Proste interwencje poprawiające przestrzeganie zaleceń terapeutycznych przez dzieci z przewlekłymi chorobami wątroby — doświadczenia jednego ośrodka i przegląd piśmiennictwa
Leczenie przewlekłych chorób wątroby u dzieci często jest nieskuteczne ze względu na
nieprzestrzeganie zaleceń terapeutycznych.
Celem pracy było wypracowanie prostych, łatwych do wprowadzenia interwencji poprawiających
akceptację przewlekłego leczenia przez dzieci.
W bazie PubMed przy użyciu słów kluczowych: adherence, compliance, therapy, chronic
diseases, children wyszukano anglojęzyczne publikacje z ostatnich 10 lat, poświęcone
przestrzeganiu zaleceń terapeutycznych przez dzieci, skupiając się na chorobach
wątroby. Następnie przeprowadzono dyskusję okrągłego stołu, mającą na celu wypracowanie
interwencji poprawiających akceptację długotrwałego leczenia. W dyskusji
wzięli udział pracownicy Kliniki Pediatrii i Chorób Infekcyjnych we Wrocławiu (9 lekarzy,
średni staż pracy 15 lat) — ośrodek ten zajmuje się dziećmi z przewlekłymi chorobami
wątroby w wieku 0–18 lat. W ostatnich 5 latach odnotowano: 556 przypadków zachorowań
na zapalenie wątroby typu B, 409 na zapalenie wątroby typu C, 37 na chorobę
Wilsona, 50 na autoimmunologiczne zapalenie wątroby, 39 na niealkoholowe stłuszczeniowe
zapalenie wątroby oraz 138 nieprawidłowych prób wątrobowych.
Wyszukiwanie PubMed przyniosło 321 wyników. Na podstawie streszczeń do dyskusji
wybrano 7 publikacji. Za czynniki sprzyjające nieprzestrzeganiu zaleceń uznano: przyjmowanie licznych tabletek, zwłaszcza kilka razy dziennie, terapię lekami o częstych
działaniach ubocznych, brak wiedzy o możliwych działaniach ubocznych, wieloletnie
leczenie, chorobę Wilsona, wiek dojrzewania, częstą zmianę lekarza prowadzącego
i niski status społeczno-ekonomiczny. Za interwencje poprawiające adherencję uznano:
budowanie bardziej osobistych relacji z pacjentem, zaangażowanie emocjonalne, zainteresowanie
problemami zdrowotnymi oraz osobistymi nastolatków (np. stały lekarz
prowadzący), zmniejszenie liczby przyjmowanych tabletek przez stosowanie wyższych
dawek, uprzedzenie o możliwych działaniach niepożądanych, zwiększenie częstości konsultacji,
regularną kontrolę prób wątrobowych w celu wczesnego wykrycia przerwania
leczenia, edukację dzieci na temat ich choroby i telefoniczne nadzorowanie leczenia.
Wyniki leczenia dzieci z przewlekłymi chorobami wątroby można poprawić przez wprowadzenie
prostych interwencji zwiększających akceptację dla terapii, na przykład częstsze
kontrole, bardziej osobiste relacje z lekarzem prowadzącym, zmniejszenie liczby
tabletek oraz edukację pacjentów i ich rodziców na temat choroby.

Choroby układu oddechowego stały się w ostatnich latach czwartą najczęstszą przyczyną zgonów w Polsce. Infekcja dróg oddechowych, zwłaszcza zapalenie płuc, może prowadzić do zaostrzenia przewlekłej choroby układu krążenia. Streptococcus pneumoniae jest najczęstszym bakteryjnym patogenem powodującym pozaszpitalne zapalenie płuc. Pneumokoki są również najczęstszym patogenem wikłającym przebieg grypy. Zapalenie płuc, zwłaszcza inwazyjna choroba pneumokokowa, wiąże się z ryzykiem zgonu w przebiegu niewydolności oddechowej lub posocznicy, a także z pogorszeniem rokowania istniejącej choroby układu krążenia. Pomimo to zalecenia dotyczące szczepienia przeciwko pneumokokom nadal nie są dobrze ugruntowane w wytycznych kardiologicznych. Celem niniejszego dokumentu jest podsumowanie aktualnej wiedzy na temat znaczenia profilaktyki inwazyjnej choroby pneumokokowej w kontekście pacjentów z chorobami układu sercowo-naczyniowego

River Flow Prediction for Future Climate Using Long Series of Multi-Site Synthetic Data and MIKE SHE Model
A new simulation of daily flow for Kaczawa River, south-west Poland for extra long series of generated meteorological data (comparing to previous research) and selected climate change scenarios are presented. The Representative Concentration Pathways (RCPs) scenarios vs. SRES are introduced for simulations. The flow simulation in the river catchment is made using MIKE SHE hydrological model while the multisite data are generated by spatial weather generator SWGEN. Simulations are done for 2040 and 2060 while the simulations for the year 2000 are used as a background. The large number of new simulated series determined by the lead time, three climate change scenarios (RCP2.6 RCP4.5 and RCP6.0), and number of generated years (1000 for each case) is equal to 7000 for a single station. Finally, Pdf function for flow is presented as well probability of exceedance of maximum flow

In this paper a new simulations of minimum daily flow for Kaczawa River a left side tributary of the Odra River in south-west Poland are presented. Generated data were made based on very long series of 35 years of observed data and 24 sites of meteorological stations for south-west Poland gathered from the the Institute of Meteorology and Water Management National Research Institute (IMGW). For the data generation the spatial weather generator SWGEN producing the multisite daily time series was applied. Data were generated for the present (the year 2000 are used as a background) as well for future climate condition for 2060 and 2080 according Representative Concentration Pathways (RCPs) scenarios. The flow simulation in the river catchment is made using MIKE SHE hydrological model. Simulations are done for 2060 and 2080. Evaluation of multisite synthetic data generated by spatial weather generator and long climate data series
In this paper a new validation test for the spatial weather generator SWGEN producing the multisite daily time series of solar radiation, temperature and precipitation is presented. The method was tested by comparing statistics of 1000 years of generated data with extra long series of 35 years of observed weather parameters and 24 sites of meteorological stations for south-west Poland. The method evaluation showed that the means (sums) and variances of generated data were comparable with observed climatic data aggregated for months, seasons and years
