    Drug Delivery to the Posterior Segment of the Eye: Biopharmaceutic and Pharmacokinetic Considerations

    The treatment of the posterior-segment ocular diseases, such as age-related eye diseases (AMD) or diabetic retinopathy (DR), present a challenge for ophthalmologists due to the complex anatomy and physiology of the eye. This specialized organ is composed of various static and dynamic barriers that restrict drug delivery into the target site of action. Despite numerous efforts, effective intraocular drug delivery remains unresolved and, therefore, it is highly desirable to improve the current treatments of diseases affecting the posterior cavity. This review article gives an overview of pharmacokinetic and biopharmaceutics aspects for the most commonly-used ocular administration routes (intravitreal, topical, systemic, and periocular), including information of the absorption, distribution, and elimination, as well as the benefits and limitations of each one. This article also encompasses different conventional and novel drug delivery systems designed and developed to improve drug pharmacokinetics intended for the posterior ocular segment treatment

    Nos Ășltimos anos tem aumentado progressivamente o nĂșmero de famĂ­lias que se tem confrontado com a situação de cuidar de familiares idosos em condição de dependĂȘncia, principalmente quando esta Ă© provocada em virtude da Doença de Alzheimer (DA). O objetivo deste estudo Ă© o de investigar as repercussĂ”es do cuidado na vida do cuidador familiar de idoso com demĂȘncia de Alzheimer. AtravĂ©s da epistemologia qualitativa utilizou-se a tĂ©cnica da entrevista na modalidade de histĂłria de vida temĂĄtica aplicada a vinte cuidadores de idosos cadastrados na Associação Brasileira de Alzheimer do estado do Amazonas (ABRAZ-AM). Os resultados mostraram que as renĂșncias aparecem nos depoimentos dos familiares como uma repercussĂŁo significativa do cuidado. A privação social em função do cuidado Ă© uma realidade presente na vida dos cuidadores deste estudo. A raiva, a impaciĂȘncia, o medo, a solidĂŁo e a vergonha sĂŁo sentimentos vivenciados pelos cuidadores em seu cotidiano, que se alternam com sentimentos de satisfação, felicidade, compaixĂŁo e de retribuição. É importante salientar que neste estudo, a situação de cuidado vivenciada pelos cuidadores Ă© permeada por sentimentos que se contrapĂ”em, em determinados momentos se configurando como desagradĂĄveis e em outros momentos como agradĂĄveis. Sendo assim, percebe-se que Ă© possĂ­vel e relevante os profissionais da saĂșde identificarem esses aspectos favorecedores de prazer na relação de cuidado, buscando exaltĂĄ-los perante os outros. Dessa forma, a possibilidade de enfrentar da melhor forma os percalços deve ser considerada uma realidade eminentemente presente no cotidiano de cuidado.In recent years there has been a progressive increase in the number of families confronted with the situation of caring for elderly relatives in a dependent condition, principally as a result of Alzheimer's disease. The objective of the present study was to investigate the repercussions of such care in the life of family caregivers of elderly relatives with Alzheimer's disease. By means of qualitative epistemology, the interview technique was used in the thematic life history modality, and applied to 20 caregivers of elderly individuals enrolled in the Brazilian Association of Alzheimer's Disease in the State of Amazonas, Northern Brazil. The results showed that renouncements emerge in the caregivers' testimonies as a significant repercussion of care. Social deprivation is a reality that is present in the lives of all the caregivers who took part in this study. Anger, impatience, fear, loneliness and shame are feelings experienced by the caregivers in their everyday lives, intermingling with feelings of satisfaction, happiness, compassion and retribution. It is important to highlight that in the present study the care situation lived by the caregivers was permeated by opposing feelings, being considered unpleasant in certain moments and pleasant in others. Thus, it is perceived that it is possible and relevant for the health professionals to identify the pleasure-promoting aspects in the care relationship, praising them before the others. Therefore, the possibility of facing the mishaps in the best possible way should be considered as an eminently present reality in the daily routine of care

    Garbage in, garbage out: how reliable training data improved a virtual screening approach against SARS-CoV-2 MPro

    Introduction: The identification of chemical compounds that interfere with SARS-CoV-2 replication continues to be a priority in several academic and pharmaceutical laboratories. Computational tools and approaches have the power to integrate, process and analyze multiple data in a short time. However, these initiatives may yield unrealistic results if the applied models are not inferred from reliable data and the resulting predictions are not confirmed by experimental evidence.Methods: We undertook a drug discovery campaign against the essential major protease (MPro) from SARS-CoV-2, which relied on an in silico search strategy –performed in a large and diverse chemolibrary– complemented by experimental validation. The computational method comprises a recently reported ligand-based approach developed upon refinement/learning cycles, and structure-based approximations. Search models were applied to both retrospective (in silico) and prospective (experimentally confirmed) screening.Results: The first generation of ligand-based models were fed by data, which to a great extent, had not been published in peer-reviewed articles. The first screening campaign performed with 188 compounds (46 in silico hits and 100 analogues, and 40 unrelated compounds: flavonols and pyrazoles) yielded three hits against MPro (IC50 ≀ 25 ÎŒM): two analogues of in silico hits (one glycoside and one benzo-thiazol) and one flavonol. A second generation of ligand-based models was developed based on this negative information and newly published peer-reviewed data for MPro inhibitors. This led to 43 new hit candidates belonging to different chemical families. From 45 compounds (28 in silico hits and 17 related analogues) tested in the second screening campaign, eight inhibited MPro with IC50 = 0.12–20 ÎŒM and five of them also impaired the proliferation of SARS-CoV-2 in Vero cells (EC50 7–45 ÎŒM).Discussion: Our study provides an example of a virtuous loop between computational and experimental approaches applied to target-focused drug discovery against a major and global pathogen, reaffirming the well-known “garbage in, garbage out” machine learning principle

    The purpose of this study was to analyze the potential of various types of biodegradable microspheres (MS) (i) to activate in vitro cell line-derived macrophages (RAW 264.7, Mphi), and primary peritoneal and bone marrow-derived mouse Mphi, to prolong the release and presentation of microencapsulated synthetic malaria antigens by Mphi after uptake of antigen-loaded MS, and (ii) to stimulate an immune response in mice against a microencapsulated synthetic malaria antigen. The MS were made of various types of poly(lactide-co-glycolide) (PLGA) or chitosan cross-linked with tripolyphosphate. PLGA, but not chitosan MS, were efficiently ingested by Mphi. Upon exposure to the various MS types, Mphi increased only the production of reactive oxygen intermediates (ROI), while the production of nitric oxide (NO), tumor necrosis factor alpha (TNF-alpha), and the expression of cyclooxigenase-2 (COX-2), inducible NO synthase (iNOS), the cell surface markers MHC class I and II, and CD 86 remained unaffected. In vitro release of the microencapsulated antigen from PLGA50:50 MS followed a pulsatile pattern and extended over 14 weeks. This prolonged antigen release was also mirrored in the significantly prolonged antigen presentation over more than 7 days by Mphi after uptake of antigen-loaded PLGA MS. Finally, antigen-loaded PLGA MS induced a solid immune response in mice after a single s.c.-injection, which was only slightly inferior to the antibody titers measured with the control formulation with Montanide ISA720. These results suggest that MS are well tolerated by Mphi. The prolonged antigen presentation by Mphi, as measured in vitro, along with the capacity to induce a strong immune response in animals emphasize that biodegradable MS are a very promising delivery system for both preventive and immunotherapeutic vaccines

    Blood Pressure in relation to 24-Hour Urinary Sodium and Potassium Excretion in a Uruguayan Population Sample

    Many public health policies in Latin America target an optimized sodium and potassium intake. The aims of this study were to assess the sodium and potassium intake using 24-hour urinary analysis and to study their association with blood pressure in a Uruguayan population cohort using cluster analysis. A total of 149 participants (aged 20–85 years) were included in the study, and office blood pressure, anthropometric measurements, biochemical parameters in the blood, and 24-hour urine samples were obtained. The overall mean sodium and potassium excretion was 152.9 ± 57.3 mmol/day (8.9 ± 3.4 g/day of salt) and 55.4 ± 19.6 mmol/day, respectively. The average office systolic/diastolic blood pressure was 124.6 ± 16.7/79.3 ± 9.9 mmHg. Three compact spherical clusters were defined in untreated participants based on predetermined attributes, including blood pressure, age, and sodium and potassium excretion. The major characteristics of the three clusters were (1) high systolic blood pressure and moderate sodium excretion, (2) moderate systolic blood pressure and very high sodium excretion, and (3) low systolic blood pressure and low sodium excretion. Participants in cluster three had systolic blood pressure values that were 23.9 mmHg (95% confidence interval: −29.5 to −1.84) lower than those in cluster one. Participants in cluster two had blood pressure levels similar to those in cluster one (P = 0.32) and worse metabolic profiles than those in cluster one and three (P 0.47). An effect of sodium and potassium intake on blood pressure levels was not found at the population level using regression or cluster analysis

    Humoral immune response against epidermal growth factor encapsulated in dehydration rehydration vesicles of different phospholipid composition

    We investigated the influence of dehydration-rehydration vesicles (DRV) phospholipid composition and the addition of other components on human recombinant epidermal growth factor (hrEGF) encapsulation efficiency and its release from liposomes. Encapsulation of EGF into DRV composed of phosphatidylcholine with different unsaturation levels was around 20-35%. The best result was obtained with dipalmitoyl phosphatidylcholine: cholesterol (DPPC:Ch) liposomes (35%) corresponding to the lowest hrEGF release during one month of storage. Even with this phospholipid composition, modification of the DRV procedure by including an extrusion step did not improve hrEGF encapsulation efficiency, rendering less stable particles. The inclusion of recombinant P64k from Neisseria meningitidis (rP64k), as such or conjugated to hrEGF, decreased the encapsulation efficiency of the latter protein into DRV or freeze and thaw multilamellar vesicles (FATMLV). The hrEGF release from liposomes could be related to the interaction between this polypeptide and the bilayer, as evidenced by increased carboxyfluorescein release from hrEGF-DRV; less susceptibility to fluorescence quenching by acrylamide in the presence of liposomes; and a measurable decrease of phospholipid phase transition Delta enthalpy (Delta H). DRV comprising saturated phospholipids (DPPC:Ch or distearoyl phosphatidylcholine [DSPC]:Ch) and containing the conjugate EGF-P64k induced a more efficient immune response against hrEGF than unsaturated phospholipid and alum in terms of total IgG, IgG(2a), and IgG(2b) subclasses and the ability of antibody to inhibit the interaction of the EGF receptor with hrEGF
