High doses of simvastatin in ACS decrease serum PDGF levels without influencing immune activation
Background: The positive effects of statin therapy in acute coronary syndromes (ACS) may
result from their anti-inflammatory and anti-thrombotic effects. The aim of the study was to
compare the influence of standard and high-dose statin therapies in ACS on the serum markers
of immune and platelet activation.
Material and methods: We examined 44 patients with ACS randomised into two groups:
Group S(+) - 22 patients with ACS who were administered high doses of simvastatin (80 mg
per day) over a period of one month from a cardiac event; Group S(–) - 22 patients with ACS
treated by standard doses of statins. In all patients successful percutaneous coronary interventions
(PCI) were performed. Laboratory analyses were performed at the baseline on the 7th and
30th days from an ACS and involved the following: platelet-derived growth factor (PDGF),
tumour necrosis factor (TNF) alpha, soluble forms of TNF receptor (sTNFR 1 and 2),
Interleukin-2 (IL-2), and IL-10.
Results: During a one-month follow-up we found no difference between clinical data and the
baseline levels of the assessed markers in the groups examined. There were no differences in
the consecutive measurements of TNF-a, sTNFR1, sTNFR 2, IL-2, and IL-10 levels. Serum
concentrations of PDGF were significantly lower on the 7th and 30th days in group S(+)
(7th: 6111 ± 1834 pg/ml, p = 0.037; 30th: 5735 ± 1089 pg/ml, p = 0.016, respectively) in
comparison to group S(–) (7th: 7292 ± 1952 pg/ml; 30th: 7034 ± 2008 pg/ml, respectively).
Conclusions: High doses of simvastatin administered over a period of one month following an
acute coronary syndrome were associated with a significant decrease in serum PDGF levels
without influence on the activation of serum immune markers
Zmniejszanie osoczowego stężenia płytkopodobnego czynnika wzrostu przez duże dawki simwastatyny nie wpływa na stężenia markerów aktywacji immunologicznej
Wstęp: Korzystny efekt wczesnego stosowania statyn w ostrych zespołach wieńcowych (ACS)
może wynikać z ich działania przeciwzapalnego i przeciwpłytkowego. Celem przedstawionego
badania było porównanie wpływu standardowych i dużych dawek statyn w ACS na osoczowe
wskaźniki markerów aktywacji płytek i układu immunologicznego.
Materiał i metody: Czterdziestu czterech pacjentów z ACS leczonych przezskórną angioplastyka
wieńcową przydzielono do 2 grup: S(+) - 22 chorych otrzymujących duże dawki simwastatyny
(80 mg/d.) przez 1 miesiąc od wystąpienia ACS oraz S(–) - 22 pacjentów leczonych
standardowymi dawkami statyn. Laboratoryjne oznaczenia obejmowały określenie stężeń
w surowicy krwi: TNF-α, sTNFR 1 i 2, IL-2, IL-10 oraz PDGF wyjściowo, po 7. i 30. dniu od
wystąpienia ostrego zespołu wieńcowego.
Wyniki: Przebieg kliniczny ACS i wyjściowe stężenia ocenianych markerów były porównywalne
w badanych grupach. Nie odnotowano różnic w kolejnych oznaczeniach stężeń TNF-α,
sTNFR 1, sTNFR 2, IL-2 i IL-10. Stężenie PDGF korelowało negatywnie z terapią dużą
dawką simwastatyny (r = –0,416; p = 0,014) i było istotnie mniejsze w 7. i 30. dniu w grupie
S(+) (odpowiednio: 6111 ± 1834 pg/ml; p = 0,037 i 5735 ± 1089 pg/ml; p = 0,016)
w porównaniu z grupą S(–) (odpowiednio: 7292 ± 1952 pg/ml; 7034 ± 2008 pg/ml).
Wnioski: Stosowanie dużych dawek simwastatyny w ciągu 1 miesiąca od ACS wiąże się
z istotnym obniżeniem osoczowego stężenia PDGF, natomiast nie wpływa na wartości stężeń
innych markerów aktywacji immunologicznej

Aneurysm of the sinus of Valsalva is a rare cardiac disease. It is usually an asymptomatic anomaly, however when it ruptures, symptoms appear and patient’s condition deteriorates rapidly. We describe a case of a 24-year female with a ruptured aneurysm of the non-coronary sinus of Valsalva. The aneurysm ruptured in the right atrium causing severe hemodynamic complications.
