Efficacy of pharmacological treatment of hypertension in patients hospitalized in cardiac rehabilitation centre
Wstęp Znajomość rozpowszechnienia, wykrywalności
i skuteczności leczenia nadciśnienia tętniczego
(HT) ma kluczowe znaczenie u chorych obciążonych
wysokim ryzykiem sercowo-naczyniowym. Celem
niniejszej pracy była ocena skuteczności leczenia
HT u chorych hospitalizowanych na oddziałach
rehabilitacji kardiologicznej Górnośląskiego Centrum
Rehabilitacji w Tarnowskich Górach.
Materiały i metody Epidemiologiczne badanie przekrojowe
przeprowadzono wśród 100 pacjentów, 50 mężczyzn i 50 kobiet, w wieku 64,8 ± 9,6 roku.
Skuteczność kontroli ciśnienia tętniczego (BP) badano
w podgrupach definiowanych przez płeć, obecność
choroby wieńcowej, cukrzycy, nadmiernej masy
ciała oraz liczby stosowanych leków hipotensyjnych.
Za próg skutecznej terapii HT uznano osiągnięcie
wartości BP równej 130/80 mm Hg.
Wyniki Najwięcej osób przyjmowało 2 (52% badanych)
lub 3 leki hipotensyjne (30% badanych). Najczęściej
stosowano β-adrenolityki (81 osób) oraz inhibitory
konwertazy angiotensyny (69 osób). Skuteczną
kontrolę BP stwierdzono u 69% osób (72%
kobiet i 66% mężczyzn; p = 0,5). Skuteczność leczenia
była zbliżona u osób z chorobą wieńcową oraz
bez niej (68% oraz 80%; p = 0,2) oraz u osób z cukrzycą
oraz bez niej (64% i 71,6%; p = 0,5). Pożądane
wartości BP uzyskano u 21,7% pacjentów ze wskaźnikiem
masy ciała (BMI) większym lub równym 25 kg/m²
oraz u 66,2% badanych z prawidłowym BMI
(p < 0,001). Największą skuteczność leczenia (85%)
zaobserwowano u osób przyjmujących jeden lek hipotensyjny,
następnie 2 leki - 71%, 3 leki - 67%
oraz 4 leki - 20% badanych (p = 0,009).
Wnioski Skuteczność leczenia HT u osób hospitalizowanych
na oddziałach rehabilitacji kardiologicznej
wynosi 69%. Płeć, współistnienie cukrzycy lub
choroby wieńcowej nie wpływa na skuteczność terapii
hipotensyjnej. Występowanie nadwagi i otyłości
wpływa negatywnie na skuteczność leczenia HT.
Liczba stosowanych leków hipotensyjnych ujemnie
koreluje ze skutecznością kontroli BP.

efficacy of treatment of hypertension (HT) plays key role
in patients with high cardiovascular risk. The aim of the
study was to evaluate efficacy of treatment of HT in patients
hospitalized in Upper-Silesian Cardiac Rehabilitation
Centre in Tarnowskie Gory.
Material and methods Cross-sectional study was performed
in 100 patients, 50 men and 50 women, aged 64.8 ± 9.6 y.
Efficacy was assessed by gender, occurrence of coronary
heart disease, diabetes, high body mass index and a number
of hypertensive agents. A threshold of blood pressure (BP)
130/80 mm Hg was established as a proper BP control.
Results Most patients used 2 (52%) or 3 (30%) BP lowering
agents. Beta-blockers and ACE inhibitors were the
most frequently used drugs (in 81 and 69 patients, respectively).
A proper BP control was found in 69% of patients
(72% of women and 66% of men, p = 0.5). Efficacy of
treatment was similar in patients with and without coronary
artery disease (68% and 80%, p = 0.2) and in patients
with or without diabetes (64% and 71.6%, p = 0.5). Effective
treatment was lower in those with BMI ≥ 25 kg/m²
(21.7%) compared with non-overweight persons (66.2%),
p < 0.001. The highest efficacy was revealed in patients using one (85%) or 2 drugs (71%) compared with those
taking 3 (67%) or 4 hypotensive agents (20%), p = 0.009.
Conclusions Effective treatment of HT in patients hospitalized
in cardiac rehabilitation centre is 69%. Gender, occurrence
of diabetes or coronary artery disease do not affect
efficacy of HT control. The presence of overweight or obesity
has negative impact on BP control. The number of hypertensive
agents is negatively correlated with effective BP control.

Birth weight and birth length, and blood pressure in children and adolescents
Wstęp Masa urodzeniowa ciała może determinować
wartość ciśnienia tętniczego (BP) w późniejszym
wieku. Celem pracy była ocena wpływu masy i długości
urodzeniowej ciała na wartość BP u dzieci
i młodzieży w Polsce.
Materiał i metody Badanie przekrojowe przeprowadzono
w grupie 6097 osób (2949 chłopców i 3148 dziewcząt) w wieku 7-18 lat (średnio 12,6 ± 3,4
roku), zamieszkałych w woj. śląskim (n = 2386),
mazowieckim (n = 1906) i wielkopolskim (n = 1805).
BP mierzono sfigmomanometrem rtęciowym, zgodnie
z obowiązującymi zaleceniami. Informację
o urodzeniowej masie i długości ciała uzyskano
z danych zawartych w książeczce zdrowia dziecka.
Wyniki Ciśnienie skurczowe (SBP) wynosiło średnio
110,2 ± 11,5 mm Hg, natomiast rozkurczowe
(DBP) - 67,8 ± 7,8 mm Hg. Masa urodzeniowa
ciała wynosiła średnio 3338,2 ± 527,9 g, a długość
urodzeniowa ciała - 53,2 ± 5,5 cm. Parametry te
były były one najwyższe w woj. mazowieckim i wynosiły
odpowiednio: 3370,4 ± 535,4 g i 54,5 ± 3,3
cm. Wartości BP oraz badanych parametrów noworodkowych
były istotnie statystycznie wyższe
u chłopców niż dziewcząt, z wyjątkiem woj. wielkopolskiego.
Po uwzględnieniu roli płci, wieku, aktualnego
BMI, miejsca zamieszkania oraz czasu trwania
ciąży, nie wykazano znamiennego wpływu masy
urodzeniowej ciała (w kg) na wartość BP (odpowiednie
współczynniki regresji liniowej wynosiły: bSBP =
= –0,4; p = 0,3 oraz bDBP = 0,2; p = 0,5). Udokumentowano
natomiast dodatni, istotny wpływ długości
urodzeniowej na wartość BP (bSBP = 0,05;
p = 0,04 oraz bDBP = 0,07; p = 0,0006).
Wnioski Wyniki badania ujawniły niewielki, ale
istotny statystycznie wpływ długości urodzeniowej
na wartość BP w późniejszym wieku oraz brak wpływu
masy urodzeniowej ciała na wartość BP u dzieci
i młodzieży w Polsce.Background Birth weight may influence on blood pressure
(BP) at later life. The study aimed to test whether birth
weight and birth length program blood pressure at later
life in children and adolescents in Poland.
Materials and methods A cross-sectional study was performed
in a randomly selected sample of 6097 children
and adolescents (2949 boys and 3148 girls), aged 7-18
(mean 12.6 ± 3.4), inhabitants of Silesia Region (n = 2386),
Masovia Region (n = 1906) and Wielkopolska Region (n =
= 1805). BP was taken using a mercury sphygmomanometer,
following the current guidelines. Birth outcomes were obtained
from routine obstetrical records.
Results Mean systolic blood pressure (SBP) was 110.2 ±
11.5 mm Hg and diastolic blood pressure (DBP) was
67.8 ± 7.8 mm Hg. Mean birth weight was 3338.2 ± 527.9 g,
and birth length was 53.2 ± 5.5 cm. Both parameters were
the highest in children in Masovia Region; 3370.4 ± 535.4
g and 54.5 ± 3.3 cm, respectively. BP and birth outcomes
were statistically significantly higher in boys than in girls, except
Wielkopolska Region. After adjustment for gender, age,
current BMI, place of residence and time of gestation,
no statistically significant impact of birth weight (in kg)
on BP was found, with the coefficients of linear regression
bSBP = -0.4 (p = 0.3) and bDBP = 0.2 (p = 0.5). However,
statistically significant impact of birth length on BP was confirmed:
bSBP = 0.05 (p = 0.04) and bDBP = 0.07 (p = 0.0006). Conclusions The results revealed small but positive and
statistically significant impact of birth length on BP at later
life in children in Poland. However, no role of birth weight
in BP alteration was found
Perioperative management of cardiac surgery patients who are at the risk of acute kidney injury
Acute kidney injury is one of the most frequent and clinically important of all postoperative complications in cardiac surgery. It is estimated that almost half of subjects suffer from a deterioration of kidney function after a cardio-pulmonary by-pass. Renal insufficiency impacts upon the outcome in terms of an increase in postoperative morbidity and mortality, and a decrease in quality of life. Recently, a modified and unified classification of cardio-renal syndrome has been devised, which takes into account bilateral association between the heart and the kidneys. Because acute decompensation in heart function leads to acute kidney damage, therefore cardiac surgery-associated acute kidney injury may be recognised as a type 1 cardio-renal syndrome from a pathophysiological point of view. This paper aims to review the current data on the diagnosis of acute kidney injury and preventive strategies that can be implemented in cardiac surgery perioperative care.Acute kidney injury is one of the most frequent and clinically important of all postoperative complications in cardiac surgery. A Multi-Centre, Randomised, Double-Blind, Placebo-Controlled Phase III Clinical Trial Evaluating the Impact of BCG Re-Vaccination on the Incidence and Severity of SARS-CoV-2 Infections among Symptomatic Healthcare Professionals during the COVID-19 Pandemic in Poland—Evaluation of Antibody Concentrations
Tuberculosis (TB) was the predominant cause of death from a single infectious agent worldwide before the severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) pandemic. Although TB vaccines have been successfully used for about 100 years, their full effect is still unknown. In previous studies, a reduced incidence and mortality from a coronavirus disease in TB-vaccinated populations were reported. In this article, we present the secondary analysis of a randomised controlled trial, reporting the results of a serological assessment evaluating the effect of the Bacillus Calmette–Guérin (BCG) vaccine on SARS-CoV-2. Participants—healthcare workers—were assessed 1–2 and 8 months after the second dose of the coronavirus disease 2019 (COVID-19) vaccine. We found no associations between antibody concentration, BCG revaccination, and additional characteristics, such as age, gender, or Body Mass Index. The effect of BCG vaccination on the immunological response against SARS-CoV-2 requires further research
