    Knowledge and Anxiety Related to Hereditary Ovarian Cancer in Serous Ovarian Cancer Patients

    PURPOSE: The awareness of hereditary breast and ovarian cancer (HBOC) and BRCA testing is increasing in Korea. Compared to the sizable research on HBOC knowledge among breast cancer women, studies in the ovarian cancer population are limited. This paper aimed to investigate the level of knowledge of hereditary ovarian cancer and anxiety in women diagnosed with serous ovarian cancer in Korea and determine differences in the knowledge and anxiety according to whether genetic testing was undertaken and whether BRCA1 or BRCA2 mutations were present. METHODS: Using a descriptive research design, a cross-sectional survey was conducted on 100 women diagnosed with serous ovarian cancer at N hospital in Gyeonggi-do, Korea, from July to November 2018. The collected data were analyzed by descriptive statistics, independent t-tests, one-way analysis of variance, and Pearson's correlation coefficient using the SPSS 21.0 program. RESULTS: The hereditary ovarian cancer-related knowledge score was mid-level (mean score 8.90±3.29 out of a total of 17), as was the state anxiety level was mid-level (mean score 47.96±3.26 out of possible score range of 20–80). Genetic knowledge of hereditary ovarian cancer was associated with age, education, occupation, genetic counseling, and BRCA mutations. There were no statistically significant factors related to anxiety and there were no statistically significant correlations between knowledge level and anxiety. CONCLUSION: More comprehensive education on gene-related cancer is needed for ovarian cancer patients, especially for items with low knowledge scores. A genetic counseling protocol should be developed to allow more patients to alleviate their anxiety through genetic counseling

    Dispersion of Vascular Plant in Kumo-do, Korea

    AbstractThe vascular plants observed in the area were composed of a total of 228 taxa; 72 families, 172 genus, 201 species, 25 varieties, 1 sub-species and 1 cross species. The only endangered plants found in the area were Milletia japonica (Siebold & Zucc.) A.Gray. The endemic plants growing in the Geumodo except transplanted plants were Lespedeza x maritima Nakai and Carpinus coreana Nakai. which accounted for 0.8% of the vascular plants in Geumodo, 228 taxa. Specialized plants of Geumodo were a total of 41 species; 30 taxa in Grade I, 1 taxon in Grade II, 9 taxa in Grade III and 1 taxon in Grade V. Milletia japonica (Siebold & Zucc.) A.Gray was the only species found in important Grade IV to V. Currently, ferries ply to the island, attracting many tourists. This poses a threat to the rare plants living in the island and presses down the island to develop. Therefore, in the long-term perspective, the conservation plan such as comprehensive research and monitoring on the ecosystem shall be established to protect evergreen broad-leaved forests

    Lower extremity edema in patients with early ovarian cancer

    BACKGROUND: The objective of this study was to investigate clinical manifestations of lower extremity edema (LEE) in early ovarian cancer. METHODS: Patients with early ovarian cancer who underwent staging surgery between January 2001 and December 2010. Medical records for LEE and/or responses to the Gynecologic Cancer Lymphedema Questionnaire (GCLQ) were evaluated. RESULTS: Patients had a median age of 46 years. Twenty-nine patients (40.8%) had past (13 patients, 44.8%) and/or current patient-reported LEE (16 patients, 55.2%). Symptoms reported on the GCLQ in over 20% of respondents were numbness, firmness/tightness, swelling, heaviness, limited movement of knee, and aching. GCLQ total symptoms score was significantly higher in patients with current LEE. Most of the LEE (25/29, 86.2%) developed within 12 months after surgery and LEE lasted more than 6 months in approximately two-thirds of the patients (18/29, 62.1%). Only half of the patients (52.1%) indicated knowledge of lymphedema: 86.2% of LEE patients and 28.6% of patients with no LEE. CONCLUSIONS: Although a significant proportion of patients with ovarian cancer have LEE after surgery, most are not aware of lymphedema until they develop. Education and analyses for LEE and lymphedema are needed in patients with ovarian cancer

    Learning Curve Associated with Complications in Biportal Endoscopic Spinal Surgery: Challenges and Strategies

    Study DesignDescriptions of technical strategies to overcome pitfalls associated with early learning periods in biportal endoscopic spinal surgery (BESS).PurposeTo introduce BESS for lumbar spinal diseases (LSDs) and to inform certain challenges to be overcome in mastering the technique.Overview of LiteratureBESS has shown superior benefits including excellent magnification, a wider range of view by dynamic handling of an endoscope and instruments. Clinical reports, however, have not yet been very revealing for its new introduction into minimally invasive spine surgery.MethodsTo evaluate the learning curve for BESS, the procedures for various LSDs by one surgeon were analyzed in the view of shortening of the operating times and reduction of complications. Reviewing of recorded procedures helped in finding the reasons and the implemented solutions.ResultsThe 68 cases included 25 for lumbar disc herniation (LDH), 3 for revision for recurred LDH, 39 for lumbar spinal stenosis (LSS) and 1 for synovial cyst. The operation time for the total cases averaged 83.7±33.6 minutes. According to diagnosis, it was 68.2±23.7 minutes for LDH. After the 14th case of LDH, it was nearly constant and close to the average time. One level of LSS needed 110.4±34.4 minutes. Prolonged operation times even in some later cases of LSS were mainly from struggling against blurred vision due to epidural bleeding. There were 7 cases of complications (10.3%) including 2 cases of dural tear, 1 case of root injury, and 4 cases of incomplete decompression on postoperative magnetic resonance imaging. There was no case of symptomatic hematoma or wound infection.ConclusionsBESS seemed to have a relatively short learning curve period. The overall complication rate in early learning period was 10.3%. These could be avoided by magnified regional views on an endoscope and a clear surgical field by controlling epidural bleeding

    Induction of inflammatory cytokines and toll-like receptors in chickens infected with avian H9N2 influenza virus

    H9N2 influenza virus is endemic in many Asian countries and is regarded as a candidate for the next human pandemic. Knowledge of the induction of inflammatory responses and toll-like receptors (TLRs) in chickens infected with H9N2 is limited. Here, we show that H9N2 induces pro-inflammatory cytokines such as transforming growth factor-beta 3; tumor necrosis factor-alpha; interferon-alpha, -beta, and gamma; and TLR 1, 2, 3, 4, 5, 7, and 15 in trachea, lung, and intestine of infected chickens. In the lung, TLR-15 was dominantly induced. Taken together, it seems that H9N2 infections efficiently induce inflammatory cytokines and TLRs in trachea, lung and intestine of chickens

    A Study Using a Monte Carlo Method of the Optimal Configuration of a Distribution Network in Terms of Power Loss Sensing

    Recently there have been many studies of power systems with a focus on “New and Renewable Energy” as part of “New Growth Engine Industry” promoted by the Korean government. “New And Renewable Energy”—especially focused on wind energy, solar energy and fuel cells that will replace conventional fossil fuels—is a part of the Power-IT Sector which is the basis of the SmartGrid. A SmartGrid is a form of highly-efficient intelligent electricity network that allows interactivity (two-way communications) between suppliers and consumers by utilizing information technology in electricity production, transmission, distribution and consumption. The New and Renewable Energy Program has been driven with a goal to develop and spread through intensive studies, by public or private institutions, new and renewable energy which, unlike conventional systems, have been operated through connections with various kinds of distributed power generation systems. Considerable research on smart grids has been pursued in the United States and Europe. In the United States, a variety of research activities on the smart power grid have been conducted within EPRI’s IntelliGrid research program. The European Union (EU), which represents Europe’s Smart Grid policy, has focused on an expansion of distributed generation (decentralized generation) and power trade between countries with improved environmental protection. Thus, there is current emphasis on a need for studies that assesses the economic efficiency of such distributed generation systems. In this paper, based on the cost of distributed power generation capacity, calculations of the best profits obtainable were made by a Monte Carlo simulation. Monte Carlo simulations that rely on repeated random sampling to compute their results take into account the cost of electricity production, daily loads and the cost of sales and generate a result faster than mathematical computations. In addition, we have suggested the optimal design, which considers the distribution loss associated with power distribution systems focus on sensing aspect and distributed power generation

    Endometrioid Adenocarcinoma in Urethrovaginal Septum: A Diagnostic Pitfall

    Primary endometrioid adenocarcinoma developed at urethrovaginal septum has not been reported. A 61-yr-old woman presented with recurrent urinary tract infection. She had received hormone replacement treatment with estrogen and progesterone for 5 yr. A pinpoint ulceration at slightly elevated anterior vaginal wall was found and biopsy revealed endometrioid adenocarcinoma. Magnetic resonance imaging showed the 4.3 cm sized mass in urethrovaginal septum. She has undergone anterior pelvic exenteration, pelvic lymph node dissection, and urostomy with ileal conduit. Microscopic finding of the pathology revealed endometrioid adenocarcinoma. Co-existence of endometriosis was not identified. Tumor at urethrovaginal septum was difficult to be detected till growing to be bulky, because of vaginal axis, misunderstanding of the tumor as symphysis pubis, no definitive symptom, and its rarity. This is the first reported case of extraovarian endometrioid adenocarcinoma developed at the urethrovaginal septum. Understanding normal functional anatomy and meticulous physical examination are essential to detect this rare tumor in the urethrovaginal septum

    Satellite radiance data assimilation for binary tropical cyclone cases over the western North Pacific

    A total of three binary tropical cyclone (TC) cases over the Western North Pacific are selected to investigate the effects of satellite radiance data assimilation on analyses and forecasts of binary TCs. Two parallel cycling experiments with a 6 h interval are performed for each binary TC case, and the difference between the two experiments is whether satellite radiance observations are assimilated. Satellite radiance observations are assimilated using the Weather Research and Forecasting Data Assimilation (WRFDA)'s three-dimensional variational (3D-Var) system, which includes the observation operator, quality control procedures, and bias correction algorithm for radiance observations. On average, radiance assimilation results in slight improvements of environmental fields and track forecasts of binary TC cases, but the detailed effects vary with the case. When there is no direct interaction between binary TCs, radiance assimilation leads to better depictions of environmental fields, and finally it results in improved track forecasts. However, positive effects of radiance assimilation on track forecasts can be reduced when there exists a direct interaction between binary TCs and intensities/structures of binary TCs are not represented well. An initialization method (e.g., dynamic initialization) combined with radiance assimilation and/or more advanced DA techniques (e.g., hybrid method) can be considered to overcome these limitations

    Primary Intramedullary Neuroblastoma in Adult

    Primary neuroblastoma in the central nervous system(CNS) is unusual tumor in adult, and primary intramedullary neuroblastoma is extremely rare. The authors report a case of 36-year-old woman presenting with pain in the right anterior chest wall and weakness of the right lower extremity. An upper thoracic intraspinal mass was revealed on magnetic resonance image. On operation, the demarcation plane of the tumor was good and enabled a total removal of the subpially located intramedullary mass. Histopathological examination revealed neuroblastoma. After surgery with whole spinal irradiation, the patient was followed up for one year. There was no evidence of local recurrence radiologically and clinically. However fifteen months after the surgery, multiple metastases to the breast, lung, Iioer and brain occurred
