    FETAL DISCORDANCY IN MONOCHORIONIC TWINS

    Objective: to investigate the predictive value of fetal weight discordance as a risk factor associated with adverse perinatal outcomes. Materials and Methods: 55 patients with monochorionic diamniotic twin pregnancies with fetal weight discordancy were analyzed. All patients underwent ultrasound examination in the 1st trimester of pregnancy (11-14 weeks) to confirm the chorionicity. Estimated fetal weight discordance was calculated as (larger estimated fetal weight - smaller estimated fetal weight) / larger estimated fetal weight. All monochorionic patients were divided into the groups according to the differences in birthweight in twins (≤10%, >10 ≤15%, >15 ≤20%, ≤20 ≤25%, >25%). Adverse perinatal outcomes included: preterm delivery rate (≤34 weeks), fetal distress, low Apgar score, perinatal death, pathological neurosonographic findings. Results: severe birthweight discordance (>20%) in our study was associated with the highest risk of prematurity (76.2%), IUGR (47.6%), fetal distress and low Apgar score (75.0%), pathological neurosonographic findings (46.3%). These adverse perinatal outcomes were more pronounced in the group of monochorionic patients with birthweight discordance greater than 25%. Fetal weight discordance (>20%) is associated with the high risk of adverse perinatal outcomes and twin pregnancies with this complication require careful antenatal monitoring and timing of delivery

    Outcomes of the minimally invasive aortic valve replacement

    The OBJECTIVE was to demonstrate clinical outcomes of minimally invasive aortic valve replacement (MIAVR).MATERIAL AND METHODS. We retrospectively analyzed surgical results of treatment of patients underwent isolated AVR in our Institution between 2006 and 2018. Overall number of operations was 122; 56 of patient were operated via upper ministernotomy approach. Preoperative characteristics were similar in both groups.RESULTS. In our series MIAVR procedures had prolonged CPB and aortic cross clamping time, what significantly contributed to the increase in manifestations of heart failure in the early postoperative period, but didn’t affect the perioperative mortality and major morbidity rates. MIAVR led to reduction in postoperative blood loss and perioperative red blood cell transfusion rate. Most severe complications occurred in frail patients older than 75 years, and in those with extensive aortic annular calcification.CONCLUSION. MIAVR was the safe and reproducible surgical intervention and rarely led to significant complications in low-risk patients. Prolonged CPB time adversely affected the frequency of significant complications, especially in elderly patients. Although, selected high risk patients might benefit with MIAVR

    ANGIOGRAPHIC EVALUATION OF GRAFT PATENCY AFTER MINIMALLY INVASIVE MULTIVESSEL CORONARY BYPASS SURGERY

    The OBJECTIVE was to demonstrate perioperative outcomes and angiographic graft patency rates in the mediumterm period in patients after multivessel minimally invasive coronary artery bypass grafting (MICS CABG) procedures.MATERIAL AND METHODS. In the period from 2014 to 2019, 270 patients with stable forms of coronary artery disease were operated on. All patients received left internal thoracic artery (LITA) and venous or arterial (radial artery) grafts. Off-pump surgery was performed in 264 cases (97.8 %). 127 patients underwent CT-bypass angiography (CT-BA) on a 128-slice computed tomography at least after 1 year after the operation. Mean follow-up duration was (30.3±7.9) months.RESULTS. All patients received full myocardial revascularization. Mean number of grafts was (2.6±0.5). Perioperative mortality rate was 0.4 % (1 patient). Perioperative myocardial infarctions or cerebrovascular accident occurred in 3 (1.1 %) and 1 (0.4 %) patients. Overall graft patency rate in examined patients was 89.8 % (290 of 323). At the time of LITA, the patency of mammary grafts was 98.4 % (124 of 126), venous graft –84.0 % (163 of 194) and radial artery grafts – 100 % (3 of 3).CONCLUSION. MICS CABG was safe and effective and was characterized by minimal rate of perioperative complications. This procedure allowed to achieve complete revascularization of the myocardium without sternotomy and could be performed off-pump (in most cases).     Major results of a phase III comparative multicenter study on the follitropin alfa biosimilar (Primapur®) and the original follitropin alfa (Gonal-f®)

    A clinical study on the efficacy and safety of follitropin alfa has been conducted. The aim of the study was to confirm the therapeutic equivalence between the follitropin alfa biosimilar (Primapur®) and the reference medication (Gonal-f®) in controlled induction of superovulation within the assisted reproductive technologies (ART) programs. Materials and methods. This multicenter, randomized, blind at the embryological stage, in parallel groups, comparative study of phase III (RCT 754 from 26.10.16/NCT03088137) involved 110 women aged 20-35 years with established causes of infertility (tubal factor, male factor). The patients were randomized into 2 equal groups of 55 participants each. The primary end-point for assessing the therapeutic equivalence was the number of aspirated oocytes. The secondary end-points included the number of fertilized oocytes, the number of days of stimulation, the total dose of the injected drug, the occurrence rate of biochemical and clinical pregnancies. Results. In this study, the follitropin alfa biosimilar was shown to be equivalent to the original follitropin in terms of the number of aspirated oocytes. Also, no statistically significant differences were found in the number of mature and fertilized oocytes, the days of stimulation, the dose of the drug administered during the treatment, and the rate of the onset of biochemical or clinical pregnancy. Conclusion. The therapeutic equivalence between the follitropin alfa containing Primapur® and Gonal-f® has been demonstrated

    Мета. Проаналізувати питому вагу вогнепальних поранень гомілки в структурі бойової хірургічної травми, визначити особливості лікувально-евакуаційних заходів при проведенні антитерористичної операції / операції об’єднаних сил на сході України. Матеріали і методи. Загальний масив дослідження становили 489 поранених у яких було 805 вогнепальних поранень гомілки з 2014 по 2018 роки під час проведення. Використано шкалу оцінки тяжкості бойової хірургічної травми. Результати. На підставі проведеного комплексного дослідження було встановлено, що поранення гомілки становили 23,95% в загальній структурі бойової травми кінцівок. У поранених з вогнепальними переломами кісток гомілки та відривами кінцівки на першому рівні медичного забезпечення необхідні тимчасова зупинка кровотечі, раннє накладання асептичної пов’язки, знеболення, іммобілізація підручними засобами та термінова медична евакуація на другий рівень медичної допомоги. На другому рівні переважно виконуються первинні хірургічні обробки рани (91,86%) та фасціотомія (44,19%). Важливим протишоковим заходом другого рівня є стабільне накладання стержневого апарату зовнішньої фіксації (43,02%), а також введення антибіотиків та правцевого анатоксину. На третьому та четвертому рівнях медичного забезпечення переважну більшість становили повторні хірургічні обробки ран, фасціотомії, реконструктивно-відновні оперативні втручання (рα9 балів) - допомогу надавати у скороченому обсязі за принципом контролю ушкоджень
