    Maternal satisfaction about the counseling in iron supplementation by health personnel

    Introducción. El momento de la consejería respecto a la suplementación es importante, porque permite transmitir mensajes claves para la prevención o tratamiento de la anemia, que posteriormente llegarían a acuerdos en beneficio de la salud del niño. Sin embargo, existe poca información sobre la satisfacción materna durante la consejería. El objetivo del presente estudio fue determinar el nivel de satisfacción de la madre respecto a consejería brindada por personal de salud que brinda la suplementación. Métodos. Se realizó un estudio cuantitativo, descriptivo y transversal con muestreo no probabilístico en el distrito de Puente Piedra, Lima – Perú, en el año 2016. Participaron 95 madres con niños menores de tres años. Se evaluaron tres dimensiones de satisfacción (técnico-científico, interpersonal y confort/soporte comunicacional) con una encuesta estructurada, validada por expertos. Las respuestas se calificaron entre 0 a 3 puntos (sí: 3, regularmente: 2, mínimamente: 1 y no: 0). El nivel de satisfacción se clasificó, sumando el puntaje de cada respuesta, como: bajo, medio, alto (0-7, 8-14, 15-21 puntos; respectivamente). Los resultados se presentan en frecuencias y porcentajes (variables cualitativas), y mediana y rango intercuartil (variables cuantitativas). Resultados. La satisfacción general de las madres fue media (45%), respecto a sus dimensiones la técnicocientífico fue baja (57%), y la interpersonal y confort/soporte comunicacional fue media (40% y 43%, respectivamente). Conclusión. Existe una satisfacción media y baja en la consejería consejería brindada por personal de salud que brinda la suplementación.Introduction.     Socioeconomic status impact on diet quality and body mass index in eight Latin American countries: ELANS study results

    Poor health and diet quality are associated with living within a low socioeconomic status (SES). This study aimed to investigate the impact of SES on diet quality and body mass index in Latin America. Data from the “Latin American Health and Nutrition Study (ELANS)”, a multi-country, population-based study of 9218 participants, were used. Dietary intake was collected through two 24 h recalls from participants of Argentina, Brazil, Chile, Colombia, Costa Rica, Ecuador, Peru and Venezuela. Diet quality was assessed using the dietary quality score (DQS), the dietary diversity score (DDS) and the nutrients adequacy ratio (NAR). Chi-squared and multivariate-variance analyses were used to estimate possible associations. We found that participants from the low SES consumed less fruits, vegetables, whole grains, fiber and fish and seafood and more legumes than those in the high SES. Also, the diet quality level, assessed by DQS, DDS and NAR mean, increased with SES. Women in the low SES also showed a larger prevalence of abdominal obesity and excess weight than those in the middle and high SES.     Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference) and obesity (BMI >2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesit

    An Evaluation of the COVID-19 Pandemic and Perceived Social Distancing Policies in Relation to Planning, Selecting, and Preparing Healthy Meals: An Observational Study in 38 Countries Worldwide

    Objectives: To examine changes in planning, selecting, and preparing healthy foods in relation to personal factors (time, money, stress) and social distancing policies during the COVID-19 crisis. Methods: Using cross-sectional online surveys collected in 38 countries worldwide in April-June 2020 (N = 37,207, Mage 36.7 SD 14.8, 77% women), we compared changes in food literacy behaviors to changes in personal factors and social distancing policies, using hierarchical multiple regression analyses controlling for sociodemographic variables. Results: Increases in planning (4.7 SD 1.3, 4.9 SD 1.3), selecting (3.6 SD 1.7, 3.7 SD 1.7), and preparing (4.6 SD 1.2, 4.7 SD 1.3) healthy foods were found for women and men, and positively related to perceived time availability and stay-at-home policies. Psychological distress was a barrier for women, and an enabler for men. Financial stress was a barrier and enabler depending on various sociodemographic variables (all p < 0.01). Conclusion: Stay-at-home policies and feelings of having more time during COVID-19 seem to have improved food literacy. Stress and other social distancing policies relate to food literacy in more complex ways, highlighting the necessity of a health equity lens.     Strategies used by mothers to feed preschool-aged children in a marginal urban area of Lima, Peru

    This study explored the strategies for feeding preschool-aged children used by mothers in a marginal urban area of Lima. This qualitative, grounded theory method-based study included 30 mothers who participated in 1 of 4 focus group sessions and/or 1 of 11 in-depth interviews. Previously recorded in-depth interviews and focus groups were transcribed to identify the final issues. The mothers’ ages ranged from 22 to 42 years, and 60% had not completed a basic education. The children’s ages ranged from 36 to 66 months. The following positive strategies were identified: a) feeding when hungry and b) management of feeding opportunities. The following negative strategies were also identified: a) forcing a child to eat and b) use of punishment or reward to encourage complete food consumption. In conclusion, the mothers in our study cohort employed both positive and negative strategies to encourage food consumption in their children

    Acceptability and safety of novel infant porridges containing lyophilized meat powder and iron-fortified wheat flour

    BACKGROUND: Lyophilized meat powder with iron-fortified wheat flour can be used to produce an infant porridge with bioavailable iron, but its acceptability and safety are unknown. OBJECTIVE: To evaluate the acceptability and safety of porridges containing lyophilized meat powder and iron-fortified wheat flour. METHODS: Peruvian mothers' input was used to develop porridges without (no meat) and with meat powder (low or high chicken liver, low or high chicken thigh). Acceptability was determined by maternal hedonic scoring, 9-day infant intake, and videotape analysis of how well infants liked each porridge. Dry and cooked porridges and meat ingredients were tested for microorganisms; meats were tested for pesticides. RESULTS: Mothers gave higher acceptability scores to the no-meat porridge, followed in order by low and high quantities of meat powder (e.g., mean +/- SD "taste"scores were 4.5 +/- 0.9 for the no-meat, 3.7 +/- 1.1 for the low-liver, and 3.3 +/- 1.1 for the high-liver porridges, p = .0001). Infants' porridge intake did not differ: 61.4 +/- 47.1 g of no-meat, 62.1 +/- 44.9 g of low-thigh, and 67.5 +/- 42.0 g of low-liver (p = .7), as supported by the video analysis. Microbiologic safety was acceptable except for marginally acceptable molds and yeasts in dry ingredients. No pesticide residues were detected. CONCLUSIONS: Despite mothers' clear preference for no-meat porridges, infants consumed equal amounts of porridges with and without meat. Thus, if mothers can be convinced to feed the meat-containing porridges to the infants despite their own preferences, the infants will consume these porridges. The mold and yeast content of the porridge ingredients must be reduced

    Preferencia de etiquetado nutricional frontal: octógono frente a semáforo GDA en mercados de Lima, Perú

    Objetivo: Evaluar la preferencia de dos diseños de etiquetado frontal en productos industrializados (octógono vs. semáforo-GDA). Material y métodos: Estudio descriptivo transversal realizado con usuarios de mercados de Lima Metropolitana (Chorrillos, Breña, La Victoria y La Molina). Se utilizó una encuesta previamente validada que constó de cuatro preguntas basadas en láminas con cada etiquetado nutricional: ¿cuál de los productos le parece más saludable?”, “¿por qué escogió este?”, “¿cuál de las dos etiquetas le parece más fácil de entender?” y “¿cuál de los dos etiquetados le gusta más?”. Los datos se presentaron en porcentajes y medianas (rango intercuartilar) y las diferencias entre los grupos se evaluaron con la prueba exacta de Fisher (datos categóricos). Resultados: Se encuestaron 93 usuarios, el semáforo-GDA fue elegido como el etiquetado más saludable (74,3%) y el que más les gustó (69,9%); sin embargo, el octógono fue el que les pareció más fácil de entender (58,4%). La mayoría de los usuarios consideró el semáforo como más saludable debido a que “tienen más símbolos verdes en la etiqueta”, lo que podría confundir la finalidad de la advertencia nutricional, minimizando el impacto del símbolo rojo y/o amarillo por presentarse en menor cantidad. Conclusiones: Los participantes mencionaron que el octógono era más sencillo y fácil de leer; los símbolos verdes del semáforo-GDA podrían influir en que el producto se perciba como más saludable

    Constraints on the delivery of animal-source foods to infants and young children : Case studies from five countries

    Background. Optimal feeding of infants and young children in developing countries includes daily feeding of animal-source foods. Objective. To evaluate constraints on the availability of animal-source foods at the community level, access to animal-source foods at the household level, and intake of animal-source foods at the individual level among children under 3 years of age in case studies in five developing countries: Mexico, Peru, Haiti, Senegal, and Ethiopia. Methods. Data were obtained from published and unpublished research and from program experiences of health and agriculture specialists. Results. In Ethiopia, 27% to 51% of case-study chil- dren had consumed an animal-source food on the previous day; from 56% to 87% of children in the other case-study sites had consumed an animal-source food on the previous day. Data on intake of animal-source foods in grams were only available for the Latin American case-study sites, where daily milk intake was high in Mexico and Peru (195 and 180 g/day, respectively) and the intakes of meat, fish, and poultry (MFP) (29.0 and 13.6 g/day) and of egg (18.4 and 4.9 g/day) were low. The conceptual model guiding this work identified more constraining factors at the community and household levels than at the individual level. The most common constraints on feeding animal-source foods to young children were poverty, animal health, and land degrada- tion at the community level; cost of animal-source foods and limited livestock holdings at the household level; and caregivers' perceptions of giving animal-source foods to children at the individual level. Conclusions. For program planning, it is useful to simultaneously consider factors that affect community availability of, household access to, and children's intake of animal-source foods. Efforts to overcome individual- level constraints on intake of animal-source foods should be coupled with activities to address community and household constraints
