    Issues Around Researching OHS of Samoan Migrant Workers

    Workers from Pacific nations constitute a substantial proportion of the labour force in NZ, particularly in Auckland, which has one of the largest concentrations of Pacific Island workers in the world. Samoans constitute the largest Pacific ethnic group in NZ, comprising 131,103 or 49% of the resident Pacific population (265,974) (Statistics NZ, 2010). However, Pacific Island workers in NZ are typically employed in low paid, precarious, hazardous work that often has little chance of advancement. There is also some evidence that Pacific Island workers are over­represented in NZ’s work­related injury and illness statistics (Allen & Clarke, 2006). While occupational health and safety (OHS) of Pacific Island migrant workers highlights a number of issues, studies often provide inadequate explanations of what exactly is occurring or fully capture the working experiences of Pacific Island migrant workers. This paper reports on the initial work undertaken as part of an international collaborative study located in Samoa and NZ, aimed at investigating the OHS experiences of Samoan migrant workers. In particular, the paper presents a multi­layered framework and a set of research principles that can be used to illuminate often inaccessible populations located in changing working and living environments. Finally, this study exemplifies the complex issues surrounding the migrant workers’ health and safety, workers’ compensation and rehabilitation

    Controlling tick-borne diseases through domestic animal management: a theoretical approach

    Vector-borne diseases are of global importance to human and animal health. Empirical trials of effective methods to control vectors and their pathogens can be difficult for practical, financial and ethical reasons. Here, therefore, we use a mathematical model to predict the effectiveness of a vector-borne disease control method. As a case study, we use the tick-louping ill virus system, where sheep are treated with acaricide in an attempt to control ticks and disease in red grouse, an economically important game bird. we ran the model under different scenarios of sheep flock sizes, alternative host (deer) densities, acaricide efficacies and tick burdens. The model predicted that, with very low deer densities, using sheep as tick mops can reduce the tick population and virus prevalence. However, treatment is ineffective above a certain threshold deer density, dependent on the comparative tick burden on sheep and deer. The model also predicted that high efficacy levels of acaricide must be maintained for effective tick control. This study suggests that benignly managing one host species to protect another host species from a vector and pathogen can be effective under certain conditions. It also highlights the importance of understanding the ecological complexity of a system, in order to target control methods only under certain circumstances for maximum effectiveness

    Aspects of rabies epidemiology in Tsumkwe District, Namibia

    Aspects of rabies epidemiology were investigated in the Tsumkwe District, Namibia, during December 1993 and January 1994. A cross-sectional seroepidemiological survey for rabies antibodies was carried out in domestic (n = 70) and wild dogs [Lycaon pictus (n = 6)]. An overall seroprevalence rate of 30 % was found in domestic dogs, but it must be borne in mind that seroconversion can result from infections from either rabies or rabies-related viruses. Older dogs were more likely to be seropositive and there was spatial and temporal clustering of seropositivity. No wild dogs were found seropositive. A demographic survey of the domestic-dog population in the area showed that the total dog-population size was 132, or 0,027 dogs per square km . The dog population consisted mainly of young dogs with a median age of 1 ,5 years, and had a female bias of 0,63 males per female. Questionnaire surveys suggested that spotted hyaenas (Crocuta crocuta) and black-backed jackals (Canis mesomelas) were the most common larger carnivores found in and around villages, and that dogs were kept mainly for guarding.The articles have been scanned in colour with a HP Scanjet 5590; 600dpi.     Seroepidemiological survey of sympatric domestic and wild dogs (Lycaon pictus) in Tsumkwe District, north-eastern Namibia

    Disease is a potential threat to many endangered populations and may originate from sympatric domestic species. This paper describes a cross-sectional serological survey of canine pathogens carried out in domestic (n = 70) and wild dogs (Lycaon pictus) (n = 6), in Tsumkwe District, northeastern Namibia. Evidence of past exposure to canine distemper virus, canine adenovirus and parainfluenza virus was evident in both wild and domestic dogs with this, the first, documented exposure of free-living wild dogs to canine distemper. Domestic dogs were also exposed to rabies virus, canine parvovirus and coronavirus. There was no pathogen to which wild dogs, but not domestic dogs, were exposed. With wild dogs known to be susceptible to rabies and canine distemper, these may be the greatest threat to this population of wild dogs, although some wild dogs can clearly survive infection with canine distemper.The articles have been scanned in colour with a HP Scanjet 5590; 600dpi.     Cross infection control measures and the treatment of patients at risk of Creutzfeldt Jakob disease in UK general dental practice

    AIMS: To determine the suitability of key infection control measures currently employed in UK dental practice for delivery of dental care to patients at risk of prion diseases. MATERIALS AND METHODS: Subjects: Five hundred dental surgeons currently registered with the General Dental Council of the UK. Data collection: Structured postal questionnaire. Analysis: Frequencies, cross-tabulations and chi-squared analysis. RESULTS: The valid response rate to the questionnaire was 69%. 33% of practices had no policy on general disinfection and sterilisation procedures. Only 10 of the 327 responding practices (3%) possessed a vacuum autoclave. 49% of dentists reported using the BDA medical history form but less than 25% asked the specific questions recommended by the BDA to identify patients at risk of iatrogenic or familial CJD. However, 63% of practitioners would refer such patients, if identified, to a secondary care facility. Of the 107 practitioners who were prepared to provide dental treatment, 75 (70%) would do so using routine infection control procedures. CONCLUSIONS: Most of the dental practices surveyed were not actively seeking to identify patients at risk of prion diseases. In many cases, recommended procedures for providing safe dental care for such patients were not in place

    Comprehensive Molecular Testing for Respiratory Pathogens in Community-Acquired Pneumonia.

    BACKGROUND: The frequent lack of a microbiological diagnosis in community-acquired pneumonia (CAP) impairs pathogen-directed antimicrobial therapy. This study assessed the use of comprehensive multibacterial, multiviral molecular testing, including quantification, in adults hospitalized with CAP. METHODS: Clinical and laboratory data were collected for 323 adults with radiologically-confirmed CAP admitted to 2 UK tertiary care hospitals. Sputum (96%) or endotracheal aspirate (4%) specimens were cultured as per routine practice and also tested with fast multiplex real-time polymerase-chain reaction (PCR) assays for 26 respiratory bacteria and viruses. Bacterial loads were also calculated for 8 bacterial pathogens. Appropriate pathogen-directed therapy was retrospectively assessed using national guidelines adapted for local antimicrobial susceptibility patterns. RESULTS: Comprehensive molecular testing of single lower respiratory tract (LRT) specimens achieved pathogen detection in 87% of CAP patients compared with 39% with culture-based methods. Haemophilus influenzae and Streptococcus pneumoniae were the main agents detected, along with a wide variety of typical and atypical pathogens. Viruses were present in 30% of cases; 82% of these were codetections with bacteria. Most (85%) patients had received antimicrobials in the 72 hours before admission. Of these, 78% had a bacterial pathogen detected by PCR but only 32% were culture-positive (P < .0001). Molecular testing had the potential to enable de-escalation in number and/or spectrum of antimicrobials in 77% of patients. CONCLUSIONS: Comprehensive molecular testing significantly improves pathogen detection in CAP, particularly in antimicrobial-exposed patients, and requires only a single LRT specimen. It also has the potential to enable early de-escalation from broad-spectrum empirical antimicrobials to pathogen-directed therapy

    Mind the Gap: Rigour and Relevance in Collaborative Heritage Science Research

    Background: This paper examines if there is a ‘rigour-relevance gap’ in collaborative heritage science research and what enables and impedes effective collaboration between academic researchers and users of research evidence in practice. A quantitative attitudes questionnaire was distributed amongst the heritage science community: 210 responses were received. Respondents answered in relation to one project they had worked on in the UK in the previous five years. They were asked about their personal goals in relation to the project and whether these were achieved, satisfaction levels in relation to project outcomes and impact, level of agreement with a series of attitude statements to ascertain what helped and hindered projects, personal characteristics and project characteristics. The questionnaire was analysed using a factor analytic, segmentation and profiling approach. Results: Most respondents sought both rigour and relevance in collaborative research, were generally satisfied with the outcomes of projects and reported positive experiences of collaboration. However, respondents were less satisfied that the impact of projects would be realised. Practice-focussed goals were associated with lower levels of self-rated achievement than academic goals and a sizeable minority reported challenges in collaborative research. Furthermore, researchers and users differed in terms of their goals, experiences and level of satisfaction. Users had high expectations that research would translate into practice, but did not always feel this was realised. Conclusion: Results from this project will inform how to improve heritage science research collaborations

    A sub-group of patients with hospital-acquired pneumonia do not require broad-spectrum gram-negative antimicrobial coverage

    C.D.R. is supported by an Edinburgh Clinical Academic Track (ECAT)/Wellcome Trust PhD Training Fellowship for Clinicians award (214178/Z/18/Z).Among 200 patients developing hospital-acquired pneumonia (HAP) outside the intensive care unit, 61% were treated empirically without broad-spectrum Gram-negative coverage, with clinical cure in 69.7%. Lower disease severity markers (systemic inflammatory response syndrome, hypoxia, tachypnoea, neutrophilia) and the absence of diabetes mellitus and prior doxycycline treatment (but not the time to HAP onset) identified patients not requiring broad-spectrum Gram-negative coverage.Publisher PDFPeer reviewe
