    Darier Disease: First Molecular Study of a Portuguese Family

    Background: Darier disease (DD) is a rare autosomal dominant condition characterized by skin lesions. Additionally, a wide range of neuropsychiatric symptoms is frequently reported in DD patients. This genodermatosis relies on mutations in the ATPase sarcoplasmic/endoplasmic reticulum Ca2+ transporting 2 (ATP2A2) gene, which encodes an ATPase responsible for pumping Ca2+ from the cytosol to the lumen of the ER. Objective: Herein we studied the molecular aspect of a two-generation Portuguese family with DD history with clinical variability. Methods: All exons and intron-exon borders of genomic ATP2A2, as well as coding ATP2A2, were sequenced. Relative levels of SERCA2 mRNA and protein were quantified by qPCR and western blotting, respectively. Results: The c.1287+1G > T variant was identified in all affected individuals, whereas the unaffected individual was shown to carry the wild-type ATP2A2 sequence in both alleles. This variant leads to the skipping of full exon 10, which consequently generates a frameshift originating a premature STOP codon in exon 11 (p.V395 = fs*19). Although the mutant mRNA seems to partially escape degradation, results suggest synthesis inhibition or immediate degradation of the mutant protein. Neuropsychiatric and other occurrences affecting certain patients are also reported. Conclusion: This is the first study of DD in Portugal, the variant identified, previously described in a single Japanese patient, may be considered a pathogenic mutation, and haploinsufficiency the mechanism underlying DD pathology in these patients.     Pneumocystis pneumonia (PcP) is a major cause of mortality and morbidity in immunocompromised patients. There are limited alternative therapeutic choices to trimethoprim-sulfamethoxazole (TMP-SMX) which is the standard first line therapy/prophylaxis for PcP. The efficacy of low doses of caspofungin and caspofungin in association with TMP-SMX standard-prophylactic dose was evaluated in an experimental model of Pneumocystis. Susceptibility of Pneumocystis spp. to low doses of caspofungin and caspofungin/TMP-SMX was evaluated in Balb/c immunosuppressed mice, infected intranasally with P. murina. Caspofungin was administered once daily at 0.1 mg/kg, 0.05 mg/kg, and 0.001 mg/kg and TMP-SMX was administered by oral gavage (12.25 mg/62.5 mg/day), for 21 days. Efficacy was calculated based on the reduction in organism burden determined through quantitative fluorescent-based real-time PCR (qPCR). Serum β-1,3-D-glucan was measured as an additional marker of infection. The present data showed that caspofungin demonstrated anti-Pneumomocystis effect. However, the doses administrated were too low to achieve Pneumocystis eradication, which suggests that echinocandin treatment should not be administrated as mono-therapy. After 21 days of treatment, P. murina was not detected in the lungs of mice with either TMP-SMX or caspofungin/TMP-SMX. The results showed that, even at the lowest concentrations tested, the efficacy of caspofungin in association with TMP-SMX was higher than the efficacy of either drug used alone. The administration of caspofungin/TMP-SMX was at least 1.4 times more effective against P. murina infection than TMP-SMX used alone. The most promising result was achieved with the combination of caspofungin 0.05 mg/kg/day with TMP-SMX 12.5 mg–62.5 mg/day, which reduced the parasite burden to undetectable levels immediately at the 14th day of treatment, showing a highly marked anti-Pneumomocystis effect. These data suggest that the administration of low doses of caspofungin in combination with low doses of TMP-SMX may provide an improved treatment protocol for Pneumocystis infection clearance.publishersversionpublishe

    Grisel syndrome, acute otitis media, and temporo-mandibular reactive arthritis: a rare association.

    We present a case report of a four-year-old boy with torcicollis and trismus after acute otitis media. Grisel Syndrome diagnosis in association with temporo-mandibular reactive arthritis was admitted, leading to early conservative treatment. GS should be suspected in a child presenting with torticollis after an upper respiratory tract infection or an ENT surgical procedure. The association with temporo-mandibular reactive findings is somehow rarer but not impossible, due to the close vascular communication between retropharyngeal and pterigoid spaces

    A medicina tropical tem vindo a assumir, cada vez mais, uma dimensão global. As patologias referidas como tropicais e restritas, durante muitos anos, a territórios tropicais têm vindo gradualmente a conquistar, cada vez mais, espaço geográfico em áreas anteriormente consideradas isentas destes flagelos. Atualmente, a Europa e os EUA debatem-se com surtos epidémicos de infeções por microrganismos patogénicos considerados tropicais e o impacto das doenças transmitidas por vetores na Saúde humana e veterinária encontra-se cada vez mais disseminado. O Instituto de Higiene e Medicina Tropical da Universidade NOVA de Lisboa organizou o 3º Congresso Nacional de Medicina Tropical e o 1º Congresso Lusófono de Doenças Transmitidas por Vetores, nos dias 20 e 21 de abril de 2015, tendo sido a maior parte dedicado às doenças transmitidas por vetores. A iniciativa contou com cerca de 300 participantes (oriundos de países da Lusofonia e ainda de outros centros científicos internacionais), com 57 comunicações orais e 41 posters. Este encontro teve como objetivo criar um espaço para a discussão e apresentação de trabalhos, desenvolvidos a nível nacional e internacional, nomeadamente sobre a prevenção, o controlo e a eliminação, assim como os vários desafios associados ao desenvolvimento de novas metodologias aplicadas ao diagnóstico e tratamento das doenças transmitidas por vetores. Currently, Tropical Medicine emerged as both an important medical specialty and scientific discipline assuming a preponderant global dimension. Until a few years ago, several tropical pathologies, which were exclusively associated with tropical regions, have been gradually expanding to other geographic areas previously considered free of these infections. Vector-borne diseases are included among this group of infections. Actually, Europe and the US struggle with disease outbreaks caused by pathogens primarily associated with tropical regions. In fact, the impact of vector-borne diseases in human and veterinary health is increasingly being disseminated worldwide. The Institute of Hygiene and Tropical Medicine, University NOVA of Lisbon organized the 3rd National Congress of Tropical Medicine and the 1st Congress of Portuguese Speaking Countries on Vector-borne Diseases, on 20th - 21st of April, 2015, dedicated to vector-borne diseases topics. The meeting was attended by about 300 participants from Portugal, Portuguese speaking countries, and other international scientific centers. Overall, 57 oral communications and 41 posters were presented on sessions that promoted great interest and scientific discussion, about the control, elimination, as well as the challenges associated with the development of novel methodologies applied to the diagnosis and treatment of several vector-borne diseases.publishersversionpublishe

    Genetic Algorithms as a Feature Selection Tool in Heart Failure Disease

    A great wealth of information is hidden in clinical datasets, which could be analyzed to support decision-making processes or to better diagnose patients. Feature selection is one of the data pre-processing that selects a set of input features by removing unneeded or irrelevant features. Various algorithms have been used in healthcare to solve such problems involving complex medical data. This paper demonstrates how Genetic Algorithms offer a natural way to solve feature selection amongst data sets, where the fittest individual choice of variables is preserved over different generations. In this paper, a Genetic Algorithm is introduced as a feature selection method and shown to be effective in aiding understanding of such data

    Identification of Giardia lamblia DHHC Proteins and the Role of Protein S-palmitoylation in the Encystation Process

    Protein S-palmitoylation, a hydrophobic post-translational modification, is performed by protein acyltransferases that have a common DHHC Cys-rich domain (DHHC proteins), and provides a regulatory switch for protein membrane association. In this work, we analyzed the presence of DHHC proteins in the protozoa parasite Giardia lamblia and the function of the reversible S-palmitoylation of proteins during parasite differentiation into cyst. Two specific events were observed: encysting cells displayed a larger amount of palmitoylated proteins, and parasites treated with palmitoylation inhibitors produced a reduced number of mature cysts. With bioinformatics tools, we found nine DHHC proteins, potential protein acyltransferases, in the Giardia proteome. These proteins displayed a conserved structure when compared to different organisms and are distributed in different monophyletic clades. Although all Giardia DHHC proteins were found to be present in trophozoites and encysting cells, these proteins showed a different intracellular localization in trophozoites and seemed to be differently involved in the encystation process when they were overexpressed. dhhc transgenic parasites showed a different pattern of cyst wall protein expression and yielded different amounts of mature cysts when they were induced to encyst.     Vinegar production from fruit concentrates: effect on volatile composition and antioxidant activity

    Vinegar stands as a highly appreciated fermented food product due to several functional properties and multiple applications. This work focuses on vinegar production from fruit wines derived from fruit concentrates, to attain a food product with nutritional added value. Four fruit vinegars (orange, mango, cherry and banana), were produced and characterized, with total acidities of 5.3 ± 0.3% for orange, 5.6 ± 0.2% for mango, 4.9 ± 0.4% for cherry and 5.4 ± 0.4% for banana. Acetification showed impact on aroma volatiles, mainly related to oxidative reactions. Minor volatiles associated with varietal aroma were identified, monoterpenic alcohols in orange vinegar, esters in banana vinegar, C13-norisoprenoids in cherry vinegar and lactones in mango vinegar, indicating fruit vinegars differentiated sensory quality. Total antioxidant activity analysis by FRAP, revealed fruit vinegars potential to preserve and deliver fruit functional properties. Antioxidant activity of fruit vinegars, expressed as equivalents of Fe2SO4, was of 11.0 ± 1.67 mmol L1 for orange, 4.8 ± 0.5 mmol L1 for mango, 18.6 ± 2.33 mmol L1 for cherry and 3.7 ± 0.3 mmol L1 for banana.
