    Breastfeeding Practices and Dietary Diversity among Infants and Young Children in Rural and Urban-Slum Populations in India: An Observational Study

    Background: Nutritional exposures and growth in early life are linked to immediate and also to long term health outcomes. Objective: To assess infant and young child feeding (IYCF) practices using WHO-UNICEF defined indicators in rural and urban-slum populations in India. Methods: A community-based, cross-sectional study was conducted in mothers and infants up to age 24 months. Data on socio-demographics, birth history, feeding practices (WHO-UNICEF IYCF indicators), maternal weight, height, and infantā€™s weight, length, mid-arm, and head circumferences were collected. Results: Five hundred and two (252 rural and 250 urban-slum) mother-infant dyads were studied. Proportions of IYCF indicators in rural and urban-slum infants were: Early initiation of breastfeeding 71 and 64%; Exclusive breastfeeding under six months, 59 and 25%; Minimum acceptable diet 11 and 27% respectively. Consumption of animal-source food (other than dairy products) and vitamin-A rich fruits and vegetables was below 15%. Cesarean section [aOR, 95% CI: 2.94 (1.53, 5.65)], hospitalization of newborn [aOR, 95% CI: 6.21 (2.95, 13.16)], pre-lacteal feeding [aOR, 95% CI: 3.38 (1.77, 6.45)], needing help in breastfeeding [aOR, 95% CI: 2.15 (1.04, 4.17)], and male gender [aOR, 95% CI: 2.13 (1.15, 4.25); p<0.05 for all] were associated with delayed initiation of breastfeeding, whereas lower monthly household income [aOR, 95% CI: 2.62 (1.10, 6.25)], and younger age [aOR, 95% CI: 1.24 (1.11, 1.38); p<0.05 for both] were associated with poor dietary diversity. Conclusions: Education of optimum IYCF practices, targeting early initiation of breastfeeding, increasing meal frequency and intake of vitamin-A rich and animal-source foods need urgent attention

    Re-evaluating the need for universal iron supplementation in pregnant Indian women in the light of gestational age specific low hemoglobin prevalence

    Background: Anemia prevalence among Indian pregnant women in 2015-16 was 50.4% and has not declined from 49.7% in 1998-99 despite the national policies on iron-folic acid supplementation. New gestational age-specific cut-offs (INTERGROWTH cut-off) for risk of low Hemoglobin (Hb) have been identified. Aims and objectives: Compare prevalences of low Hb Ā based on WHO and INTERGROWTH cut-offs Methods: The prevalence of anemia/low Hb among pregnant women in trimesters 2 and 3 from NFHS-4 data were estimated using the current WHO recommendations and the INTERGROWTH cut-offs. Results: Prevalence of low Hb by the INTERGROWTH cut-off was 28.1%(95% CI:26.9-29.4) and 21.7%(95% CI:20.6-22.9) in trimesters 2 and 3. Anemia prevalence by WHO cut-off was much higher at 41.2%(95% CI:39.8-42.5) and 54.8%(95 CI:53.2-56.3) in trimesters 2 and 3. The prevalence of low-Hb was similar between ANC and no-ANC reported groups in both trimesters (26.1% and 28.9% in trimester-2; 20.1% and 22.4% in trimester-3). Conclusion: TheĀ  prevalence of low-Hb with gestational age specific cut-offs is much lower compared to earlier estimates using WHO cut-off. The universal iron supplementation program for pregnant women in India need to be re-examined in this light and a tragetter Hb testing based supplementation may be more beneficial in reducing anemia prevalence

    Maternal serum selenium as a predictor of preeclampsia

    Background: Preeclampsia (PE) is a hypertension disorder condition occurring in 7-10% of all pregnancies. Preeclampsia if unidentified and left untreated is associated with poor maternal and fetal adverse outcomes. The objective of the present study was to characterize maternal serum selenium levels as a predictor of preeclampsia and to correlate dietary selenium intake with serum selenium levels in first trimester of pregnancyMethods: A retrospective case-control study of 107 pregnant women was conducted over 1.5 years at St. Johnā€™s Medical College Hospital. On screening for inclusion criteria, at baseline, information on maternal socio-demography, anthropometry, dietary intake and clinical examination was collected. A venous blood sample at baseline and 2nd or 3rdtrimester of pregnancy was collected for estimation of selenium concentrations. Blood pressure was measured at baseline and followed up during pregnancy to select cases and controls. Pregnant women were termed ā€˜casesā€™ based on NHBPEP (National High Blood Pressure Education Program) classification and subsequent 22 women with normal blood pressure controlled for age were termed as ā€˜controls.Results: No statistically significant differences were observed for baseline characteristics, biochemical parameters and blood pressure at recruitment among cases and controls. Cases had significant lower levels of energy (P=0.032) and micronutrients like zinc (P=0.027), selenium (P=0.022), magnesium (P=0.047) at first trimester. The serum selenium levels were significantly higher in cases as compared to the controls (69.2Ā±13.7 vs. 59.6Ā±12.9; P=0.021) at baseline.Conclusions: Our findings suggest that serum selenium levels may not be an independent predictor of preeclampsia. Assessment of other micronutrients, oxidative stress markers and other complementary elements may be useful in predicting preeclampsia

    Co-ingestion of black tea reduces the indispensable amino acid digestibility of hensā€™ egg in Indian adults

    Background: Tea, a commonly consumed beverage, contains high amounts of polyphenols that can impair protein digestibility, as demonstrated in vitro. There are no human studies examining the inhibitory influence of tea polyphenols (TPP) on high-quality protein digestibility. Objective: The aim of this study was to determine the effect of black tea on the true indispensable amino acid (IAA) digestibility of whole boiled egg protein, in healthy adult humans, through use of a dual isotope tracer approach. Methods: The effect of black TPP (4.6 mg/mL, ingested as a beverage with the meal) on 2H-labeled whole boiled egg protein, administered with ghee rice and tomato curry, was measured with reference to 13C-spirulina protein in healthy Indian adults aged 20ā€“27 y of both sexes with BMI of 22.0 Ā± 2.8 kg/m2. The results were then compared to previously determined whole egg mean IAA digestibility measured by the same method, without black tea, in the same subjects (n = 5). To correct for any independent effect of TPP on spirulina protein (used as a standard protein), the true IAA digestibility of 13C-spirulina protein was independently measured with reference to a 2H-amino acid mixture, with and without co-ingestion of black tea, in 3 of the same subjects. Results: The true IAA digestibility of whole boiled egg protein significantly decreased by 17% when co-ingested with black tea. However, there was no significant reduction in the true IAA digestibility of spirulina protein when co-ingested with black tea. Conclusions: TPP protein interactions reduced whole egg digestibility in healthy Indian adults but had minimal effect on spirulina protein digestibility. In populations who are at risk of dietary quality protein inadequacy, the consumption of tea during or after a meal can further increase the risk of inadequacy.     True ileal digestibility of legumes determined by dual isotope tracer method in Indian adults

    Background: Good-quality plant protein sources are important for protein adequacy in a balanced diet. Legumes are known to be a source of good quality plant protein, but the true ileal digestibility of indispensable amino acids (IAAs) of commonly consumed legumes is not known in humans. Objectives: In this study we measured the true ileal IAA digestibility of 2H-intrinsically labeled chickpea, yellow pea, and mung bean (hulled and dehulled) protein, using the dual-isotope tracer technique referenced to a standard protein ([U-13C] spirulina). The study also aimed to validate the use of [U-13C] spirulina as a reference protein in this method. Methods: 2H-intrinsically labeled legumes, obtained by watering plants with deuterium oxide (2H2O), were administered in a plateau feeding method to healthy Indian adults to measure their true ileal IAA digestibility with the dual-isotope tracer technique, using [U-13C] spirulina protein or a 13C-algal IAA mixture as the standard. Result: The true ileal IAA digestibilities (mean Ā± SD) of chickpea, yellow pea, and mung bean were 74.6 Ā± 0.8%, 71.6 Ā± 1.3%, and 63.2 Ā± 1.5%, respectively. The true mean ileal IAA digestibility of mung bean when referenced to [U-13C] spirulina protein or a 13C-algal IAA mixture did not differ significantly (63.2 Ā± 1.5% versus 64.0 Ā± 2.4%, P > 0.05). The true ileal IAA digestibility of mung bean improved to 70.9 Ā± 2.1% after dehulling. Conclusions: The true mean ileal IAA digestibility of legumes in healthy Indian adults was lower than expected. Traditional processing techniques such as dehulling improve protein digestibility by about 8%.     Protein-quality evaluation of complementary foods in Indian children

    Background: The types of food in complementary feeding of infants and young children are important for growth and development. Food protein quality, as measured by the Digestible Indispensable Amino Acid Score (DIAAS), requires the determination of true ileal digestibility of indispensable amino acids (IAAs) in children. Objectives: First, the aim of this study was to measure the true ileal IAA digestibility of 4 (rice, finger millet, mung bean, and hen egg) commonly consumed complementary foods in children aged <2 y using the dual-isotope tracer method. Second, we calculated the DIAAS of complementary feeding diets and their relation to stunting in a representative Indian rural population. Design: Rice, finger millet, and mung bean were intrinsically labeled with deuterium oxide (2H2O), whereas egg was labeled through oral dosing of hens with a uniformly 2H-labeled amino acid mixture. True ileal IAA digestibility was determined by the dual-isotope tracer technique. The DIAAS of complementary food protein was calculated in children aged 1ā€“3 y from a nationally representative survey to evaluate its relation with stunting. Results: True ileal IAA digestibility was lowest in mung bean (65.2% Ā± 7.1%), followed by finger millet (68.4 %Ā± 5.3%) and rice (78.5% Ā± 3.5%), and was highest for egg (87.4% Ā± 4.0%). There was a significant inverse correlation of complementary food DIAAS with stunting in survey data (r = āˆ’0.66, P = 0.044). The addition of egg or milk to nationally representative complementary diets theoretically improved the DIAAS from 80 to 100. Conclusions: The true ileal IAA digestibility of 4 foods commonly consumed in complementary diets showed that the DIAAS was associated with stunting and reinforces the importance of including animal source food (ASF) in diets to improve growth.     Evaluation of the Indian Migration Study Physical Activity Questionnaire (IMS-PAQ): a cross-sectional study.

    BACKGROUND: Socio-cultural differences for country-specific activities are rarely addressed in physical activity questionnaires. We examined the reliability and validity of the Indian Migration Study Physical Activity Questionnaire (IMS-PAQ) in urban and rural groups in India. METHODS: A sub-sample of IMS participants (n = 479) was used to examine short term (ā‰¤ 1 month [n = 158]) and long term (> 1 month [n = 321]) IMS-PAQ reliability for levels of total, sedentary, light and moderate/vigorous activity (MVPA) intensity using intraclass correlation (ICC) and kappa coefficients (k). Criterion validity (n = 157) was examined by comparing the IMS-PAQ to a uniaxial accelerometer (ACC) worn ā‰„ 4 days, via Spearman's rank correlations (Ļ) and k, using Bland-Altman plots to check for systematic bias. Construct validity (n = 7,000) was established using linear regression, comparing IMS-PAQ against theoretical constructs associated with physical activity (PA): BMI [kg/m2], percent body fat and pulse rate. RESULTS: IMS-PAQ reliability ranged from ICC 0.42-0.88 and k = 0.37-0.61 (ā‰¤ 1 month) and ICC 0.26 to 0.62; kappa 0.17 to 0.45 (> 1 month). Criterion validity was Ļ = 0.18-0.48; k = 0.08-0.34. Light activity was underestimated and MVPA consistently and substantially overestimated for the IMS-PAQ vs. the accelerometer. Criterion validity was moderate for total activity and MVPA. Reliability and validity were comparable for urban and rural participants but lower in women than men. Increasing time spent in total activity or MVPA, and decreasing time in sedentary activity were associated with decreasing BMI, percent body fat and pulse rate, thereby demonstrating construct validity. CONCLUSION: IMS-PAQ reliability and validity is similar to comparable self-reported instruments. It is an appropriate tool for ranking PA of individuals in India.
