Prevalence of mixed genotype hepatitis C virus infections in the UK as determined by genotypeâspecific PCR and deep sequencing
The incidence of mixed genotype hepatitis C virus infections in the UK is largely unknown. As the efficacy of direct acting antivirals is variable across different genotypes, treatment regimens are tailored to the infecting genotype, which may pose issues for the treatment of underlying genotypes within undiagnosed mixed genotype HCV infections. There is therefore a need to accurately diagnose mixed genotype infections prior to treatment. PCR-based diagnostic tools were developed to screen for the occurrence of mixed genotype infections caused by the most common UK genotypes, 1a and 3, in a cohort of 506 individuals diagnosed with either of these genotypes. The overall prevalence rate of mixed infection was 3.8% however this rate was unevenly distributed, with 6.7% of individuals diagnosed with genotype 3 harbouring genotype 1a strains and only 0.8% of samples from genotype 1a patients harbouring genotype 3 (p<0.05). Mixed infection samples consisted of a major and a minor genotype, with the latter constituting less than 21% of the total viral load and, in 67% of cases, less than 1% of the viral load. Analysis of a subset of the cohort by Illumina PCR-next generation sequencing resulted in a much greater incidence rate than obtained by PCR. This may have occurred due to the non-quantitative nature of the technique and despite the designation of false positive thresholds based on negative controls

A Compact Ring for Thom X-Ray Source
International audienceThe goal of X-ray sources based on Compton back scattering processes is to develop a compact device, which could produce an intense flux of monochromatic X-rays. Compton back-scattering resuls from collisions between laser pulses and relativistic electron bunches. Due to the relative low value of the Compton cross section, a high charge electron beam, a low emittance and a high focusing at the interaction point are required for the electron beam. In addition, the X-ray flux is related to the characteristics of the electron beam, which are themselves dynamically affected by the Compton interaction. One possible configuration is to inject frequently into a storage ring with a low emittance linear accelerator without waiting for the synchrotron equilibrium. As a consequence, the optics should be designed taking into account the characteristics of the electron beam from the linear accelerator. The accelerator ring design for a 50 MeV electron beam, aiming at producing a flux higher than 1013 ph/s, will be presented
CHIANTI - an Atomic Database for Emission Lines. Paper VI: Proton Rates and Other Improvements
The CHIANTI atomic database contains atomic energy levels, wavelengths,
radiative transition probabilities and electron excitation data for a large
number of ions of astrophysical interest. Version 4 has been released, and
proton excitation data is now included, principally for ground configuration
levels that are close in energy. The fitting procedure for excitation data,
both electrons and protons, has been extended to allow 9 point spline fits in
addition to the previous 5 point spline fits. This allows higher quality fits
to data from close-coupling calculations where resonances can lead to
significant structure in the Maxwellian-averaged collision strengths. The
effects of photoexcitation and stimulated emission by a blackbody radiation
field in a spherical geometry on the level balance equations of the CHIANTI
ions can now be studied following modifications to the CHIANTI software. With
the addition of H I, He I and N I, the first neutral species have been added to
CHIANTI. Many updates to existing ion data-sets are described, while several
new ions have been added to the database, including Ar IV, Fe VI and Ni XXI.
The two-photon continuum is now included in the spectral synthesis routines,
and a new code for calculating the relativistic free-free continuum has been

Invasive Disease Caused by Nontuberculous Mycobacteria, Tanzania
Data on nontuberculous mycobacterial (NTM) disease in sub-Saharan Africa are limited. During 2006â2008, we identified 3 HIV-infected patients in northern Tanzania who had invasive NTM; 2 were infected with âMycobacterium sherrisiiâ and 1 with M. avium complex sequevar MAC-D. Pulmonary histoplasmosis presenting as chronic productive cough, fever, and massive unilateral consolidation in a 15-year-old immune-competent boy: a case report
Severe histoplasmosis is known to be among the AIDS-defining opportunistic infections affecting patients with very low CD4 cell counts in histoplasmosis-endemic areas. Histoplasma capsulatum var. duboisii is common in West and Central Africa, where it occurs in both HIV/AIDS and non-HIV patients. Few cases of life-threatening histoplasmosis in immune-competent individuals have been reported worldwide. We describe a case of pulmonary histoplasmosis diagnosed on the basis of autopsy and histological investigations. A 15-year old East African immune-competent boy with a history of smear-positive tuberculosis and a two-year history of rock cutting presented to our hospital with chronic productive cough, fever, and massive unilateral consolidation. At the time of presentation to our hospital, this patient was empirically treated for recurrent tuberculosis without success, and he died on the seventh day after admission. The autopsy revealed a huge granulomatous lesion with caseation, but no acid-fast bacilli were detected on several Ziehl-Neelsen stains. However, periodic acid-Schiff staining was positive, and the histological examination revealed features suggestive of Histoplasma yeast cells. Severe pulmonary histoplasmosis should be considered in evaluating immune-competent patients with risk factors for the disease who present with pulmonary symptoms mimicking tuberculosis
