Perspectives and control of hepatitis B virus infection in Taiwan
Hepatitis B virus (HBV) infection is endemic in Taiwan. After the implementation of universal hepatitis B vaccination, there was a significant reduction of hepatitis B surface antigen (HBsAg) seropositivity and HBV-related hepatocellular carcinoma (HCC) incidence in children, teenagers, and young adults. However, the incidence of HBV-related HCC in adults remains high. Through several community- and hospital-based cohort studies, the viral factors affecting the prognosis of HBV carriers have been illustrated. Serum HBV DNA level > 2000Â IU/mL at study entry starts to increase the risks of cirrhosis and HCC in adult patients with chronic HBV infection. In addition, serum HBsAg level > 1000Â IU/mL is associated with a higher risk of HCC in HBeAg-negative patients with low viral load. Virologically, HBV genotype C/D and core promote/pre-S mutations correlate with an increased HCC risk. Recently, a risk calculator has been developed to predict HCC in noncirrhotic patients with external validation. Therapeutically, hospital-based cohort and population-based nationwide studies indicated that interferon and nucleos(t)ide analogue treatments could reduce the incidence of HCC over time. Towards the ultimate goal of HBV eradication, several novel agents aiming at viral and host targets are under development. In addition, the immune therapy may play a key role in HBV cure in the foreseeable future
Room temperature gas sensing with a hybrid poly-Si/ZnO TFT cell
In this work, we study the capability of a novel poly-Si/Zno hybrid TFT cell in sensing NO2 gas. Fabrication and structural features of this cell are identical to that reported in one of our previous works [1] except that the IGZO TFT is replaced by a ZnO one. The equivalent circuit of the hybrid cell is shown in Fig. 1, in which the poly-Si TFT and ZnO TFT are employed as the amplifier and sensor, respectively. In the configuration, the top gate of the poly-Si TFT is electrically connected to the drain of the bottom-gated ZnO TFT, while the top surface of the ZnO channel is exposed to the environment for sensing purposes. For the electrical measurements conducted at room temperature, the current source (IIN) was set at 100 pA with a compliance VG of 2 V. Concentration of the NO2 gas in the ambient was varied from 0 ~100 ppm. Transfer characteristics of the cell are expressed by showing the drain current of the poly-Si TFT as a function of the back-gate bias (VBG) of the ZnO TFT. In the figure, we can see the I-V curves show a parallel and positive shift as the concentration of the NO2 gas is increased. Meanwhile, the transitions in the figure are steep with a slope of around -60 mV/dec whose absolute value is much smaller than the subthreshold slopes of the individual ZnO TFT (\u3e300 mV/dec). The finding provides good evidence showing the potential of this scheme in promoting measurement sensitivity compared with conventional oxide-semiconductor TFTs. The experimental results also show that UV irradiation can recover the characteristics.

Comparison of Renal Function and Other Health Outcomes in Vegetarians versus Omnivores in Taiwan
Renal disease is one of the top 10 leading causes of death, and the incidence of end-stage renal disease in Taiwan is the highest in the world. Many dietitians consider the diet of plant origin consumed by vegans to be ‘lighter’ and ‘more healthful’ than the diet of both plant and animal origin consumed by omnivores. Dietary protein has significant effects on renal functions. The study explored the effects of both the diets on renal functions. The study subjects included 102 Buddhist nun vegetarians and an equal number of matched control group (omnivores). A cross-sectional study was performed to investigate the effects of the diet of plant origin and the diet of both plant and animal origin on renal functions. There was no difference in the renal functions between the two groups. However, systolic blood pressure, blood urea nitrogen, serum sodium, glucose, cholesterol levels, and urinary specific gravity were lower in the vegetarian group. Although these results were compatible with general concepts regarding diet of plant origin, after adjusting for age, the duration of intake of this diet had no effect on the renal functions. Based on the findings, it is concluded that the renal functions, in terms of the estimated glomerular filtration rate, were not different between the vegetarians and the omnivores
A junctionless SONOS nonvolatile memory device constructed with in situ-doped polycrystalline silicon nanowires
In this paper, a silicon-oxide-nitride-silicon nonvolatile memory constructed on an n+-poly-Si nanowire [NW] structure featuring a junctionless [JL] configuration is presented. The JL structure is fulfilled by employing only one in situ heavily phosphorous-doped poly-Si layer to simultaneously serve as source/drain regions and NW channels, thus greatly simplifying the manufacturing process and alleviating the requirement of precise control of the doping profile. Owing to the higher carrier concentration in the channel, the developed JL NW device exhibits significantly enhanced programming speed and larger memory window than its counterpart with conventional undoped-NW-channel. Moreover, it also displays acceptable erase and data retention properties. Hence, the desirable memory characteristics along with the much simplified fabrication process make the JL NW memory structure a promising candidate for future system-on-panel and three-dimensional ultrahigh density memory applications
Early utilization of hypertonic peritoneal dialysate and subsequent risks of non-traumatic amputation among peritoneal dialysis patients: a nationwide retrospective longitudinal study
BACKGROUND: The hemodialysis (HD) population has a particularly high incidence of amputation, which is likely associated with decreased tissue oxygenation during HD. However, information about the risk factors leading to amputation in peritoneal dialysis (PD) patients is limited. Here, we have investigated the association between the use of hypertonic peritoneal dialysate (HPD) and subsequent amputation in PD patients. METHODS: Based on the data from the Taiwan National Health Insurance research database, this observational cohort study enrolled 203 PD patients who had received HPD early during treatment and had not undergone amputation and 296 PD controls who had not undergone amputation. Subjects were followed through until the end of 2009 and the event rates of new non-traumatic amputation were compared between groups. RESULTS: The incidence of amputation was 3 times higher for the HPD cohort than for the comparison cohort (23.68 vs. 8.01 per 1000 person-years). The hazard ratio (HR) for this group, estimated using a multivariable Cox model, was 2.48 (95% confidence interval [CI] = 1.06–5.79). The HR for patients with both diabetes and early adoption of HPD increased to 44.34 (95% CI = 5.51-357.03), compared to non-HPD non-diabetic PD controls. CONCLUSION: Early utilization of HPD in PD patients is associated with increasing risk of amputation; this risk considerably increases for those with concomitant diabetes
The Volcanic Earthquake Swarm of October 20, 2009 in the Tatun Area of Northern Taiwan
On October 20, 2009, a series of felt earthquakes with local magnitudes ranging from 2.8 - 3.2 occurred in the Tatun volcanic area off the northern tip of Taiwan. Although there was no damage caused by those earthquakes, many residents in the Taipei metropolitan area, particularly for people who live near the Yangminshan National Park, felt strong ground shaking. In order to know what the possible mechanisms were that generated those earthquakes, we carefully examined seismic data recorded by a dense seismic array in the Tatun volcanic area. During the period between October 18 and 22, 2009 we detected at least 202 micro-earthquakes. Most of the earthquakes were relocated using the double-difference method and were clustered in the shallow crust beneath the Dayoukeng area, which is the strongest fumarole in the Tatun volcanic area. Among these earthquakes, 72 focal mechanisms were determined by polarizing the first P-wave motion. Most earthquakes belonged to normal faulting. An extremely high b-value of 2.17 was obtained from those earthquakes. Based on the seismic variations in both the temporary and spatial distribution as well as an extremely high b-value, we conclude that the earthquake sequence on October 20, 2009 was a typically seismic swarm associated with possible active volcanism in the Tatun volcanic area
Topical application of marine briarane-type diterpenes effectively inhibits 12-O-tetradecanoylphorbol-13-acetate-induced inflammation and dermatitis in murine skin
<p>Abstract</p> <p>Background</p> <p>Skin is the largest organ in the body, and is directly exposed to extrinsic assaults. As such, the skin plays a central role in host defense and the cutaneous immune system is able to elicit specific local inflammatory and systemic immune responses against harmful stimuli. 12-O-tetradecanoylphorbol-13-acetate (TPA) can stimulate acute and chronic inflammation and tumor promotion in skin. TPA-induced dermatitis is thus a useful <it>in vivo </it>pharmacological platform for drug discovery. In this study, the inhibitory effect of briarane-type diterpenes (BrDs) from marine coral <it>Briareum excavatum </it>on TPA-induced dermatitis and dendritic cell (DC) function was explored.</p> <p>Methods</p> <p>Evans blue dye exudation was used to determine vascular permeability. H&E-stained skin section was used to determine the formation of edema in mouse abdominal skin. We also used immunohistochemistry staining and western blot assays to evaluate the activation of specific inflammation makers and key mediators of signaling pathway in the mouse skin. Furthermore, mouse bone marrow DCs were used to determine the relationship between the chemical structure of BrDs and their regulation of DC function.</p> <p>Results</p> <p>BrD1 remarkably suppressed TPA-induced vascular permeability and edema in skin. At the biochemical level, BrD1 inhibited TPA-induced expression of cyclooxygenase-2, inducible nitric oxide synthase and matrix metalloproteinase-9, the key indicators of cutaneous inflammation. This inhibition was apparently mediated by interference with the Akt/NF-κB-mediated signaling network. BrD1 also inhibited TNF-α and IL-6 expression in LPS-stimulated BMDCs. The 8, 17-epoxide of BrDs played a crucial role in the inhibition of IL-6 expression, and replacement of the C-12 hydroxyl group with longer esters in BrDs gradually decreased this inhibitory activity.</p> <p>Conclusions</p> <p>Our results suggest that BrDs warrant further investigation as natural immunomodulatory agents for control of inflammatory skin diseases.</p
Reduced expression of alpha-1,2-mannosidase I extends lifespan in Drosophila melanogaster and Caenorhabditis elegans
Exposure to sub-lethal levels of stress, or hormesis, was a means to induce longevity. By screening for mutations that enhance resistance to multiple stresses, we identified multiple alleles of alpha-1,2-mannosidase I (mas1) which, in addition to promoting stress resistance, also extended longevity. Longevity enhancement is also observed when mas1 expression is reduced via RNA interference in both Drosophila melanogaster and Caenorhabditis elegans. The screen also identified Edem1 (Edm1), a gene downstream of mas1, as a modulator of lifespan. As double mutants for both mas1 and Edm1 showed no additional longevity enhancement, it appeared that both mutations function within a common pathway to extend lifespan. Molecular analysis of these mutants revealed that the expression of BiP, a putative biomarker of dietary restriction (DR), is down-regulated in response to reductions in mas1 expression. These findings suggested that mutations in mas1 may extend longevity by modulating DR
