    Endosteal biologic augmentation for surgical fixation of displaced femoral neck fractures

    Objectives: To report outcomes of a cohort with displaced Femoral Neck Fractures (FNF) treated with a length/angle-stable construct augmented with an endosteal fibular allograft serving as a biologic dowel. Design: Prospective Setting: Level I Trauma Center Patients: The study group consists of twenty-seven patients with isolated FNF surgically treated by a single surgeon. Intervention: Open reduction of the femoral neck, fixed with a length- and angle stable-construct of two fully threated cannulated screws augmented with an endosteal fibular allograft serving as a biologic dowel. Main Outcome Measurements: Clinical and radiographic outcomes of the fixation construct and the viability of both the femoral head and the fibular allograft, host response to the allograft and osseous union was evaluated using a specialized sequence of contrast-enhanced MRI obtained at 3 and 12 months postoperatively. Results: This construct resulted in high union rates (89%; 24/27). Two patients suffered early catastrophic failure and one patient developed fracture non-union, all of wish underwent uneventful conversion to total hip arthroplasty. Three (11%) additional patients had removal of symptomatic implants. The clinical and radiographic outcomes were excellent. Twelve months MRIs revealed either partial or complete osseous incorporation of 86% the fibular allografts without signs of adverse reaction of the host to the allograft. Femoral head osteonecrosis segments were noted in 76% of patients on MRI, however radiographically there were no sign of osteonecrosis or segmental collapse. Conclusion: The fibular allograft reconstructs the comminuted femoral neck, and the osteointegration overtime increases the strength of the host-bone-graft interface. This added strength seems to provide the stability needed to better preserve the intra-operative reduction, obtain good outcomes and reduce the complications associated with FNF.     Evaluation and Management of Proximal Humerus Fractures

    Proximal humerus fractures are common injuries, especially among older osteoporotic women. Restoration of function requires a thorough understanding of the neurovascular, musculotendinous, and bony anatomy. This paper addresses the relevant anatomy and highlights various management options, including indication for arthroplasty. In the vast majority of cases, proximal humerus fractures may be treated nonoperatively. In the case of displaced fractures, when surgical intervention may be pursued, numerous constructs have been investigated. Of these, the proximal humerus locking plate is the most widely used. Arthroplasty is generally reserved for comminuted 4-part fractures, head-split fractures, or fractures with significant underlying arthritic changes. Reverse total shoulder arthroplasty is reserved for patients with a deficient rotator cuff, or highly comminuted tuberosities

    TEFTOM: A Promising General Trauma Expectation/Outcome Measure—Results of a Validation Study on Pan-American Ankle and Distal Tibia Trauma Patients

    Background. In orthopedics, there is no instrument specifically designed to assess patients’ expectations of their final surgery outcome in general trauma populations. We developed the Trauma Expectation Factor Trauma Outcome Measure (TEFTOM) to investigate the fulfilment of patients’ expectations one year after surgery as a measure of general trauma surgical outcomes. The aim of this paper was to assess the psychometric characteristics of this new general trauma outcome measure. Methods. The questionnaire was tested in 201 ankle and distal tibia fracture patients scheduled for surgery. Patients were followed up for twelve months. The TEFTOM questionnaire was evaluated for its criterion validity, internal consistency, reproducibility, and responsiveness. Results. TOM showed good criterion validity against the American Academy of Orthopaedic Surgeons Foot and Ankle Scale (Pearson’s correlation coefficient = 0.69–0.77). Internal consistency was acceptable for TEF (Cronbach’s alpha = 0.65–0.76) and excellent for TOM (Cronbach’s alpha = 0.76–0.85). Reproducibility was moderate to very good (intraclass coefficient correlation (ICC) ) for TEF and very good (ICC ) for TOM. TOM also proved to be responsive to changes in patients’ condition over time (Wald test; ). Conclusions. TEFTOM is a promising tool for measuring general trauma outcomes in terms of patients’ expectation fulfilment that proved to be valid, internally consistent, reproducible, and responsive to change.Peer Reviewe
