A Passivity-based Nonlinear Admittance Control with Application to Powered Upper-limb Control under Unknown Environmental Interactions
This paper presents an admittance controller based on the passivity theory
for a powered upper-limb exoskeleton robot which is governed by the nonlinear
equation of motion. Passivity allows us to include a human operator and
environmental interaction in the control loop. The robot interacts with the
human operator via F/T sensor and interacts with the environment mainly via
end-effectors. Although the environmental interaction cannot be detected by any
sensors (hence unknown), passivity allows us to have natural interaction. An
analysis shows that the behavior of the actual system mimics that of a nominal
model as the control gain goes to infinity, which implies that the proposed
approach is an admittance controller. However, because the control gain cannot
grow infinitely in practice, the performance limitation according to the
achievable control gain is also analyzed. The result of this analysis indicates
that the performance in the sense of infinite norm increases linearly with the
control gain. In the experiments, the proposed properties were verified using 1
degree-of-freedom testbench, and an actual powered upper-limb exoskeleton was

Long-term evaluation of the prognosis of super hydrophilic surface treated CA implants: a retrospective clinical study
This study was performed to evaluate the long-term clinical efficacy of the CA implants (Osstem Implant, Busan, Korea), calcium-modified surfaced treated implants on acid-etched surfaces sandblasted with alumina.
From January 2013 to December 2015, 258 implants of 120 patients placed between 2013 and 2015 were retrospectively studied. Using medical records and periapical radiographs, sex, age, location, fixture width and length of placed implants, presence or absence of bone graft, types of bone substitutes and membrane used for bone grafting, primary and secondary stability, initial and delayed complications, and marginal bone loss were investigated. The success rate and survival rate of the implants in each group were analyzed retrospectively based on the criteria suggested by Albrektsson et al.
Between 2013 and 2015, with a follow-up longer than 5years, 258 implants with an average diameter of 4.63mm (3.5โ5.5mm) and an average length of 9.94mm (7.0โ13.0mm) were placed in a total of 120 patients (61 males and 59 females) with a mean age of 63.7years for an average of 62months of observation period. The survival rate was 97.3%, the success rate was 94.2%, and the average final marginal bone loss was 0.074mm.
The CA implants manufactured with the improved surface treatment method exhibited a survival rate of 97.3% and a success rate of 94.2% over an average observation period of 62months. The implants were not affected by most factors and had very high survival and success rates over a long period of observation. In particular, the stability of the implant was excellent, with no cases of failed implants in delayed placement after bone grafting and a healing period
Epileptic nystagmus: A case report and systematic review
AbstractPurposeWe aimed to define the characteristics of epileptic nystagmus and correlate those with other clinical findings in a large number of patients.MethodsWe report a patient with epileptic nystagmus and additionally reviewed the reported clinical features of 36 more patients through a systematic literature search. We analyzed the characteristics of epileptic nystagmus and attempted correlations of those with alertness of the patients and epileptic foci on EEG.ResultsAll 33 patients with unilateral horizontal nystagmus showed nystagmus beating away from the side of ictal discharges. Epileptic nystagmus was preceded by gaze deviation in 21 patients, with contraversive in 19 and ipsiversive in 2. Seizures associated with epileptic nystagmus were mostly focal (25/29, 86.2%) with or without loss of awareness. Ictal discharges originated from the occipital (n=16), parietal (n=9), temporo-occipital (n=6), frontal (n=4), and temporal (n=3) areas, and two patients had multiple epileptic foci. Seizures were usually symptomatic (24/37, 64.9%). The presence of preceding gaze deviation and midline crossing of the nystagmus did not correlate with the ictal onset zone or alertness of the patients. Recording of epileptic nystagmus was available only in 6 patients, and the epileptic nystagmus could be localized to the saccadic areas in two and to the smooth pursuit areas in another two. Two patients showed the features of epileptic nystagmus from both areas.ConclusionEven though the localizing value of epileptic nystagmus seems limited in previous reports, the fast phase of epileptic nystagmus was almost always directed away from the epileptic focus that mostly arose from the posterior part of the cerebral hemisphere
Prognosis of single tooth implants following alveolar ridge preservation with two recombinant human bone morphogenetic protein-2 delivery systems
Background
We previously reported similar efficacies of alveolar ridge preservation (ARP) on single extraction socket with two different E. coli derived recombinant human bone morphogenetic protein-2 (rhBMP-2) delivery systems (Cowell BMP, Cowell medi Co, Busan, Korea; ฮฒ-tricalcium phosphate and hydroxyapatite particle & O-BMP, Osstem Implant Co, Busan, Korea; absorbable collagen sponge). After the trial, we completed implant therapy and observed over an average of 3 years. This follow-up study was performed retrospectively to compare result of implant treatment at the preserved alveolar ridge site.
Methods
Patients who underwent extraction of single tooth and received ARP with one of two rhBMP-2 delivery systems from October 2015 to October 2016 were enrolled. Twenty-eight patients (Group 1: Cowell BMP 14; Group 2: O-BMP 14) who underwent implant therapy and prosthetic treatment were included in study. Stability and marginal bone loss (MBL) of each implant were collected from medical charts and radiographs, and analyzed. The survival and success rates of implants were calculated.
Results
The primary implant stability represented by implant stability quotient (ISQ) for Groups 1 and 2 was 69.71 and 72.86, respectively. The secondary implant stability for Groups 1 and 2 was 78.86 and 81.64, respectively. Primary and secondary stabilities were not statistically different (Pโ=โ0.316 and 0.185, respectively). MBL at the latest follow-up was 0.014 mm in Group 1 over 33.76โยฑโ14.31 months and 0.021 mm in Group 2 over 40.20โยฑโ9.64 months, with no significant difference (Pโ=โ0.670). In addition, the success rate of implants was 100% (14/14) in Group 1 and 92.9% (13/14) in Group 2, with survival rate of 100% (14/14) in Group 1 and 92.9% (13/14) in Group 2.
Conclusions
We confirmed good prognosis in both groups as a result of implant therapy after ARP with each of two rhBMP-2 carriers
Rotational Resistance of Surface-Treated Mini-implants
Objective: To test the hypothesis that there is no difference in the stability and resistance to rotational moments of early loaded sandblasted and acid-etched (SLA) mini-implants and those of machined-surface implants of the same size and shape.
Materials and Methods: A randomized complete block design was used in 12 skeletally mature male beagle dogs. Ninety-six orthodontic mini-implants were tested. Two types of implants were used: some had SLA surface treatment and some had machined surfaces without coating. After 3 weeks of healing, rotational moments of 150 g were applied. The success rates, maximum torque values, angular momentum, and total energy absorbed by the bone were compared. All values were subjected to mixed-model analysis to evaluate the influence of surface treatment, rotational force direction, and site of implantation.
Results: The maximum insertion torque and angular momentum of SLA implants were significantly lower than those of machined implants (P = .034, P = .039). The SLA implants had a significantly higher value for total removal energy than the machined implants (P = .046). However, there were no significant differences in total insertion energy, maximum removal torque, and removal angular momentum between the 2 groups. There was no significant difference between clockwise and counterclockwise rotation in all measurements.
Conclusion: SLA mini-implants showed relatively lower insertion torque value and angular momentum and higher total energy during removal than the machined implants, suggesting osseointegration of the SLA mini-implant after insertion
Cervical Facet Joint Injections in the Neck and Shoulder Pain
The effects from cervical facet joint injections in those patients who have been complaining cervical zygapophyseal joint pain were compared. The patients were diagnosed originally as myofascial pain syndrome (MPS), cervical herniated nucleus pulposus (HNP), and whiplash-associated disorders (WAD). Patients with the zygapophyseal joints pain of C5-6 and C6-7 were classified by their pain origin as MPS, HNP, and WAD. All patients had been undergone cervical zygapophyseal joints injections with the mixture of lidocaine and triamcinolone unilaterally or bilaterally through the posterior approach under C-arm imaging guide. The therapeutic effects were compared with reduction of numeric rating scale (NRS) of pain before and immediately after blockade and symptom-free periods in each group after 12 months. Symptom durations before injections were 16.1ยฑ9.6, 4.6ยฑ1.9 and 4.1ยฑ1.1 months in each MPS, HNP, and WAD groups. The reductions of NRS immediately after the blockade among the three groups were not different. However, the symptom-free duration after blockade lasted longer in the HNP group than the other two groups. In patients with cervical zygapophyseal pain syndromes, the analgesic effect from cervical facet joint blocks lasted longer in cervical HNP than MPS or WAD
Associations between mandibular torus and types of temporomandibular disorders, and the clinical usefulness of temporary splint for checking bruxism
Abstract
Background
Occlusal stress from oral parafunctional habits is one of the causes of temporomandibular disorders (TMD) and mandibular torus (MT). Although some studies have investigated the correlation between TMD and MT, understanding of the relationships between types of TMD and MT is insufficient. Therefore, we conducted this study to investigate the associations between presence of MT and TMD types.
Methods
This study included 77 patients diagnosed with TMD who first visited our clinic for TMD between March 2019 and July 2020. Among them, 30 (38.9%) had MT, and 54 (70.1%) had oral parafunction. Parafunctional activity during sleep was confirmed using a temporary splint for checking bruxism (TSCB).
Results
The relationship between prevalence of MT and oral parafunction in TMD patients was not statistically significant (Pโ=โ0.131), but the odds ratio was relatively high at 2.267. An analysis of TMD type revealed that Type I, which is classified as myalgia of the masticatory muscles, and MT had a significant association (Pโ=โ0.011). We fabricated a TSCB for 27 patients to wear during sleep and confirmed that 23 (85.2%) had nocturnal bruxism. The TSCB results and presence of MT showed a significant relationship (Pโ=โ0.047).
Conclusion
Through the results of this study, clinicians may consider the hyperactivity of masticatory muscles in the presence of MT when treating TMD patients. In addition, TSCB has a great diagnostic value as it can be easily manufactured and be useful for discovering pre-existing oral parafunctions that patients are not aware of
Vardenafil Enhances Oxytocin Expression in the Paraventricular Nucleus without Sexual Stimulation
PurposeOxytocin is associated with the ability to form normal social attachments. c-Fos is an immediate early gene whose expression is used as a marker for stimulus-induced changes in neurons. The effect of phosphodiesterase-5 (PDE-5) inhibitors on oxytocin activation in the brain without sexual stimuli has not yet been reported. In the present study, we investigated the effects of vardenafil on oxytocin and c-Fos expression in the paraventricular nucleus (PVN) of conscious rats.MethodsMale Sprague-Dawley rats weighing 300ยฑ10 g were divided into 6 groups (n=5 in each group): the control group, the 1-day-0.5 mg/kg, the 1-day-1 mg/kg, the 1-day-2 mg/kg, the 3-day-1 mg/kg, and the 7-day-1 mg/kg vardenafil administration group. The experiment was conducted without sexual stimulation. Vardenafil was orally administered. The animals in the control group received an equivalent amount of distilled water orally. The expression of oxytocin and c-Fos in the PVN was detected by immunohistochemistry.ResultsOxytocin expression in the PVN was increased by 1 day administration of 2 mg/kg vardenafil, and this effect of vardenafil appeared in a duration-dependent manner. c-Fos in the oxytocin neurons of the PVN was increased by 1 day administration of 2 mg/kg vardenafil, and this effect of vardenafil also appeared in a duration-dependent manner. These results showed that vardenafil augments the expression of oxytocin with activation of oxytocin neurons in the PVN.ConclusionsIn this study, we showed that the PDE-5 inhibitor, vardenafil directly enhances oxytocin expression and also activates oxytocin neurons in the PVN, which indicates that vardenafil may exert positive effects on affiliation behavior and social interaction
