Goffman, Action, and Risk Society: Aesthetic Reflexivity in Late Modernity
When I decided to write about edgework over twenty-five years ago, I had no idea that I had put myself on a pathway of sustained engagement with one of the most important sociologists of the twentieth century. As an academic sociologist in the early stages of my career, I felt that I had a good general understanding of Erving Goffman’s most important ideas and a proper appreciation of his unique contributions to my discipline. Although I had a certain affinity for Goffman’s brand of sociology, I deliberately avoided pressure from close friends and colleagues to become a “Goffmaniac,” the kind of disciple that Goffman’s work often seems to inspire. However, when I took up the task of formulating a theoretical explanation of volitional risk taking, I had no choice but to dig more deeply into Goffman’s work, in particular, his famous essay entitled Where the Action Is (1967), which at that time was one of the very few sociological theories of risk-taking behavior. The most immediate result of this excavation was a better understanding of the theoretical differences between my concept of edgework and Goffman’s notion of action, but the long term consequence was a profound appreciation of the power and relevance of Goffman’s ideas for making sense of the contemporary social world
Raman Spectroscopy of Liquid-Based Cervical Smear Samples as a Triage to Stratify Women Who Are HPV-Positive on Screening
Persistent high-risk human papillomavirus (HPV) infection can lead to cervical precancer and cancer. Recently, HPV testing has been introduced for primary cervical screening, but the HPV DNA test cannot distinguish between transient and persistent HPV infection. Thus, there is an unmet clinical need to develop a new test to identify women with a high-risk persistent HPV infection. Raman spectra were recorded from cervical smear samples (n = 60) and, on the basis of HPV DNA and HPV mRNA test results, a classifier was developed to identify persistent HPV infection. A further blinded independent test set (n = 14) was used to validate the model, and sensitivity of 90% and specificity of 100% were achieved. Improved triage would allow women with a high-risk persistent HPV infection to be referred for immediate treatment, while women with a low-risk transient infection could avoid overtreatment
The role of collaborative learning in resilience in healthcare—a thematic qualitative meta‑synthesis of resilience narratives.
Background
To provide high quality services in increasingly complex, constantly changing circumstances, healthcare organizations worldwide need a high level of resilience, to adapt and respond to challenges and changes at all system levels. For healthcare organizations to strengthen their resilience, a significant level of continuous learning is required. Given the interdependence required amongst healthcare professionals and stakeholders when providing healthcare, this learning needs to be collaborative, as a prerequisite to operationalizing resilience in healthcare. As particular elements of collaborative working, and learning are likely to promote resilience, there is a need to explore the underlying collaborative learning mechanisms and how and why collaborations occur during adaptations and responses. The aim of this study is to describe collaborative learning processes in relation to resilient healthcare based on an investigation of narratives developed from studies representing diverse healthcare contexts and levels.
Methods
The method used to develop understanding of collaborative learning across diverse healthcare contexts and levels was to first conduct a narrative inquiry of a comprehensive dataset of published health services research studies. This resulted in 14 narratives (70 pages), synthesised from a total of 40 published articles and 6 PhD synopses. The narratives where then analysed using a thematic meta-synthesis approach.
Results
The results show that, across levels and contexts, healthcare professionals collaborate to respond and adapt to change, maintain processes and functions, and improve quality and safety. This collaboration comprises activities and interactions such as exchanging information, coordinating, negotiating, and aligning needs and developing buffers. The learning activities embedded in these collaborations are both activities of daily work, such as discussions, prioritizing and delegation of tasks, and intentional educational activities such as seminars or simulation activities.
Conclusions
Based on these findings, we propose that the enactment of resilience in healthcare is dependent on these collaborations and learning processes, across different levels and contexts. A systems perspective of resilience demands collaboration and learning within and across all system levels. Creating space for reflection and awareness through activities of everyday work, could support individual, team and organizational learning.publishedVersio
A Pilot Study for Early Detection of Oral Premalignant Diseases Using Oral Cytology and Raman Micro-Spectroscopy: Assessment of Confounding Factors
This study demonstrates the efficacy of Raman micro-spectroscopy of oral cytological samples for differentiating dysplastic, potentially malignant lesions from those of normal, healthy donors. Cells were collected using brush biopsy from healthy donors (n = 20) and patients attending a Dysplasia Clinic (n = 20). Donors were sampled at four different sites (buccal mucosa, tongue, alveolus, gingiva), to ensure matched normal sites for all lesions, while patient samples were taken from clinically evident, histologically verified dysplastic lesions. Spectra were acquired from the nucleus and cytoplasm of individual cells of all samples and subjected to partial least squares-discriminant analysis. Discriminative sensitivities of 94% and 86% and specificity of 85% were achieved for the cytoplasm and nucleus, respectively, largely based on lipidic contributions of dysplastic cells. Alveolar/gingival samples were differentiated from tongue/buccal samples, indicating that anatomical site is potentially a confounding factor, while age, gender, smoking and alcohol consumption were confirmed not to be
Raman microspectroscopic study for the detection of oral field cancerisation using brush biopsy samples
Field cancerisation (FC) is potentially an underlying cause of poor treatment outcomes of oral squamous cell carcinoma (OSCC). To explore the phenomenon using Raman microspectroscopy, brush biopsies from the buccal mucosa, tongue, gingiva and alveolus of healthy donors (n = 40) and from potentially malignant lesions (PML) of Dysplasia Clinic patients (n = 40) were examined. Contralateral normal samples (n = 38) were also collected from the patients. Raman spectra were acquired from the nucleus and cytoplasm of each cell, and subjected to partial least squares-discriminant analysis (PLS-DA). High discriminatory accuracy for donor and PML samples was achieved for both cytopalmic and nuclear data sets. Notably, contralateral normal (patient) samples were also accurately discriminated from donor samples and contralateral normal samples from patients with multiple lesions showed a similar spectral profile to PML samples, strongly indicating a FC effect. These findings support the potential of Raman microspectroscopy as a screening tool for PML using oral exfoliated cells
Audience Participation and Neoliberal Value: Risk, Agency and Responsibility in Immersive Theatre.
This article identifies a value set shared between the neoliberal ethos and modes of audience participation frequently promoted in immersive theatre: values such as risk-taking, individual freedoms and personal responsibility. The promotion of self-made opportunity, premised either on opportunistic risk-taking, or the savvy attitude that comes with experience and familiarity with immersive theatre participation, will be addressed as valorising another shared value: entrepreneurialism. A participatory mode will be introduced that I call ‘entrepreneurial participation’: a kind of audience participation privileged in much immersive theatre performance identifying the enactment of neoliberal value. While entrepreneurial participation may be deliberately deployed by audiences as a participatory tactic, it will be argued that this particular participatory mode is frequently expected of audiences, or at least privileged as a means of engaging with performance. Work by the British immersive theatre company Punchdrunk will be taken as a means of illustrating this suggestion, particularly The Masque of the Red Death (2007). The article begins with a definition of immersive theatre that focuses on the figuring of participating audiences, paying particular attention to the relativity of participatory freedoms and the centrality of experience production. Hedonistic and narcissistic experiences will pull focus and will be approached as a possible reason behind immersive theatre's susceptibility to absorption within the experience industry and co-optation by innovative marketers. The article then establishes a set of shared values between the neoliberal ethos and audience participation in The Masque of the Red Death. Risk perception research, especially that arising from the Oregon Group and Stephen Lyng, will be touched on as a means of introducing some political considerations arising from the notion of entrepreneurial participation. A more optimistic, but ultimately sobering set of responses will be offered in conclusion
Embodiment in High-altitude Mountaineering: Sensing and Working with the Weather
In order to address sociological concerns with embodiment and learning, in this article we explore the ‘weathering’ body in a currently under-researched physical-cultural domain. Weather experiences too are under-explored in sociology, and here we examine in-depth the lived experience of weather, and more specifically ‘weather work’ and ‘weather learning’, in one of the most extreme and corporeally-challenging environments on earth: high-altitude mountains. Drawing on a theoretical framework of phenomenological sociology, and an interview-based research project with 19 international, high-altitude mountaineers, we investigate weather as lived and experienced both corporeally and cognitively. We are particularly interested in conceptualising and theorising the ways in which embodied beings relate to the environment through different aspects of their being. The novel concepts of ‘weather work’ and ‘weather learning’, we argue, provide salient examples of the mind-body-world nexus at work, as an embodied practice and mode of thinking, strongly contoured by the physical culture of high-altitude mountaineering
Microarray scanner calibration curves: characteristics and implications
BACKGROUND: Microarray-based measurement of mRNA abundance assumes a linear relationship between the fluorescence intensity and the dye concentration. In reality, however, the calibration curve can be nonlinear. RESULTS: By scanning a microarray scanner calibration slide containing known concentrations of fluorescent dyes under 18 PMT gains, we were able to evaluate the differences in calibration characteristics of Cy5 and Cy3. First, the calibration curve for the same dye under the same PMT gain is nonlinear at both the high and low intensity ends. Second, the degree of nonlinearity of the calibration curve depends on the PMT gain. Third, the two PMTs (for Cy5 and Cy3) behave differently even under the same gain. Fourth, the background intensity for the Cy3 channel is higher than that for the Cy5 channel. The impact of such characteristics on the accuracy and reproducibility of measured mRNA abundance and the calculated ratios was demonstrated. Combined with simulation results, we provided explanations to the existence of ratio underestimation, intensity-dependence of ratio bias, and anti-correlation of ratios in dye-swap replicates. We further demonstrated that although Lowess normalization effectively eliminates the intensity-dependence of ratio bias, the systematic deviation from true ratios largely remained. A method of calculating ratios based on concentrations estimated from the calibration curves was proposed for correcting ratio bias. CONCLUSION: It is preferable to scan microarray slides at fixed, optimal gain settings under which the linearity between concentration and intensity is maximized. Although normalization methods improve reproducibility of microarray measurements, they appear less effective in improving accuracy
