    Can External Interventions in the Field of Traditional Medicine Help Conserve Natural Resources and Enhance Ancestral Heritage?

    In Madagascar some development programs interested in local medical knowledge have been working simultaneously to enhance the life of people and conserve the natural habitat. A study of these practices casts a different light on their work. After presenting several misconceptions concerning traditional medicine, I discuss the use of local taboos by development organisations as a means for conserving natural environment. Secondly I examine attempts to plant medicinal garens to stem the loss of natural habitat, and finally, I discuss misunderstandings concerning the notion of development and the selective screening of therapeutic practices. I suggest in conclusion that programs should primarly be less concerned with assessing the immediate prcaticalities of the proposed action and more concerned with an understanding of the soical whole

    Understanding the pharmacokinetics of Coartem®

    Artemether and lumefantrine (AL), the active constituents of Coartem® exhibit complementary pharmacokinetic profiles. Artemether is absorbed quickly; peak concentrations of artemether and its main active metabolite, dihydroartemisinin (DHA) occur at approximately two hours post-dose, leading to a rapid reduction in asexual parasite mass and a prompt resolution of symptoms. Lumefantrine is absorbed and cleared more slowly (terminal elimination half-life 3-4 days in malaria patients), and accumulates with successive doses, acting to prevent recrudescence by destroying any residual parasites that remain after artemether and DHA have been cleared from the body. Food intake significantly enhances the bioavailability of both artemether and lumefantrine, an effect which is more apparent for the highly lipophilic lumefantrine. However, a meal with only a small amount of fat (1.6 g) is considered sufficient to achieve adequate exposure to lumefantrine. The pharmacokinetics of artemether or lumefantrine are similar in children, when dosed according to their body weight, compared with adults. No randomized study has compared the pharmacokinetics of either agent in pregnant versus non-pregnant women. Studies in healthy volunteers and in children with malaria have confirmed that the pharmacokinetic characteristics of crushed standard AL tablets and the newly-developed Coartem® Dispersible tablet formulation are similar. Studies to date in healthy volunteers have not identified any clinically relevant drug-drug interactions; data relating to concomitant administration of HIV therapies are limited. While dose-response analyses are difficult to undertake because of the low rate of treatment failures under AL, it appears that artemether and DHA exposure impact on parasite clearance time while lumefantrine exposure is associated with cure rate, consistent with their respective modes of action. In conclusion, knowledge of the pharmacokinetic profiles of artemether and lumefantrine is increasing within a range of settings, including infants and children. However, additional data would be warranted to better characterize artemether and lumefantrine pharmacokinetics in patients with hepatic impairment, in pregnant women, and in patients undergoing HIV/AIDS chemotherapy

    Galaxy-Galaxy Lensing by Non-Spherical Haloes I:Theoretical Considerations

    We use Monte Carlo simulations to investigate the theory of galaxy-galaxy lensing by non-spherical dark matter haloes. The simulations include a careful accounting of the effects of multiple deflections. In a typical data set where the mean tangential shear of sources with redshifts zs ~ 0.6 is measured with respect to the observed symmetry axes of foreground galaxies with redshifts zl ~ 0.3, the signature of anisotropic galaxy-galaxy lensing differs substantially from the expectation that one would have in the absence of multiple deflections. The observed ratio of the mean tangential shears, g+/g-, is strongly suppressed compared to the function that one would measure if the intrinsic symmetry axes of the foreground galaxies were known. Depending upon the characteristic masses of the lenses, the observed ratio of the mean tangential shears may be consistent with an isotropic signal (despite the fact that the lenses are non-spherical), or it may even be reversed from the expected signal (i.e., the mean tangential shear for sources close to the observed minor axes of the lenses may exceed the mean tangential shear for sources close to the observed major axes of the lenses). These effects are caused primarily by the fact that the lens galaxies have, themselves, been lensed and therefore the observed symmetry axes of the lenses differ from their intrinsic symmetry axes. The effects of lensing of the foreground galaxies on the observed function g+/g- cannot be eliminated by the rejection of foreground galaxies with small image ellipticities, nor by focusing the analysis on sources that are located very close to the observed symmetry axes of the foreground galaxies. We conclude that any attempt to use a measurement of g+/g- to constrain the shapes of dark matter galaxy haloes must include Monte Carlo simulations that take multiple deflections properly into account.Comment: 15 pages, 17 figures, submitted to MNRAS, full manuscript with high-resolution version of Fig. 4 can be found at http://firedrake.bu.edu/preprints/preprints.htm
