    Cancer patients' attitudes towards Chinese medicine: a Hong Kong survey

    <p>Abstract</p> <p>Background</p> <p>This article reports a survey conducted in Hong Kong on the cancer patients' attitudes towards Chinese medicine treatment.</p> <p>Methods</p> <p>Cancer patients from three Chinese medicine clinics and one oncology clinic were interviewed with a structured questionnaire.</p> <p>Results</p> <p>Of a total of 786 participants included in the study, 42.9% used Western medicine only; 57.1% used at least one form of Chinese medicine; 5 participants used Chinese medicine only; and 56.5% used Chinese medicine before/during/after Western medicine treatment. Commonly used Western medicine and Chinese medicine treatments included chemotherapy (63.7%), radiotherapy (62.0%), surgery (57.6%), Chinese herbal medicine (53.9%) and Chinese dietary therapy (9.5%). Participants receiving chemotherapy used Chinese medicine (63.3%) more than those receiving any other Western medicine treatments. Spearman correlation coefficients showed that the selection of Chinese medicine was associated with the cancer type (r<sub>s </sub>= -1.36; <it>P </it>< 0.001), stage (r<sub>s </sub>= 0.178; <it>P </it>< 0.001), duration (r<sub>s </sub>= -0.074; <it>P </it>= 0.037), whether receiving chemotherapy (r<sub>s </sub>= 0.165; <it>P </it>< 0.001) and palliative therapy (r<sub>s </sub>= 0.087; <it>P </it>= 0.015). Nearly two-thirds of the participants (N = 274) did not tell their physicians about using Chinese medicine. Over two-thirds of all participants (68.2%) believed that integrated Chinese and Western medicine was effective.</p> <p>Conclusion</p> <p>Chinese medicine is commonly used among Hong Kong cancer patients. The interviewed cancer patients in Hong Kong considered integrative Chinese and Western medicine is an effective cancer treatment.</p

    STUDENT-CONTROLLED SOCIAL NETWORKS FOR PROMOTING HOLISTIC DEVELOPMENT FROM THE PRESPECTIVES OF STUDENT COACHES

    Many previous research works have studied the impact of online social networks for educational purposes. We examine in particular how Facebook is being used as a platform to communicate among students of an on-going student development project run by a local tertiary institute in Hong Kong so as to facilitate promotion and foster participation and interaction. The study focuses on the perspectives from student coaches and evaluate on the facilitation and difficulties in promoting self-initiated holistic development via Facebook. The study shows that instant interaction between participants and student coaches via Facebook can lead to information circulation in a much faster and effective manner compared with traditional communication channels such as email or bulletins. However, limitations are found on the lack of proactive discussions initiated by participants, and the difficulties in establishing active interactions between coaches and participants. This has undermined the effectiveness of promoting to participants’ in self-initiated holistic development

    A Randomized Controlled Trial of Auricular Transcutaneous Electrical Nerve Stimulation for Managing Posthysterectomy Pain

    Background. A patient- and assessor-blinded randomized controlled trial was conducted to examine the effectiveness of auricular transcutaneous electrical nerve stimulation (TENS) in relieving posthysterectomy pain. Method. Forty-eight women who had undergone a total abdominal hysterectomy were randomly assigned into three groups (n = 16 each) to receive either (i) auricular TENS to therapeutic points (the true TENS group), (ii) auricular TENS to inappropriate points (the sham TENS group), or (iii) 20 minutes of bed rest with no stimulation (the control group). The intervention was delivered about 24 hours after the operation. A visual analogue scale was used to assess pain while resting (VAS-rest) and upon huffing (VAS-huff) and coughing (VAS-cough), and the peak expiratory flow rate (PEFR) was assessed before and at 0, 15, and 30 minutes after the intervention. Result. As compared to the baseline, only the true TENS group reported a significant reduction in VAS-rest (P = .001), VAS-huff (P = .004), and VAS-cough (P = .001), while no significant reduction in any of the VAS scores was seen in the sham TENS group (all P > .05). In contrast, a small rising trend was observed in the VAS-rest and VAS-huff scores of the control group, while the VAS-cough score remained largely unchanged during the period of the study. A between-group comparison revealed that all three VAS scores of the true TENS group were significantly lower than those of the control group at 15 and 30 minutes after the intervention (all P < .02). No significant between-group difference was observed in PEFR at any point in time. Conclusion. A single session of auricular TENS applied at specific therapeutic points significantly reduced resting (VAS-rest) and movement-evoked pain (VAS-huff, VAS-cough), and the effects lasted for at least 30 minutes after the stimulation. The analgesic effects of auricular TENS appeared to be point specific and could not be attributed to the placebo effect alone. However, auricular TENS did not produce any significant improvement in the performance of PEFR

    Effective Photodegradation of Methyl Orange Using Fluidized Bed Reactor Loaded with Cross-Linked Chitosan Embedded Nano-CdS Photocatalyst

    Chitosan-based photocatalyst composites containing CdS nanocrystals with and without glutaraldehyde or epichlorohydrin cross-linking treatments were investigated and the catalyzed photodegradation of methyl orange in aqueous solution was examined. In addition, the effects of catalyst dosage, initial dye concentration, and initial pH of the dye solution on the photodegradation kinetics were investigated. In this study, the effect of initial solution pH was more important than other factors. The photocatalyst composite could remove 99% dye in 80 minutes at pH 4. The catalyst composite was characterized by using X-ray diffraction (XRD), transmission electron microscopy (TEM), thermogravimetric analysis (TGA), fourier transform infrared (FT-IR), and visible reflectance spectroscopy. The dye removal mechanism of methyl orange involved an initial sorption process followed by photodegradation. The sorption process underwent the pseudo-second order kinetics, while photodegradation followed the Langmuir-Hinshelwood kinetics. Although the glutaraldehyde cross-linked chitosan enhanced the initial dye sorption, the epichlorohydrin cross-linked catalyst composite demonstrated a better overall dye removal performance, especially in the photodegradation step. Both chitosan encapsulated catalyst with and without epichlorohydrin cross-linking demonstrated the same pseudo-first order photodegradation kinetic constant of 0.026 min−1 and the same dye removal capacity. The catalyst composite could be reused but the photocatalytic activity dropped successively in each cycle

    Treatment of Idiopathic Parkinson's Disease with Traditional Chinese Herbal Medicine: A Randomized Placebo-Controlled Pilot Clinical Study

    The objective of this clinical study is to examine the effects of a Chinese herbal medicine formula (Jia Wei Liu Jun Zi Tang: JWLJZT) on motor and non-motor symptoms, and on complications of conventional therapy in idiopathic Parkinson's disease (PD), using an add-on design. Fifty-five patients with PD were randomly allocated to receive either Chinese herbal medicine or placebo for 24 weeks. Primary outcome measure was the 39-item Parkinson's Disease Questionnaire (PDQ-39). Secondary outcome measures included the Unified Parkinson's Disease Rating Scale (UPDRS), Short-Form-36 Health Survey (SF-36), Geriatric Depression Scale (GDS), home diaries, and a range of category rating scales. JWLJZT resulted in a significant improvement in the UPDRS IVC when compared with placebo at 12 weeks (P = .039) and 24 weeks (P = .034). In addition, patients in the Chinese herbal medicine group also showed significant improvement in PDQ-39 communication scores at 12 weeks (P = .024) and 24 weeks (P = .047) when compared with the placebo group. There were no significant differences between treatment and control groups for SF-36 variables, GDS score or the mean daily “on-off” time. One case of mild diarrhea was noted in the treatment group. The findings suggest that JWLJZT can relieve some non-motor complications of conventional therapy and improve the communication ability in patients with PD. The results of this pilot study warrant larger multi-center clinical studies to assess long-term efficacy and tolerability of JWLJZT, and to elucidate the mechanisms by which it affects PD function

    A proof-of-concept study on endoscopic ultrasound-guided biopsy of detrusor muscle in porcine bladders

    IntroductionConventionally, we rely on transurethral resection of bladder tumour (TURBT) for local staging of muscle-invasive bladder cancer (MIBC). However, the procedure is limited by its staging inaccuracy which may delay the definitive treatment of MIBC.MethodsWe conducted a proof-of concept study on endoscopic ultrasound (EUS)-guided biopsy of detrusor muscle in porcine bladders. Five porcine bladders were used in this experiment. Upon EUS, four layers of tissue including the mucosa (hypoechoic), submucosa (hyperechoic), detrusor muscle (hypoechoic) and serosa (hyperechoic) could be identified.ResultsA total of 37 EUS-guided biopsies were taken from 15 sites (three sites per bladder), and the mean number of biopsies taken from each site was 2.47±0.64. Among the 37 biopsies, 30 of them (81.1%) obtained detrusor muscle in the biopsy specimen. For the per biopsy site analysis, detrusor muscle was obtained in 73.3% if only one biopsy was taken, and 100% if two or more biopsies were taken from the same biopsy site. Overall, detrusor muscle was successfully obtained from all 15 biopsy sites (100%). No bladder perforation was observed throughout all biopsy processes.ConclusionEUS-guided biopsy of the detrusor muscle could be performed during the initial cystoscopy session, thus expediting the histological diagnosis and subsequent treatment of MIBC

    A single residue substitution in the receptor-binding domain of H5N1 hemagglutinin is critical for packaging into pseudotyped lentiviral particles

We have previously characterized lentiviral particles pseudotyped with H5-HA (H5pp) and validated an H5pp-based assay as a safe alternative for high-throughput serological studies in BSL-2 facilities. Here we show that H5-HAs from different clades do not always give rise to efficient production of H5pp and the underlying mechanisms are addressed. Methodology/Findings: We have carried out mutational analysis to delineate the molecular determinants responsible for efficient packaging of HA from A/Cambodia/40808/2005 (H5Cam) and A/Anhui/1/2005 (H5Anh) into H5pp. Our results demonstrate that a single A134V mutation in the 130-loop of the receptor binding domain is sufficient to render H5Anh the ability to generate H5Anh-pp efficiently, whereas the reverse V134A mutation greatly hampers production of H5Cam-pp. Although protein expression in total cell lysates is similar for H5Anh and H5Cam, cell surface expression of H5Cam is detected at a significantly higher level than that of H5Anh. We further demonstrate by several independent lines of evidence that the behaviour of H5Anh can be explained by a stronger binding to sialic acid receptors implicating residue 134. Conclusions: We have identified a single A134V mutation as the molecular determinant in H5-HA for efficient incorporation into H5pp envelope and delineated the underlying mechanism. The reduced binding to sialic acid receptors as a result of the A134V mutation not only exerts a critical influence in pseudotyping efficiency of H5-HA, but has also an impact at the whole virus level.
