globe, but data on the involvement of individual walnut components are scarce. Objectives: To identify geographical differences in walnut component sensitization across Europe, explore cosensitization and cross-reactivity, and assess associations of clinical and serological determinants with severity of walnut allergy. Methods: As part of the EuroPrevall outpatient surveys in 12 European cities, standardized clinical evaluation was conducted in 531 individuals reporting symptoms to walnut, with sensitization to all known walnut components assessed in 202 subjects. Multivariable Lasso regression was applied to investigate predictors for walnut allergy severity. Results: Birch-pollen-related walnut sensitization (Jug r 5) dominated in Northern and Central Europe and lipid transfer protein sensitization (Jug r 3) in Southern Europe. Profilin sensitization (Jug r 7) was prominent throughout Europe. Sensitization to storage proteins (Jug r 1, 2, 4, and 6) was detected in up to 10% of subjects. The walnut components that showed strong correlations with pollen and other foods differed between centers. The combination of determinants best predicting walnut allergy severity were symptoms upon skin contact with walnut, atopic dermatitis (ever), family history of atopic disease, mugwort pollen allergy, sensitization to cat or dog, positive skin prick test result to walnut, and IgE to Jug r 1, 5, 7, or carbohydrate determinants (area under the curve = 0.81; 95% CI, 0.73-0.89). Conclusions: Walnut-allergic subjects across Europe show clear geographical differences in walnut component sensitization and cosensitization patterns.     Dementias Platform UK (DPUK) Data Portal - supporting multi-modal data analysis, data linkage and real-world outcomes

    DPUK relaunched the Data Portal in November 2017 to present openly available information on the data availability and technical capability of the Data Portal, which supports multi-modal research studies with various objectives from disease model validation to observation investigation. DPUK not only brings clinical data together from cohorts, but is now supporting multi-modal studies in genetics and imaging, as well as linkage opportunities to routine data using world-leading technical solutions to data sharing. The capacity, adaptability and sophistication of the UK Secure eResearch Platform which the Portal is housed on, allows for unprecedented levels of centralised access to rich cohort and routine data, which is consequentially leading to international collaboration and development ambition within epidemiology, bioinformatics, research methodology and technical research solutions. As of March 2018, DPUK is supporting 50 cohorts, 41 from the UK and 9 from across the rest of the world, alongside furthering links and access to routine data held in the UK and across the world. 20 research studies are underway, and the DPUK mission to enhance data science within dementia research is leading the conversation for developing a community of excellence in this field and across other research genres

    Network analysis of differential Ras isoform mutation effects on intestinal epithelial responses to TNF-α

    Tumor necrosis factor alpha (TNF-α) is an inflammatory cytokine that can elicit distinct cellular behaviors under different molecular contexts. Mitogen activated protein kinase (MAPK) pathways, especially the extracellular signal-regulated kinase (Erk) pathway, help to integrate influences from the environmental context, and therefore modulate the phenotypic effect of TNF-α exposure. To test how variations in flux through the Erk pathway modulate TNF-α-elicited phenotypes in a complex physiological environment, we exposed mice with different Ras mutations (K-Ras activation, N-Ras activation, and N-Ras ablation) to TNF-α and observed phenotypic and signaling changes in the intestinal epithelium. Hyperactivation of Mek1, an Erk kinase, was observed in the intestine of mice with K-Ras activation and, surprisingly, in N-Ras null mice. Nevertheless, these similar Mek1 outputs did not give rise to the same phenotype, as N-Ras null intestine was hypersensitive to TNF-α-induced intestinal cell death while K-Ras mutant intestine was not. A systems biology approach applied to sample the network state revealed that the signaling contexts presented by these two Ras isoform mutations were different. Consistent with our experimental data, N-Ras ablation induced a signaling network state that was mathematically predicted to be pro-death, while K-Ras activation did not. Further modeling by constrained Fuzzy Logic (cFL) revealed that N-Ras and K-Ras activate the signaling network with different downstream distributions and dynamics, with N-Ras effects being more transient and diverted more towards PI3K-Akt signaling and K-Ras effects being more sustained and broadly activating many pathways. Our study highlights the necessity to consider both environmental and genomic contexts of signaling pathway activation in dictating phenotypic responses, and demonstrates how modeling can provide insight into complex in vivo biological mechanisms, such as the complex interplay between K-Ras and N-Ras in their downstream effects.National Institute of General Medical Sciences (U.S.) (Grant R01-GM088827)National Cancer Institute (U.S.) (U54-CA112967)United States.     Short-term health and social impacts of energy-efficiency investments in low-income communities: a controlled field study

    Background During 2012–15, £45 million was invested to improve the energy-efficiency of 4800 houses in low-income areas across Wales. Houses received measures such as external wall insulation, new windows and doors, upgrades to the heating system, and connection to the gas network. This study aimed to establish the short-term health and social impacts of these investments. Methods A quasi-experimental field study with a controlled, before and after design was conducted (364 individuals in improved houses [intervention], 418 in houses with no improvements [control]). Any adult living in 24 selected intervention areas and matched control areas (n=23) was eligible for inclusion. Self-completed questionnaires, administered via a drop-off-and-collect method, were collected in the winter months (December to February) before and after installation of the energy efficiency measures. Health outcomes were mental health composite scale (MCS) and physical health composite scale (PCS) scores of the SF-12v2, SF-6D utility scores derived from the SF-12v2, self-reported respiratory symptoms, and subjective wellbeing. Social outcomes were financial difficulties and stress, food security, thermal comfort, housing conditions, and social isolation. The study used measures validated in previous research. Linear, ordered multinomial, and logistic multilevel models were constructed with measurement occasions nested within individuals. Findings After controlling for sex, age, housing benefit, household income, and smoking status, we found that investments were not associated with improvements in MCS (B=0·00, 95% CI −1·60 to 1·60) or PCS (0·98, −0·34 to 2·28) scores, SF-6D utilities (−0·01, −0·04 to 0·02), or self-reported respiratory symptoms (−0·14, −0·54 to 0·26). However, people who received energy-efficiency measures reported improved subjective wellbeing compared with controls (B=0·38, 95% CI 0·12 to 0·65), and fewer financial difficulties (−0·15, −0·25 to −0·05); they reported higher thermal comfort (odds ratio 3·83, 95% CI 2·40 to 5·90), higher satisfaction with the improvement of their homes (3·87, 2·51 to 5·96), and less reluctance to invite friends or family to their homes (0·32, 0·13 to 0·77). Interpretation Although there is no evidence that energy-efficiency investments provide physical health benefits in the short term, they improve social and economic conditions that are conducive to better health. Longer term studies are needed to establish the health impacts of energy-efficiency investments

    Class solutions for SABR-VMAT for high-risk prostate cancer with and without elective nodal irradiation

    BACKGROUND: The purpose of this study is to find the optimal planning settings for prostate SABR-VMAT for high-risk prostate cancer patients irradiated to prostate only (PO) or prostate and pelvic lymph nodes (PPLN). METHODS: For 10 patients, plans using 6MV flattened, flattening-filter-free (FFF) 6MV (6 F) and FFF 10MV (10 F) photon beams with full and partial arc arrangements were generated and compared. The prescribed dose was 40Gy to the prostate with 25Gy to the PLN in 5 fractions. Plans were then evaluated for PTV coverage, dose fall-off, and OAR doses. The number of monitor units and the treatment delivery times were also compared. Statistical differences were evaluated using a paired sample Wilcoxon signed rank test with a significance level of 0.05%. RESULTS: A total of 150 plans were generated for this study. Acceptable PO plans were obtained using single arcs, while two arcs were necessary for PPLN. All plans were highly conformal (CI ≥1.3 and CN ≥0.90) with no significant differences in the PTV dose coverage. 6MV plans required significantly longer treatment time and had higher dose spillage compared to FFF plans. Superior plans were obtained using 10 F 300° partial arcs for PO with the lowest rectal dose, dose spillage and the shortest treatment times. For PPLN, 6 F and 10 F plans were equivalent. CONCLUSIONS: SABR-VMAT with FFF photon beams offers a clear benefit with respect to shorter treatment delivery times and reduced dose spillage. Class solutions using a single 10 F 300° arc for PO and two 10 F or 6 F partial 300° arcs for PPLN are proposed.
