Factors influencing the spatial extent of mobile source air pollution impacts: a meta-analysis
<p>Abstract</p> <p>Background</p> <p>There has been growing interest among exposure assessors, epidemiologists, and policymakers in the concept of "hot spots", or more broadly, the "spatial extent" of impacts from traffic-related air pollutants. This review attempts to quantitatively synthesize findings about the spatial extent under various circumstances.</p> <p>Methods</p> <p>We include both the peer-reviewed literature and government reports, and focus on four significant air pollutants: carbon monoxide, benzene, nitrogen oxides, and particulate matter (including both ultrafine particle counts and fine particle mass). From the identified studies, we extracted information about significant factors that would be hypothesized to influence the spatial extent within the study, such as the study type (e.g., monitoring, air dispersion modeling, GIS-based epidemiological studies), focus on concentrations or health risks, pollutant under study, background concentration, emission rate, and meteorological factors, as well as the study's implicit or explicit definition of spatial extent. We supplement this meta-analysis with results from some illustrative atmospheric dispersion modeling.</p> <p>Results</p> <p>We found that pollutant characteristics and background concentrations best explained variability in previously published spatial extent estimates, with a modifying influence of local meteorology, once some extreme values based on health risk estimates were removed from the analysis. As hypothesized, inert pollutants with high background concentrations had the largest spatial extent (often demonstrating no significant gradient), and pollutants formed in near-source chemical reactions (e.g., nitrogen dioxide) had a larger spatial extent than pollutants depleted in near-source chemical reactions or removed through coagulation processes (e.g., nitrogen oxide and ultrafine particles). Our illustrative dispersion model illustrated the complex interplay of spatial extent definitions, emission rates, background concentrations, and meteorological conditions on spatial extent estimates even for non-reactive pollutants. Our findings indicate that, provided that a health risk threshold is not imposed, the spatial extent of impact for mobile sources reviewed in this study is on the order of 100–400 m for elemental carbon or particulate matter mass concentration (excluding background concentration), 200–500 m for nitrogen dioxide and 100–300 m for ultrafine particle counts.</p> <p>Conclusion</p> <p>First, to allow for meaningful comparisons across studies, it is important to state the definition of spatial extent explicitly, including the comparison method, threshold values, and whether background concentration is included. Second, the observation that the spatial extent is generally within a few hundred meters for highway or city roads demonstrates the need for high resolution modeling near the source. Finally, our findings emphasize that policymakers should be able to develop reasonable estimates of the "zone of influence" of mobile sources, provided that they can clarify the pollutant of concern, the general site characteristics, and the underlying definition of spatial extent that they wish to utilize.</p
A cost-benefit analysis of a pellet boiler with electrostatic precipitator versus conventional biomass technology: A case study of an institutional boiler in Syracuse, New York
BACKGROUND: Biomass facilities have received increasing attention as a strategy to increase the use of renewable fuels and decrease greenhouse gas emissions from the electric generation and heating sectors, but these facilities can potentially increase local air pollution and associated health effects. Comparing the economic costs and public health benefits of alternative biomass fuel, heating technology, and pollution control technology options provides decision-makers with the necessary information to make optimal choices in a given location.
METHODS: For a case study of a combined heat and power biomass facility in Syracuse, New York, we used stack testing to estimate emissions of fine particulate matter (PM2.5) for both the deployed technology (staged combustion pellet boiler with an electrostatic precipitator) and a conventional alternative (wood chip stoker boiler with a multicyclone). We used the atmospheric dispersion model AERMOD to calculate the contribution of either fuel-technology configuration to ambient primary PM2.5 in a 10 km x 10 km region surrounding the facility, and we quantified the incremental contribution to population mortality and morbidity. We assigned economic values to health outcomes and compared the health benefits of the lower-emitting technology with the incremental costs.
RESULTS: In total, the incremental annualized cost of the lower-emitting pellet boiler was 1.7 million annually, greatly exceeding the differential costs even when accounting for uncertainties. Our analyses also showed complex spatial patterns of health benefits given non-uniform age distributions and air pollution levels.
CONCLUSIONS: The incremental investment in a lower-emitting staged combustion pellet boiler with an electrostatic precipitator was well justified by the population health improvements over the conventional wood chip technology with a multicyclone, even given the focus on only primary PM2.5 within a small spatial domain. Our analytical framework could be generalized to other settings to inform optimal strategies for proposed new facilities or populations.This research was supported by the New York State Energy Research and Development Authority (NYSERDA), via an award to the Northeast States for Coordinated Air Use Management (Agreement #92229). Using Physiologically-Based Pharmacokinetic Models to Incorporate Chemical and Non-Chemical Stressors into Cumulative Risk Assessment: A Case Study of Pesticide Exposures
Cumulative risk assessment has been proposed as an approach to evaluate the health risks associated with simultaneous exposure to multiple chemical and non-chemical stressors. Physiologically based pharmacokinetic/pharmacodynamic (PBPK/PD) models can allow for the inclusion and evaluation of multiple stressors, including non-chemical stressors, but studies have not leveraged PBPK/PD models to jointly consider these disparate exposures in a cumulative risk context. In this study, we focused on exposures to organophosphate (OP) pesticides for children in urban low-income environments, where these children would be simultaneously exposed to other pesticides (including pyrethroids) and non-chemical stressors that may modify the effects of these exposures (including diet). We developed a methodological framework to evaluate chemical and non-chemical stressor impacts on OPs, utilizing an existing PBPK/PD model for chlorpyrifos. We evaluated population-specific stressors that would influence OP doses or acetylcholinesterase (AChE) inhibition, the relevant PD outcome. We incorporated the impact of simultaneous exposure to pyrethroids and dietary factors on OP dose through the compartments of metabolism and PD outcome within the PBPK model, and simulated combinations of stressors across multiple exposure ranges and potential body weights. Our analyses demonstrated that both chemical and non-chemical stressors can influence the health implications of OP exposures, with up to 5-fold variability in AChE inhibition across combinations of stressor values for a given OP dose. We demonstrate an approach for modeling OP risks in the presence of other population-specific environmental stressors, providing insight about co-exposures and variability factors that most impact OP health risks and contribute to children’s cumulative health risk from pesticides. More generally, this framework can be used to inform cumulative risk assessment for any compound impacted by chemical and non-chemical stressors through metabolism or PD outcomes

Modeling Spatial Patterns of Traffic-Related Air Pollutants in Complex Urban Terrain
Background: The relationship between traffic emissions and mobile-source air pollutant concentrations is highly variable over space and time and therefore difficult to model accurately, especially in urban settings with complex terrain. Regression-based approaches using continuous real-time mobile measurements may be able to characterize spatiotemporal variability in traffic-related pollutant concentrations but require methods to incorporate temporally varying meteorology and source strength in a physically interpretable fashion. Objective: We developed a statistical model to assess the joint impact of both meteorology and traffic on measured concentrations of mobile-source air pollutants over space and time. Methods: In this study, traffic-related air pollutants were continuously measured in the Williamsburg neighborhood of Brooklyn, New York (USA), which is affected by traffic on a large bridge and major highway. One-minute average concentrations of ultrafine particulate matter (UFP), fine particulate matter [ in aerodynamic diameter ], and particle-bound polycyclic aromatic hydrocarbons were measured using a mobile-monitoring protocol. Regression modeling approaches to quantify the influence of meteorology, traffic volume, and proximity to major roadways on pollutant concentrations were used. These models incorporated techniques to capture spatial variability, long- and short-term temporal trends, and multiple sources. Results: We observed spatial heterogeneity of both UFP and concentrations. A variety of statistical methods consistently found a 15–20% decrease in UFP concentrations within the first 100 m from each of the two major roadways. For , temporal variability dominated spatial variability, but we observed a consistent linear decrease in concentrations from the roadways. Conclusions: The combination of mobile monitoring and regression analysis was able to quantify local source contributions relative to background while accounting for physically interpretable parameters. Residential exposure to aircraft noise and hospital admissions for cardiovascular diseases: multi-airport retrospective study
Objective: To investigate whether exposure to aircraft noise increases the risk of hospitalization for cardiovascular diseases in older people (≥65 years) residing near airports. Design: Multi-airport retrospective study of approximately 6 million older people residing near airports in the United States. We superimposed contours of aircraft noise levels (in decibels, dB) for 89 airports for 2009 provided by the US Federal Aviation Administration on census block resolution population data to construct two exposure metrics applicable to zip code resolution health insurance data: population weighted noise within each zip code, and 90th centile of noise among populated census blocks within each zip code. Setting: 2218 zip codes surrounding 89 airports in the contiguous states. Participants: 6 027 363 people eligible to participate in the national medical insurance (Medicare) program (aged ≥65 years) residing near airports in 2009. Main outcome measures Percentage increase in the hospitalization admission rate for cardiovascular disease associated with a 10 dB increase in aircraft noise, for each airport and on average across airports adjusted by individual level characteristics (age, sex, race), zip code level socioeconomic status and demographics, zip code level air pollution (fine particulate matter and ozone), and roadway density. Results: Averaged across all airports and using the 90th centile noise exposure metric, a zip code with 10 dB higher noise exposure had a 3.5% higher (95% confidence interval 0.2% to 7.0%) cardiovascular hospital admission rate, after controlling for covariates. Conclusions: Despite limitations related to potential misclassification of exposure, we found a statistically significant association between exposure to aircraft noise and risk of hospitalization for cardiovascular diseases among older people living near airports

Nitrogen dioxide concentrations in neighborhoods adjacent to a commercial airport: a land use regression modeling study
BACKGROUND: There is growing concern in communities surrounding airports regarding the contribution of various emission sources (such as aircraft and ground support equipment) to nearby ambient concentrations. We used extensive monitoring of nitrogen dioxide (NO(2)) in neighborhoods surrounding T.F. Green Airport in Warwick, RI, and land-use regression (LUR) modeling techniques to determine the impact of proximity to the airport and local traffic on these concentrations. METHODS: Palmes diffusion tube samplers were deployed along the airport's fence line and within surrounding neighborhoods for one to two weeks. In total, 644 measurements were collected over three sampling campaigns (October 2007, March 2008 and June 2008) and each sampling location was geocoded. GIS-based variables were created as proxies for local traffic and airport activity. A forward stepwise regression methodology was employed to create general linear models (GLMs) of NO(2 )variability near the airport. The effect of local meteorology on associations with GIS-based variables was also explored. RESULTS: Higher concentrations of NO(2 )were seen near the airport terminal, entrance roads to the terminal, and near major roads, with qualitatively consistent spatial patterns between seasons. In our final multivariate model (R(2 )= 0.32), the local influences of highways and arterial/collector roads were statistically significant, as were local traffic density and distance to the airport terminal (all p < 0.001). Local meteorology did not significantly affect associations with principal GIS variables, and the regression model structure was robust to various model-building approaches. CONCLUSION: Our study has shown that there are clear local variations in NO(2 )in the neighborhoods that surround an urban airport, which are spatially consistent across seasons. LUR modeling demonstrated a strong influence of local traffic, except the smallest roads that predominate in residential areas, as well as proximity to the airport terminal
Urogenital dysfunction in male patients with Charcot-Marie-Tooth: a systematic review
Aims Purposes of this study were to describe lower urinary tract symptoms (LUTS) and related urodynamic patterns in patients with hereditary spastic paraplegia (HSP), and to characterize LUTS management and associated uronephrological complications. Methods We retrospectively reviewed medical files of HSP patients, consecutively followed in our Physical and Rehabilitation Medicine Department between 1999 and 2016. Clinical, urodynamic, and radiological data were collected and analyzed. Different treatments which have been prescribed and uronephrological complications were also recorded. Patients with other neurological or urological diseases were excluded. Results Thirty-three patients with HSP were included. Mean duration of follow-up was 8.1 ± 5 years, mean age 62 ± 14 years, and 70% were men. The most frequent LUTS was urgency and voiding dysfunction (both 69.7%). Incontinence and retention with a significant postvoid residue above 100 mL accounted for 66.7% and 57.6% of initial symptoms respectively. Neurogenic detrusor overactivity was diagnosed in 80.7% of patients. Two-thirds of our cohort were treated with anticholinergics and 9.1% required intradetrusor botulinum-toxin injections. Only 27.3% of patients performed clean intermittent self-catheterization. Febrile urinary tract infections (21.2%), urolithiasis (15,1%), hydronephrosis (6%), and chronic renal failure (9.1%) were found. Conclusion Given their high prevalence and the risk of uronephrological complications, LUTS should be systematically assessed in HSP patients. The systematic screening of urological dysfunction in this population would improve its management, decrease the incidence of uronephrological complications, and increase the quality of life
