Difficulties in recognizing granulomatosis with polyangiitis (GPA) in elderly patients undergoing diagnostic thoracotomy twice — a report of two cases
Granulomatosis with polyangiitis (GPA) is defined as a necrotizing granulomatous inflammation usually involving the upper and lower respiratory tract with necrotizing vasculitis affecting predominantly small to medium vessels. Because of non-specific symptoms, its radiological presentation, and the diversity of its clinical expression, it is not uncommon to for it to be misdiagnosed, especially in the elderly. Although biopsy and histological examination seem to be essential for GPA diagnosis, their results are sometimes ambiguous and not helpful in making a decision. In this report, we present difficulties in the recognition of GPA in two elderly patients in whom, despite twice performing a diagnostic thoracotomy, GPA was recognized almost 4 and 6 years after the first symptoms
Benign pulmonary metastasizing leiomyoma uteri. Case report and review of literature
Benign metastasizing leiomyoma (BML) is a rare condition in middle-aged women with a history of uterine leiomyomata. It is
characterized by the proliferation of, usually multiple, smooth muscle nodules. Approximately 100 cases have been reported
in the literature, and the lungs were the most common site of metastases.
We report a case of 52-year-old obese woman (BMI 31), hospital worker, smoker, admitted to the hospital with exertional
dyspnoea, night sweats, loss of weight, and productive cough.
Hysterectomy for a uterine leiomyoma was performed 9 years earlier. In addition, a history of two episodes of superficial
vein thrombosis 3 and 2 years before admission was noted. Chest X-ray and subsequently CT chest examinations revealed
multiple, non-calcified nodules within the middle and lower parts of both lungs. Specimens obtained by transbronchial
biopsy (TBLB) and from open lung biopsy displayed benign muscle cell proliferation compatible with BML. The levels of sex
hormones were characteristic for the menopause; therefore, observation was advised. Additionally, Streptococcus
pneumoniae was cultured from bronchial washing, and bronchitis was diagnosed. Antibiotics, bronchodilators, and
mucolytics were administered, and dyspnoea and cough with expectoration were diminished. Two years later pulmonary
lesions have been stable; however, she has put on weight. Subsequently the patient has developed deep vein thrombosis
with pulmonary embolism. Anticoagulant treatment was introduced, with some improvement
Pulmonary pathology in patients with ulcerative colitis treated with mesalazine — a challenging and complex diagnostic problem. Case series and literature review
Pulmonary involvement in the course of inflammatory bowel disease has been a subject of interest to clinicians for long time, but despite this, its epidemiology and potential pathomechanisms remain obscured. Equally unclear is the role of medications used for bowel disease treatment in lung disease development. We present three patients with ulcerative colitis, all treated with mesalazine, in whom unexplained lung disease developed. Due to different clinical and radiological presentation, different conditions were initially placed on the top of the differential list in each of them. The outcome was favourable in all patients despite differences in management. We compared our patients with similar cases from literature. We show the level of difficulty and complexity in the issue of lung disease in patients with inflammatory bowel disease.Zagadnienie zajęcia płuc w przebiegu nieswoistych zapaleń jelit pozostaje przedmiotem zainteresowania klinicystów od dawna. Pomimo to epidemiologia tego zjawiska ani potencjalne patomechanizmy nie są znane. Niewyjaśniona jest także rola leków stosowanych w zapaleniach jelit w rozwoju patologii płucnej. W pracy przedstawiono troje pacjentów z wrzodziejącym zapaleniem jelita grubego leczonych mesalazyną, u których rozwinęła się choroba płuc. Z powodu różnej prezentacji klinicznej i radiologicznej, wstępna diagnostyka u każdego z nich ukierunkowana była na inne jednostki chorobowe. Zastosowane postępowanie u każdego z nich również było inne. Dokonaliśmy porównania naszych pacjentów do podobnych przypadków opisanych w literaturze. Opisani przez nas pacjenci dobrze obrazują, jak trudne i złożone jest zagadnienie chorób płuc rozwijających się u pacjentów z nieswoistymi zapaleniami jelit

Leflunomide-induced acute interstitial pneumonia in a patient treated for rheumatoid arthritis
Leflunomid należy do leków przeciwreumatycznych modyfikujących przebieg choroby. Jest on stosowany u chorych z reumatoidalnym zapaleniem stawów, u których leczenie pierwszej linii było nieskuteczne. Leczenie leflunomidem może być jednak związane z istotnymi powikłaniami płucnymi, zwłaszcza u osób predysponowanych. Autorzy przedstawiają opis przypadku chorego na reumatoidalne zapalenie stawów leczonego leflunomidem, u ktorego wystąpiły powikłania ze strony układu oddechowego. Omówiono czynniki ryzyka wystąpienia uszkodzenia płuc w przebiegu leczenia tym preparatem oraz sposób postępowania w przypadku ich wystąpienia.Leflunomide is a disease-modifying anti-rheumatic drug that is used in patients with rheumatoid arthritis (RA), who do not respond well to standard RA treatment. Leflunomide therapy may, however, be related with significant pulmonary complications in predisposed individuals. We present a patient with RA treated with leflunomide, in whom leflunomide lung injury had a fatal outcome. Potential risk factors for pulmonary complications of leflunomide treatment and the management of patients with leflunomide lung injury are discussed
Fetal adenocarcinoma — a rare variant of lung adenocarcinoma with good prognosis
Fetal adenocarcinoma (FA) is a rare variant of lung adenocarcinoma with a good prognosis which represents approximately 0.5% of all primary lung neoplasms. The tumour has nonciliated tubules with morules in the lumen resembling fetal lung at 10 to 16 weeks of gestation. FA typically occurs in young patients 75–80% of whom are tobacco smokers. FA occurs in low- and high-grade forms. Surgical procedure is usually necessary to obtain final diagnosis and a complete surgical resection is the standard treatment. The 5-year survival rate for FA is about 75–80%. We report a case of 32-year-old male, a smoker, who had an incomplete regression of parenchymal consolidations after antibiotic treatment, which led to further diagnostics. An intravenous tumour in one of the pulmonary veins with concomitant development of collateral circulation and pulmonary venous infarction were additional diagnostic problems. Based on the intraoperative microscopic examination, fetal adenocarcinoma was diagnosed. An upper right bilobectomy with intrapericardial ligation of the right superior pulmonary vein was performed as radical treatment. At the 9-month follow-up stage, there has been no evidence of disease recurrence.
Pulmonary epithelioid haemangioendothelioma - interferon 2-alpha treatment - case report
W pracy przedstawiono przypadek 62-letniej chorej, otyłej palaczki tytoniu (10 paczko/lat), przyjętej do Instytutu Gruźlicy i Chorób Płuc
w Warszawie w celu diagnostyki stwierdzanych w badaniu radiologicznym klatki piersiowej dobrze wysyconych cieni okrągłych.
Zmiany te były widoczne w badaniach radiologicznych od 5 lat, jednak w ostatnim okresie uległy zdecydowanemu powiększeniu.
Chora leczyła się z powodu nadciśnienia tętniczego, cukrzycy insulinoniezależnej oraz łuszczycy z zajęciem skóry. W chwili przyjęcia
uskarżała się na kaszel z niewielkim odkrztuszaniem śluzowej wydzieliny i okresowe bóle kostno-stawowe. W badaniu przedmiotowym
stwierdzono zmiany skórne o charakterze łuszczycowym, niewielkie obrzęki na kończynach dolnych oraz palce pałeczkowate.
Odczyn tuberkulinowy był dodatni. W badaniu tomograficznym klatki piersiowej wykazano obecność częściowo uwapnionych
guzków o średnicy około 1 cm zlokalizowanych głównie w dolnych i środkowych polach płucnych bez powiększenia węzłów
chłonnych wnęk i śródpiersia. Badaniem histologicznym wycinków uzyskanych drogą biopsji otwartej płuca stwierdzono obecność
w obrębie pęcherzyków i naczyń płucnych nacieku nowotworu pochodzenia naczyniowego. Komórki tego guza wykazywały
obecność antygenów: czynnika VIII, CD31 i CD34, co pozwoliło postawić rozpoznanie haemangioendothelioma epithelioides płuc.
Po 6 miesiącach od biopsji otwartej płuca doszło do istotnej progresji choroby, co było powodem podjęcia próby leczenia interferonem
2-alfa. W trakcie leczenia zaobserwowano nieznaczną regresję zmian, a przez następne 6 miesięcy stabilizację choroby.A 62-year-old, obese woman, smoking 10 pack/year was admitted to the National Tuberculosis and Lung Diseases Research Institute
to diagnose small, round opacities revealed by routine chest X-ray examination. These lesions had been observed for 5 years. The
patient had been treated for psoriasis, hypertension, and insulin-independent diabetes. On admission she was in good condition,
complaining of a slight productive cough as well as intermittent osteoarticular pain. Physical examination revealed cutaneous psoriatic
lesions, slight edema of the lower limbs, and clubbed fingers. Tuberculin test was positive.
Chest Computer Tomography scanning showed partially calcified nodules (up to 1cm in diameter) located in the middle and
base areas of both lungs. No evidence of hilar nor mediastinal lymph node enlargement was seen. Lung specimens displayed
intraalveolar and intravascular growth of neoplastic cells. Immunohistochemical expression of Factor VIII, CD31 and CD34
antigens was present. Pulmonary epithelioid haemangioendothelioma was diagnosed.
After 6 months of observation, progression of the disease was shown. Interferon alpha treatment was introduced. During the
therapy, a slight regression of pulmonary changes was noticed and since then stabilization of the disease was observed
Pulmonary neuroendocrine tumors. The spectrum of histologic subtypes and current concept on diagnosis and treatment
Neuroendocrine tumors of the lung represent a broad spectrum of morphologic types that share specific morphologic,
immunohistochemical, ultrastructural, and molecular characteristics. The classification of neuroendocrine lung tumors has
changed over the last decades and currently four categories are distinguished: typical carcinoid tumor, atypical carcinoid
tumor, large cell neuroendocrine carcinoma and small cell carcinoma.
Neuroendocrine tumors of the lung comprise approximately 20% of all primary lung cancers. Among them, the most frequent
is small cell carcinoma (13-17%). Because of differences in clinical behavior, therapy, and prognosis, a reliable histological
diagnosis, as well as clinical and pathological staging system are essential for an appropriate medical proceedings. The
most effective treatment of bronchial carcinoids and large cell neuroendocrine carcinoma in an early stage is complete
surgical resection, whereas chemotherapy remains the primary treatment for small cell carcinoma.
All carcinoids are malignant tumors with the potential to metastasize. The majority of patients with pulmonary carcinoid
have an excellent survival, even if they present with lymph node metastases. Large cell neuroendocrine and small cell
carcinoma progress rapidly and are generally widespread at the moment of diagnosis. Their overall prognosis is poor.
Increased knowledge about pulmonary neuroendocrine tumors biology and the genetic characteristics, imply that carcinoid
tumors appear to have a different etiology and pathogenesis than large cell neuroendocrine and small cell carcinoma.
In practice, it could be easiest to conceptualize this group of pulmonary tumors as a spectrum of malignancy ranging from
the low grade typical carcinoid to the highly malignant large cell neuroendocrine and small cell carcinoma. Typical carcinoid
tumors associated with a fairly benign behavior should be classified as low-grade neuroendocrine tumor/carcinoma (G1) and
atypical carcinoid tumors as intermediate-grade tumor/carcinoma (G2). Whereas, large cell neuroendocrine and small cell
carcinoma should be grouped together under the designation of high-grade neuroendocrine tumor/carcinoma (G3).Rozrosty neuroendokrynne w płucach stanowią specyficzną grupę zmian, głównie nowotworowych, charakteryzujących się
wspólną morfologią, budową ultrastrukturalną, reakcjami immunohistochemicznymi i biologią molekularną. W ciągu ostatnich
lat klasyfikacja nowotworów neuroendokrynnych płuc wielokrotnie ulegała zmianom. Obecnie wyodrębniono 4 typy
nowotworów neuroendokrynnych, wśród których znalazły się rakowiaki typowe i atypowe, raki wielkokomórkowe neuroendokrynne
oraz raki drobnokomórkowe. Nowotwory neuroendokrynne stanowią około 20% pierwotnych złośliwych guzów kwapłuc.
Wśród nich największy procent stanowią raki drobnokomórkowe (13-17%). Właściwe rozpoznanie histopatologiczne,
określenie stopnia zaawansowania klinicznego i patologicznego choroby są istotne ze względu na różny przebieg, rokowanie
i odmienne postępowanie terapeutyczne. Rakowiaki i raki wielkokomórkowe neuroendokrynne są zwykle leczone operacyjnie,
natomiast raki drobnokomórkowe - chemioterapią. Rakowiaki, zwłaszcza typowe, należą do grupy złośliwych nowotworów
neuroendokrynnych o dobrym rokowaniu, nawet w przypadkach, w których stwierdza się przerzuty w okolicznych
węzłach chłonnych. Raki wielkokomórkowe neuroendokrynne i drobnokomórkowe odpowiadają postaciom o niezwykle
agresywnym przebiegu, złym rokowaniu i o podobnym czasie przeżycia. Zarówno badania molekularne, jak i genetyczne
wskazują na udział odmiennych mechanizmów leżących u podstaw rozwoju rakowiaków oraz raków wielkokomórkowych
neuroendokrynnych i drobnokomórkowych płuca. W związku z tym proponowana jest klasyfikacja nowotworów neuroendokrynnych
w zależności od wzrastającego stopnia złośliwości. Najlepiej rokująca grupa typowych rakowiaków odpowiadałaby
nowotworom neuroendokrynnym o niskim stopniu złośliwości (G1), atypowe rakowiaki - nowotworom o pośrednim
stopniu złośliwości (G2), a raki wielkokomórkowe neuroendokrynne i drobnokomórkowe stanowiłyby grupę nowotworów
neuroendokrynnych o wysokim stopniu złośliwości (G3)
