Fomento de la autoeficacia, esperanza hacia el futuro y compromiso con la comunidad
El objetivo del presente artículo es reflexionar acerca de la importancia de la participación juvenil en la sociedad contemporánea, presentando algunos resultados de una investigación empírica interdisciplinar realizada en Italia. Se ha analizado el impacto de la participación sobre algunas habilidades individuales estratégicas en los jóvenes de una sociedad profundamente individualizada. Para ello se ha aplicado una encuesta on-line a una muestra de 1410 jóvenes de entre 14 y 30 años. La hipótesis de partida es que la participación activa, en diferentes contextos vitales, puede tener un efecto positivo en el sentido de autoeficacia, la esperanza hacia el futuro y el compromiso con la comunidad. Los resultados obtenidos indicaron que la participación activa en la familia, en el contexto escolar y en las asociaciones logra fortalecer las mencionadas habilidades y contribuir a la formación de jóvenes capaces de enfrentarse a los retos actuales
Desarrollo de software dirigido por modelos en entornos ágiles
Las prácticas de desarrollo ágil se han consolidado como un estándar de la industria en los últimos años. Su piedra fundamental fue plantada por profesionales, que en 2001, realizaron un manifiesto donde propusieron cuatro valores y doce principios. Usando como base dicho manifiesto surgieron distintas iniciativas como: eXtreme Programming, Crystal Family, o Scrum y algunas más actuales como Integración y Entrega Continua, DevOps, Managment 3.0 o Lean.
Actualmente existen reportes de la industria que demuestran una creciente adopción de estas prácticas en equipos de diverso tamaño y características. Sin embargo, muchas de estas técnicas no tienen definido completamente sus procesos y mucho menos existen herramientas que brinden soporte a dichos procesos.
Proponemos aplicar las técnicas de la Ingeniería Dirigida por Modelos (MDE) al modelado de dichos procesos y a la posterior construcción de herramientas que los soporten. Para ello se plantea, aplicando MDE, definir un marco para soportar el modelado de los procesos ágiles, definiendo nuevos DSL, editores, transformaciones y generando modelos de trazas. Para la
validación, además de las herramientas de desarrollo, se llevarán a cabo casos de estudio
reales en colaboración con empresas del medio e internacionales.

Level II Oncoplastic Surgery as an Alternative Option to Mastectomy with Immediate Breast Reconstruction in the Neoadjuvant Setting: A Multidisciplinary Single Center Experience
: Oncoplastic surgery level II techniques (OPSII) are used in patients with operable breast cancer. There is no evidence regarding their safety and efficacy after neoadjuvant chemotherapy (NAC). The aim of this study was to compare the oncological and aesthetic outcomes of this technique compared with those observed in mastectomy with immediate breast reconstruction (MIBR), in post-NAC patients undergoing surgery between January 2016 and March 2021. Local disease-free survival (L-DFS), regional disease-free survival (R-DFS), distant disease-free survival (D-DFS), and overall survival (OS) were compared; the aesthetic results and quality of life (QoL) were evaluated using BREAST-Q. A total of 297 patients were included, 87 of whom underwent OPSII and 210 of whom underwent MIBR. After a median follow-up of 39.5 months, local recurrence had occurred in 3 patients in the OPSII group (3.4%), and in 13 patients in the MIBR group (6.1%) (p = 0.408). The three-year L-DFS rates were 95.1% for OPSII and 96.2% for MIBR (p = 0.286). The three-year R-DFS rates were 100% and 96.4%, respectively (p = 0.559). The three-year D-DFS rate were 90.7% and 89.7% (p = 0.849). The three-year OS rates were 95.7% and 95% (p = 0.394). BREAST-Q highlighted significant advantages in physical well-being for OPSII. No difference was shown for satisfaction with breasts (p = 0.656) or psychosocial well-being (p = 0.444). OPSII is safe and effective after NAC. It allows oncological and aesthetic outcomes with a high QoL, and is a safe alternative for locally advanced tumors which are partial responders to NAC
The Role of the E2F Transcription Factor Family in UV-Induced Apoptosis
The E2F transcription factor family is traditionally associated with cell cycle control. However, recent data has shown that activating E2Fs (E2F1-3a) are potent activators of apoptosis. In contrast, the recently cloned inhibitory E2Fs (E2F7 and 8) appear to antagonize E2F-induced cell death. In this review we will discuss (i) the potential role of E2Fs in UV-induced cell death and (ii) the implications of this to the development of UV-induced cutaneous malignancies
The role of the LISTANet Consortium in the European DEDIPAC-KH project
Aim:To improve understanding of the determinants of dietary, physical activity (PA), and sedentary behaviours, the European multi-disciplinary consortium on “Determinants of Diet and Physical Activity Knowledge Hub” (DEDIPAC-KH) includes 46 consortia and organisations supported by joint programming grants from 12 countries across Europe (Lakerveld et al., 2014). Six Italian Universities (e.g., Cassino, Chieti-Pescara, Palermo, Roma Foro Italico, Roma Sapienza, and UCSC) participating in the LISTANet consortium supported by MIUR (B84G14000040008) contributed to the Thematic Area2 “Determinants of dietary, PA, and sedentary behaviours across the life course and in vulnerable groups”. In particular, the coordinator of LISTANet Prof Capranica and Prof. MacDonncha from the Irish Physical Activity and Health Consortium act as Work Package (WP) Leaders of PA determinants (WP2.2). Methods: A mix of methods has been used in identifying PA determinants by developing PA taxonomy and a European framework (EU-PAD), seven umbrella systematic literature reviews (e.g., behavioural, biological, economic, physical, policy, psychological, and socio-cultural), and identifying ongoing/recently completed European-funded projects and data sets for secondary data analyses. Results: LISTANet participated in DEDIPAC-KH meetings/seminars/courses/conferences, and organized two workshops dedicated to the EU-PAD framework and umbrella SLRs. Outcomes included internal reports, presentations to international conferences, and scientific papers submitted for publications. Conclusions: The DEDIPAC-KH project represents an excellent start in setting up a complex, cross-country, organisational structure to: 1) guide a European strategic plan for novel and multi-disciplinary research addressing the complexity of determinants of PA behaviours across the life course; and 2) identify key aspects for potential strategies and intervention programmes to implement multi-sectoral European policies in PA. Finally, the cumulated experience of LISTANet could be valuable to fully exploit effective research and actions to increase PA levels of Italian citizens
Characteristics and patterns of care of endometrial cancer before and during COVID-19 pandemic
Objective: Coronavirus disease 2019 (COVID-19) outbreak has correlated with the disruption of screening activities and diagnostic assessments. Endometrial cancer (EC) is one of the most common gynecological malignancies and it is often detected at an early stage, because it frequently produces symptoms. Here, we aim to investigate the impact of COVID-19 outbreak on patterns of presentation and treatment of EC patients. Methods: This is a retrospective study involving 54 centers in Italy. We evaluated patterns of presentation and treatment of EC patients before (period 1: March 1, 2019 to February 29, 2020) and during (period 2: April 1, 2020 to March 31, 2021) the COVID-19 outbreak. Results: Medical records of 5,164 EC patients have been retrieved: 2,718 and 2,446 women treated in period 1 and period 2, respectively. Surgery was the mainstay of treatment in both periods (p=0.356). Nodal assessment was omitted in 689 (27.3%) and 484 (21.2%) patients treated in period 1 and 2, respectively (p<0.001). While, the prevalence of patients undergoing sentinel node mapping (with or without backup lymphadenectomy) has increased during the COVID-19 pandemic (46.7% in period 1 vs. 52.8% in period 2; p<0.001). Overall, 1,280 (50.4%) and 1,021 (44.7%) patients had no adjuvant therapy in period 1 and 2, respectively (p<0.001). Adjuvant therapy use has increased during COVID-19 pandemic (p<0.001). Conclusion: Our data suggest that the COVID-19 pandemic had a significant impact on the characteristics and patterns of care of EC patients. These findings highlight the need to implement healthcare services during the pandemic

COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study
Background:
The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms.
Methods:
International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms.
Results:
‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country.
Interpretation:
This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men
From Delivery to Adoption of Physical Activity Guidelines: Realist Synthesis
Background: Evidence-based guidelines published by health authorities for the promotion of health-enhancing physical activity (PA), continue to be implemented unsuccessfully and demonstrate a gap between evidence and policies. This review synthesizes evidence on factors influencing delivery, adoption and implementation of PA promotion guidelines within different policy sectors (e.g., health, transport, urban planning, sport, education). Methods: Published literature was initially searched using PubMed, EBSCO, Google Scholar and continued through an iterative snowball technique. The literature review spanned the period 2002–2017. The realist synthesis approach was adopted to review the content of 39 included studies. An initial programme theory with a four-step chain from evidence emersion to implementation of guidelines was tested. Results: The synthesis furthers our understanding of the link between PA guidelines delivery and the actions of professionals responsible for implementation within health services, school departments and municipalities. The main mechanisms identified for guidance implementation were scientific legitimation, enforcement, feasibility, familiarity with concepts and PA habits. Threats emerged to the successful implementation of PA guidelines at national/local jurisdictional levels. Conclusions: The way PA guidelines are developed may influence their adoption by policy-makers and professionals. Useful lessons emerged that may inform synergies between policymaking and professional practices, promoting win-win multisectoral strategies
