La biblioteca digital revisitada
This article presents some standards and initiatives on digital libraries written or updated since 2001, when Ernesto García Camarero and the author of this article published the book The digital library. Briefly examines the latest trends in digital publishing and digital industry and reviews the concepts of digital collection, digital library and repository. The basic ideas of the open access movement and the OAI-PMH protocol (which is considered the most appropriate mechanism to establish cooperative digital library projects and to facilitate the retrieval of electronic objects) are described. After describing the two major Spanish collectors (Recolecta and Hispana) certain standards for the description of objects are set: Dublin Core, MARCXML, MODS, MADS, the organization of descriptive metadata, administrative and structural and information transfer (METS), preservation of digital documents (PREMIS) and for consultation and information retrieval (SRU)

Tocilizumab in giant cell arteritis. Observational, open-label multicenter study of 134 patients in clinical practice
OBJECTIVE:
Tocilizumab (TCZ) has shown efficacy in clinical trials on giant cell arteritis (GCA). Real-world data are scarce. Our objective was to assess efficacy and safety of TCZ in unselected patients with GCA in clinical practice Methods: Observational, open-label multicenter study from 40 national referral centers of GCA patients treated with TCZ due to inefficacy or adverse events of previous therapy. Outcomes variables were improvement of clinical features, acute phase reactants, glucocorticoid-sparing effect, prolonged remission and relapses. A comparative study was performed: (a) TCZ route (SC vs. IV); (b) GCA duration (?6 vs. >6 months); (c) serious infections (with or without); (d) ?15 vs. >15 mg/day at TCZ onset.
RESULTS:
134 patients; mean age, 73.0 ± 8.8 years. TCZ was started after a median [IQR] time from GCA diagnosis of 13.5 [5.0-33.5] months. Ninety-eight (73.1%) patients had received immunosuppressive agents. After 1 month of TCZ 93.9% experienced clinical improvement. Reduction of CRP from 1.7 [0.4-3.2] to 0.11 [0.05-0.5] mg/dL (p < 0.0001), ESR from 33 [14.5-61] to 6 [2-12] mm/1st hour (p < 0.0001) and decrease in patients with anemia from 16.4% to 3.8% (p < 0.0001) were observed. Regardless of administration route or disease duration, clinical improvement leading to remission at 6, 12, 18, 24 months was observed in 55.5%, 70.4%, 69.2% and 90% of patients. Most relevant adverse side-effect was serious infections (10.6/100 patients-year), associated with higher doses of prednisone during the first three months of therapy.
CONCLUSION:
In clinical practice, TCZ yields a rapid and maintained improvement of refractory GCA. Serious infections appear to be higher than in clinical trials

Dendritic cell deficiencies persist seven months after SARS-CoV-2 infection
Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV)-2 infection induces an exacerbated inflammation driven by innate immunity components. Dendritic cells (DCs) play a key role in the defense against viral infections, for instance plasmacytoid DCs (pDCs), have the capacity to produce vast amounts of interferon-alpha (IFN-α). In COVID-19 there is a deficit in DC numbers and IFN-α production, which has been associated with disease severity. In this work, we described that in addition to the DC deficiency, several DC activation and homing markers were altered in acute COVID-19 patients, which were associated with multiple inflammatory markers. Remarkably, previously hospitalized and nonhospitalized patients remained with decreased numbers of CD1c+ myeloid DCs and pDCs seven months after SARS-CoV-2 infection. Moreover, the expression of DC markers such as CD86 and CD4 were only restored in previously nonhospitalized patients, while no restoration of integrin β7 and indoleamine 2,3-dyoxigenase (IDO) levels were observed. These findings contribute to a better understanding of the immunological sequelae of COVID-19
Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries
Abstract
Background
Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres.
Methods
This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries.
Results
In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia.
Conclusion
This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
La recopilación y conservación del patrimonio digital
As technologies have developed information and communications technology (ICT), digital documents have been created directly in digital format or derived from the digitization of analog library materials that are distributed as tangible media or through computer networks and telecommunications. Its volume and cultural and scientific significance can not be ignored any longer, since they provide a vital heritage for training and information for individuals and societies. The European Union, UNESCO and IFLA, among other organizations, as well as recognize and promote actions for identification, description, organization and preservation
Hacia una Ley de Depósito Legal : aspectos técnicos bibliotecarios a tener en cuenta : Grupo De Trabajo Sobre Depósito Legal Coordinador: Luis Ángel García Melero
One of the agreements of the first conference on Library Cooperation. Ministry of Education and Culture and the Autonomous Communities, held in the Castillo de la Mota (Medina del Campo, Valladolid Spain) in October 1997 was to set up a Working Group which, amongst other functions, to produce a document that incorporates the technical aspects librarians to consider in a Legal Deposit Act and defining the concepts and terms therein expressed. This is a proposal for the library sector of some technical aspects which should consider a future legal deposit law and not a preliminary draft la
