    Clinical presentation and outcome of pediatric ANCA-associated glomerulonephritis

    Background Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis is a small- and medium-sized vasculitis classically seen in adult patients, with peak onset near the fifth to seventh decade of life. There is little data on ANCA-associated vasculitis in pediatric patients, and most studies have limited follow-up. Methods This is a retrospective chart review of 22 patients with ANCA-positive glomerulonephritis in a single institution from 1991 to 2013. Results Of the 22 patients, eight (36 %) required renal replacement therapy (RRT) at diagnosis; four of these patients recovered sufficient renal function to initially discontinue dialysis. Five patients (23 %) were treated with plasmapheresis at presentation. The median time from presentation until first clinical or serologic relapse was 1.7 ± 1.2 years. After a median follow-up of 5.8 years, just over half of our patients had chronic kidney disease (CKD) stages 1–3 (55 %). Seven (32 %) patients progressed to end-stage renal disease (ESRD) and eventually required kidney transplant. Conclusion ANCA-associated glomerulonephritis is a rare disorder in children. Presentation and outcomes vary significantly among patients. More research is required to follow these patients who are diagnosed in childhood to further characterize the long-term outcome of the disease

    Evaluation of T2Candida Panel for detection of Candida in peritoneal dialysates

    Fungal peritonitis in the peritoneal dialysis population is difficult to diagnose promptly due to the inherently slow cultivation-based methods currently required for identification of peritonitis pathogens. Because of the moderate risk for severe complications, the need for rapid diagnostics is considerable. One possible solution to this unmet need is the T2Candida Panel, a new technology designed to detect the most common pathogenic Candida spp. directly from whole blood specimens in as little as a few hours. We hypothesized that this technology could be applied to the detection of Candida in peritoneal dialysate, a matrix not currently approved by the Food and Drug Administration for testing by this system. Remnant dialysate samples from three healthy (noninfected) pediatric peritoneal dialysis patients were spiked with Candida glabrata, serially diluted, and tested in triplicate with unaltered dialysate specimens. The assay detected C. glabrata in 100% of spiked dialysate samples across the full spectrum of dilutions tested, and no assay inhibition or cross-reactivity was noted. These findings suggest one of possibly more applications of this technology. The positive clinical implications of this test will continue to be realized as its use is validated in peritoneal dialysate and other patient specimen types

    BTN3A2 Expression in Epithelial Ovarian Cancer Is Associated with Higher Tumor Infiltrating T Cells and a Better Prognosis

    BTN3A2/BT3.2 butyrophilin mRNA expression by tumoral cells was previously identified as a prognostic factor in a small cohort of high grade serous epithelial ovarian cancer (HG-EOC). Here, we evaluated the prognostic value of BT3.2 at the protein level in specimen from 199 HG-EOC patients. As the only known role of butyrophilin proteins is in immune regulation, we evaluated the association between BT3.2 expression and intratumoral infiltration of immune cells by immunohistochemistry with specific antibodies against BT3.2, CD3, CD4, CD8, CD20, CD68 and CD206. Epithelial BT3.2 expression was significantly associated with longer overall survival and lower risk of disease progression (HR = 0.651, p = 0.006 and HR = 0.642, p = 0.002, respectively) and significantly associated with a higher density of infiltrating T cells, particularly CD4+ cells (0.272, p<0.001). We also observed a strong association between the relative density of CD206+ cells, as evaluated by the ratio of intratumoral CD206+/CD68+ expression, and risk of disease progression (HR = 1.355 p = 0.044, respectively). In conclusion, BT3.2 protein is a potential prognostic biomarker for the identification of HG-EOC patients with better outcome. In contrast, high CD206+/CD68+ expression is associated with high risk of disease progression. While the role of BT3.2 is still unknown, our result suggest that BT3.2 expression by epithelial cells may modulates the intratumoral infiltration of immune cells

    Return to 'a new normal': discourses of resilience to natural disasters in Australian newspapers 2006–2010

    Resilience, as a concept that conceptualises response to change, is gaining currency in the public discourse. In Australia, the term 'resilience' is frequently used by the news media in connection with natural disasters. Media representation of resilience to natural disasters—as rapid onset events characterised by visible thresholds—provides an instructive lens through which to learn about resilience, and bring into focus, differences between academic and broader public perspectives on the concept. In this paper we analyse resilience discourses in Australian newspaper articles from 2006 to 2010. We consider the use of the term 'resilience' and three attributes of resilience that are important in determining how communities respond to disasters: structure and function, self-organisation, and learning and adaptation. Our results show that while the media discourse helps to illuminate what makes communities resilient to disasters, it also highlights how resilience can be undermined when: the term, used most often by actors at from outside the affected community, becomes an 'aspirational rhetorical device'; place attachment manifests as 'lock in' whereby individuals cannot easily leave a disaster-affected community; emphasis post disaster is on reinstating the status quo rather than encouraging transformation; and excessive or inequitably distributed external assistance to a community threatens self-efficacy and cohesion. Media discourse tends to lack reflection on learning beyond formal preparedness programs, but places value on sharing experience. Our analysis has theoretical and practical outcomes: theoretically, this analysis further enriches the descriptions of the three attributes as central concepts in resilience theory. Practically, this work highlights the difficulty in communicating about resilience to encourage constructive response to disasters, but also provides insight into making resilience theory more accessible and relevant to the disaster management community for Australia and globally
