Lack of association between serotonin transporter gene polymorphism 5-HTTLPR and smoking among Polish population: a case-control study
<p>Abstract</p> <p>Background</p> <p>A better understanding of the genetic determinants of tobacco smoking might help in developing more effective cessation therapies, tailored to smokers' genotype. Insertion/deletion polymorphism in the promoter region of the serotonin transporter gene (<it>5-HTTLPR</it>) has been linked to vulnerability to smoking and ability to quit. We aimed to determine whether <it>5-HTTLPR </it>genotype is associated with smoking behavior in Caucasians from Northern Poland and to investigate other risk factors for tobacco smoking.</p> <p>Methods</p> <p><it>5-HTTLPR </it>genotypes were determined in 149 ever smokers (66 females; mean age 53.0 years) and 158 gender and ethnicity matched never smoking controls (79 females; mean age 45.0 years) to evaluate the association of this polymorphism with ever smoking status. Analysis of smokers was performed to evaluate the role of <it>5-HTTLPR </it>in the age of starting regular smoking, the number of cigarettes smoked daily, pack-years, FTND score, duration of smoking, and the mean length of the longest abstinence on quitting. Genotype was classified according to the presence or absence of the short (<it>S</it>) allele vs. the long (<it>L</it>) allele of <it>5-HTTLPR </it>(i.e., <it>S/S </it>+ <it>S/L </it>vs. <it>L/L</it>). Logistic regression analysis was also used to evaluate correlation between ever smoking and several selected variables.</p> <p>Results</p> <p>We found no significant differences in the rates of <it>S </it>allele carriers in ever smokers and never smokers, and no relationship was observed between any quantitative measures of smoking and the polymorphism. Multivariate analysis demonstrated significant association between the older age (OR = 4.03; 95% CI: 2.33–6.99) and alcohol dependence (OR = 10.23; 95% CI: 2.09–50.18) and smoking.</p> <p>Conclusion</p> <p><it>5-HTTLPR </it>seems to be not a major factor determining cigarette smoking in Poles. Probably, the risk of smoking results from a large number of genes, each contributing a small part of the overall risk, while numerous non-genetic factors might strongly influence these genetic undergrounds of susceptibility to smoking.</p
Management of cancer pain in Primary Care in Poland
Cancer pain remains a big challenge to modern medicine. General Practitioners (GPs) supported by Palliative
Care teams play a key role in the care of terminally ill cancer patients. The quality of pain management
depends very much on GPs’ knowledge and skills.
The aim of this study was to assess GPs’ knowledge about pain management.
The questionnaire with clinical situations based on recommended guidelines connected with pain management
was sent to all GPs within the Kujawsko-Pomorski and Podlaski provinces.
The response rate was 51%. The GPs in Kujawsko-Pomorski province had better access to palliative care for
their patients. In the whole sample, the median of appropriate responses to eight questions equalled 4. The
biggest percentage of correct answers (85.6%) was given to question concerning the characteristic of
neuropathic pain. Besides, more than 50% of correct answers were given to questions which concerned the
breakthrough pain (54.4%) and pain management of an unconscious patient (56.9%). The lowest percentage
of correct answers (24.6%) concerned the conversion of oral to subcutaneous opioids in case of
vomiting and reappearance of intensive pain.
The results of this study indicate a considerable difference between the official guidelines and every day
practice of pain management.
The results underline also the need for changes in vocational training of general practitioners, with special

The adult patient experiencing bereaved after the death of a loved person - the role of the general practitioner
Bereavement as the consequence of a loss of a loved person begins after his death. Its aim is the real
acceptance of the loss. Everybody experiences bereavement in a specific way. This unique process depends
on many factors and usually lasts 1-2 years. It affects all spheres of life - mental, physical, emotional and
spiritual. Availability of support from relatives, physician, psychologist, nurse and priest plays an important
role in grief. The general practitioner supports the bereaved and tries to monitor the course of grief.
Knowledge about the phases of bereavement, defense mechanisms, and bereavement-related stereotypes
is very helpful. There is a difference in the grief of a widow, a widower and orphans. When caring for the
bereaved, the general practitioner tries to recognize the abnormalities of this process which could require
consultation. Bereavement is also a time of increased somatic morbidity and mortality.Żałoba jest konsekwencją straty, procesem rozpoczynającym się po śmierci bliskiej osoby. Jej celem jest
rzeczywiste zaakceptowanie utraty. Każdy człowiek przeżywa żałobę inaczej. Jest to proces jedyny, niepowtarzalny,
zależny od wielu czynników, trwający zwykle 1–2 lata. Dotyka on wszystkich sfer ludzkiego
funkcjonowania: psychicznej, fizycznej, emocjonalnej i duchowej. Istotną rolę w przebiegu żałoby odgrywa
wsparcie ze strony najbliższych, lekarza, psychologa, pielęgniarki, duchownego. Lekarz rodzinny sprawujący
opiekę nad osobami w żałobie - poza udzielaniem wsparcia - stara się kontrolować prawidłowość
przebiegu tego procesu. Pomocna jest w tym wiedza na temat faz żałoby, uruchamianych mechanizmów
obronnych oraz stereotypów odnoszących się do tego okresu. Należy pamiętać, że inaczej przebiega żałoba
wdowy, wdowca czy osób osieroconych po śmierci rodziców. Sprawując opiekę nad osobami w żałobie,
lekarz rodzinny stara się rozpoznać nieprawidłowości przebiegu tego procesu wymagające dalszej konsultacji.
Lekarz rodzinny musi także pamiętać, że okres żałoby to również czas, w którym u pacjentów zwiększa się
ryzyko zachorowalności i śmiertelności z powodu chorób somatycznych
Pacjenci z zespołem MUS w podstawowej opiece zdrowotnej. Wskazówki diagnostyczne i terapeutyczne
Patients who suffer from medically unexplained symptoms (MUS) constitute nearly 45% of all primary care patients. The most common symptoms are headache, dizziness, blurred vision, back pain, tingling and trembling of upper and lower limbs, excessive sweating, itching, abdominal pain, flatulence, sleeplessness and many others. Medical tests and additional technical examinations present no organic changes. In case of lack clinical improvement, patients expect their GPs to use more diagnostic methods and more referrals to other specialists. Many primary care doctors present a negative attitude towards MUS patients. The main reason is limited financial resources as well as lack of effective therapeutic results. Because the reason for MUS is not known, there is no satisfactory therapy. The best strategy the GP can offer to their patients is listening carefully and not referring them for additional test or to other medical institutions. There is established a six stage s talking therapy model for MUS patients in the United Kingdom. The aim of this strategy is to educate MUS patients, reassure them, and explain to them that they suffer from functional ailments. An effective mode of coping with psychosomatic symptoms should be the final aim of this model. Pharmaceutical intervention with the use of antidepressants, anti-anxiety drugs, sleeping pills, or relaxants should be considered in resistant cases. It is very important to exclude serious organic diseases such as thyroid gland pathology, iron deficiency or electrolytic imbalance. There are special guidelines for GPs concerning appropriate treatment of MUS patients in many EU countries. There is also a research project underway in Poland, aiming to establish diagnostic and therapeutic guidelines for treating MUS patients.Pacjenci z niewyjaśnionymi objawami medycznymi stanowią około 45% wszystkich osób zgłaszających się do podstawowej opieki zdrowotnej. Do najczęstszych dolegliwości należą bóle głowy, zawroty głowy, zamglone widzenie, bóle pleców, mrowienia, cierpnięcia palców stóp i rąk, nadmierne pocenie się, świąd skóry, bóle brzucha, wzdęcia, bezsenność. Wykonywane przez lekarza rodzinnego badania diagnostyczne nie wykazują odchyleń od normy. Pacjenci wobec braku poprawy oczekują skierowania na dalsze test y diagnostyczne i na konsultacje do innych specjalistów. Lekarze podstawowej opieki mają często negatywną ocenę wobec tej grupy pacjentów. Głównym powodem jest duże obciążenie finansowe i brak efektów leczenia. Nie jest znana przyczyna powstawania dolegliwości, a więc nie ma skutecznego sposobu postępowania. Najlepszą strategią medyczną jest cierpliwe słuchanie pacjenta, niezlecanie badań i nieodsyłanie do innych placówek medycznych. W Wielkiej Brytanii wypracowano sześcioetapowy model rozmowy z pacjentem (talking therapy). Ma on na celu edukację chorego i przekonanie go, że objawy MUS mają podłoże czynnościowe. Końcowym etapem terapii powinno być opanowanie przez pacjenta podstawowych technik radzenia sobie z zaburzeniami psychosomatycznymi. W sytuacjach klinicznych opornych na psychoterapię należy rozważyć włączenie leków przeciwdepresyjnych, przeciwlękowych, uspokajających lub nasennych. Należy również pamiętać o wykluczeniu chorób somatycznych, jak na przykład chorób tarczycy, niedokrwistości lub zaburzeń elektrolitowych. W wielu krajach europejskich opracowano dla lekarzy podstawowej opieki wytyczne postępowania z pacjentami z MUS. Trwają badania naukowe w Polsce mające na celu opisanie podstawowych zasad przy diagnozowaniu i leczeniu osób z niewyjaśnionymi objawami medycznymi
Postępowanie z pacjentem palącym tytoń w warunkach praktyki lekarza rodzinnego
Palenie tytoniu jest najważniejszym, pojedynczym, możliwym do wyeliminowania czynnikiem
odpowiedzialnym za wiele chorób i zgonów. Negatywny wpływ dotyczy palenia
czynnego, jak i biernego. Do rozwoju uzależnienia dochodzi wskutek farmakologicznego
oddziaływania nikotyny na ośrodkowy układ nerwowy i równocześnie rozwijających
się zmian behawioralnych. Palenie tytoniu traktowane jest jako choroba przewlekła występująca
w fazach remisji, gdy pacjent nie pali, i nawrotów, czyli powrotu do palenia.
Większość pacjentów, która pali, deklaruje chęć rzucenia palenia, chociaż tylko niewielki
odsetek jest w stanie zrobić to skutecznie bez pomocy. Szczególnym miejscem, gdzie
pacjent mógłby otrzymać pomoc podczas rzucania palenia, jest praktyka lekarza rodzinnego.
Prostą i skuteczną metodą pomocy jest minimalna interwencja antynikotynowa.
Jest to krótkie kilkuminutowe działanie podejmowane w trakcie wizyty lekarskiej, polegające
na identyfikacji pacjenta palącego tytoń i udzieleniu mu fachowej pomocy w zerwaniu
z nałogiem. Schemat przeprowadzania minimalnej interwencji antynikotynowej
opiera się na zasadzie 5 × P. W trakcie prowadzenia interwencji często włącza się leczenie
farmakologiczne. Lekami o udowodnionej skuteczności są: nikotynowa terapia
zastępcza, chlorowodorek bupropionu i wareniklina. Lekarze rodzinni poprzez swoje zaangażowanie
w działania antynikotynowe mogą wpływać na zmniejszenie częstości palenia
wśród swoich pacjentów

Nowotwory złośliwe skóry należą do najczęściej spotykanych u człowieka. Wczesne rozpoznanie i szybkie
leczenie prowadzą zwykle do pełnego uleczenia. Ponieważ wyleczalność tego rodzaju guzów sięga niemal
100%, niewiele wiadomo o przebiegu choroby w przypadku, kiedy pacjent odmawia leczenia chirurgicznego.

Diagnosis and treatment of Alzheimer’s disease in general practice
Choroba Alzheimera (AD) jest chorobą pierwotnie zwyrodnieniową mózgu, spowodowaną odkładaniem się w nim patologicznych białek. Charakteryzuje się postępującą utratą pamięci i pogorszeniem funkcji poznawczych. W Polsce około 500 tys. osób choruje na otępienie, w tym połowa na AD. Bezspornym czynnikiem ryzyka jest wiek. Patofizjologia choroby jest złożona i wieloczynnikowa. Podstawą rozpoznania otępienia typu alzheimerowskiego powinny być kryteria ICD-10 lub DSM-IV. Przebieg kliniczny otępienia możemy podzielić na 3 fazy: otępienie łagodne, średnio zaawansowane i bardzo zaawansowane. Szczegółowy opiso bjawów i przebiegu klinicznego zawiera skala Global Deterioration Scale (GDS), która także określa stopień zaawansowania AD. Lekarz rodzinny może wykonać dwa testy przesiewowe: krótką skalę oceny stanu psychicznego MINI-MENTAL (MMSE) oraz test rysowania zegara (CDT). W leczeniu objawów AD stosuje się inhibitory acetylocholinesterazy, memantynę. Opieka nad pacjentem z AD wymaga współpracy lekarza rodzinnego z wieloma specjalistami.Alzheimer disease (AD) is chronic neurodegenerative disorder caused by accumulation of pathological proteins. It is characterized by progressive memory loss and cognitive dysfunction. There are 500 thousand people suffering from dementia in Poland and half of them suffer from AD. The pathogenesis of AD is complex and prevalence is connected with age. Making diagnosis should be based on criteria proposed in ICD-10 or DSM-IV. There are three main stages: moderate, moderately severe and severe dementia. Global Deterioration Scale describes stages of AD according to deficits in cognition and function. Family physician can use two screening tests: mini-mental state examination (MMSE) and clock drawing test (CDT). Cholinesterase inhibitors and memantine are used in pharmacotherapy. The cancer delusion as a depression symptom initiating Alzheimer disease - diagnostic difficulties
Depresja u osób w wieku podeszłym przejawia się często objawami somatycznymi. Jej przebieg może
przypominać rozwój choroby nowotworowej, co powoduje wdrożenie szerokiej diagnostyki. Poniżej przedstawiono
opis pacjenta z depresją w przebiegu choroby Alzheimera, która przejawiała się wieloma objawami
somatycznymi, połączonymi z występowaniem urojeń choroby nowotworowej. Taki przypadek może
stanowić dla lekarzy poważny problem diagnostyczny.Somatic complains are often seen in patients with depression. Its course might resemble the neoplastic
process and lead to wide diagnostic attemps. A case study of a patient with depression in course of
Alzheimer disease who demonstrated many somatic symptoms connected with delusion of cancer is presented.
Such a case may cause diagnostic problems for physicians
Postrzeganie kontynuacji palenia papierosów po rozpoznaniu choroby nowotworowej — wyniki badania jakościowego przeprowadzonego wśród pacjentów chorych na raka płuca
Introduction. Continuation of smoking by people with cancer is an important factor that influences the course of treatment and prognosis. Therefore, it is important to help smokers to stop smoking even after cancer diagnosis. The aim of the study was to analyze the process of smoking continuation by patients with lung cancer. Materials and methods: The study used a qualitative method — individual in-depth interviews. Interviews were conducted with three patients in the terminal phase of lung cancer. Interviews were recorded, followed by preparation of transcripts. Content analysis technique was used for the data analysis. Results: Based on the analysis of transcripts two categories were found: reasons of smoking and the impact of the diagnosis of lung cancer on smoking. Respondents indicated stress and its reduction after lighting a cigarette as the main reason of smoking. All participants were highly motivated to stop smoking after the diagnosis of cancer. They made such an attempt in a spontaneous manner, hoping that a strong will suffice to deal with addiction to nicotine. They all went back to smoking seeing reasons for failure in the absence of strong will and stress. Conclusions: Smokers with a diagnosis of cancer require professional support for more effective smoking cessation. Wstęp. Kontynuacja palenia tytoniu przez osoby z chorobą nowotworową jest ważnym czynnikiem negatywnie wpływającym na przebieg leczenia i rokowanie. Dlatego istotna jest pomoc palaczom w zaprzestaniu palenia również po rozpoznaniu choroby nowotworowej. Celem badania była analiza procesu kontynuacji palenia przez osoby z rozpoznana chorobą nowotworową płuc. Materiał i metody. W badaniu zastosowano metodę jakościową — indywidualne wywiady pogłębione. Przeprowadzono wywiady z trzema pacjentami w terminalnej fazie choroby nowotworowej płuc. Wywiady były nagrywane, następnie przygotowano transkrypty. Do analizy zebranych danych zastosowano technikę analizy treści. Wyniki. Na podstawie analizy transkryptów odkryto dwie kategorie: przyczyny palenia i wpływ rozpoznania choroby nowotworowej płuc na palenie. Jako główny powód palenia badani wskazywali stres i jego złagodzenie po zapaleniu papierosa. Po rozpoznaniu choroby nowotworowej wszyscy byli silnie zmotywowani do zaprzestania palenia. Podjęli taką próbę w sposób spontaniczny, licząc, że silna wola wystarczy do poradzenia sobie z uzależnieniem od nikotyny. Wszyscy powrócili do palenia, upatrując przyczyny niepowodzenia w braku silnej woli i stresie. Wnioski. Pacjenci po rozpoznaniu choroby nowotworowej wymagają profesjonalnego wsparcia w celu skuteczniejszego zaprzestania palenia
