    Office management of lost intrauterine devices either with or without strings

    AbstractObjectiveTo evaluate the efficacy of our method for retrieval of lost intrauterine devices (IUDs) either with or without strings in an office-based setting.MethodsA total of 38 women underwent retrieval of lost IUD. After preevaluation with ultrasonography and hysteroscopy, a Lin polyp grasper was used to remove the IUD under ultrasound monitoring without using a simultaneous hysteroscopy.ResultsOut of 38 women, 12 (31.6%) had IUD insertion for 10–19 years, whereas in another 12 women (31.6%), the duration was 20–40 years. Participants were divided into two groups: (1) premenopausal group (n = 21). The removed IUDs were 11 Chinese IUDs, seven FD-1 IUDs, one Yusei ring IUD, one Lippe loop IUD, and one Mirena IUD; and (2) postmenopausal group (n = 17). The removed IUDs were five soft type Ota ring IUDs, eight FD-1 IUDs, one Saf-T-Coil IUD, one KS wing IUD, and one Chinese IUD. A very hard type Ota ring IUD inserted for 40 years could not be removed. All of the other IUDs were removed uneventfully. Most of the patients could tolerate the procedure without the use of analgesia or anesthesia. No subsequent complication except bleeding for several days was encountered.ConclusionUsing our method, lost IUDs either with or without strings can be effectively and safely retrieved in the office-based setting without analgesia or anesthesia

    Near-infrared and Mid-infrared Spectroscopy with the Infrared Camera (IRC) for AKARI

    The Infrared Camera (IRC) is one of the two instruments on board the AKARI satellite. In addition to deep imaging from 1.8-26.5um for the pointed observation mode of the AKARI, it has a spectroscopic capability in its spectral range. By replacing the imaging filters by transmission-type dispersers on the filter wheels, it provides low-resolution (lambda/d_lambda ~ 20-120) spectroscopy with slits or in a wide imaging field-of-view (approximately 10'X10'). The IRC spectroscopic mode is unique in space infrared missions in that it has the capability to perform sensitive wide-field spectroscopic surveys in the near- and mid-infrared wavelength ranges.     The Canal, a vibrant community of Latino immigrant families, is rich in diversity and cultural traditions, strong family networks, and a determination towards economic selfsufficiency. Latino immigrants in Marin County are heavily concentrated in the Canal and have the highest labor force participation rates in the County.i Despite being a vital part of Marin’s social, economic, and cultural society, Canal residents continue to struggle to meet basic necessities for their families. To this end, a coalition of resident leaders from the community came together to form Voces del Canal to lead an unprecedented community-driven research project. Residents wanted to affirm their role as authentic, local experts and give power to the collective experiences of 678 residents who shared their personal stories, opinions, and vision for a stronger and safer Canal

    Purpose Delayed hyponatremia can occur after pituitary surgery, resulting in prolonged hospitalization. However, the influence of surgical factors after such a procedure has not been well established. The impact of surgery and related factors on delayed hyponatremia was investigated. Methods This was a retrospective analysis of 137 consecutive patients who underwent transsphenoidal surgery for a nonfunctioning pituitary adenoma between 2008 and 2019. Preoperative (demographics, comorbidities), intraoperative (resection extent, operation time, blood loss volume, cerebrospinal fluid leak, tumor consistency), and postoperative [hematoma, meningitis, diabetes insipidus (DI), hormonal assessment] data were collected, with statistical analysis of each factor performed. Results Among the 137 patients, delayed hyponatremia occurred in 31 (22.6%). Multivariate analysis revealed that those with hypertension had a significantly higher likelihood of avoiding delayed hyponatremia (p = 0.004). Although no correlations of direct surgical factors with delayed hyponatremia were found, multivariate analysis of indirect surgical factors showed that presence of a firm tumor, transient DI, and meningitis were significantly associated with delayed hyponatremia (p = 0.014, 0.001, and 0.047, respectively). There was also a significant association of severe hyponatremia with appearance of symptoms (p = 0.002). Conclusion There was a tendency for hypertension to be associated with delayed hyponatremia avoidance, with indirect surgical factors including tumor consistency, transient DI, and meningitis found to have an influence on delayed hyponatremia. It was concluded that attention should be given to non-hypertensive patients with a firm tumor, transient DI, or meningitis after pituitary surgery, as delayed hyponatremia may occur.博士(医学)・甲第871号・令和5年3月15

    Human endogenous retrovirus K14C drove genomic diversification of the Y chromosome during primate evolution

    金沢大学医薬保健研究域医学系The male-specific region of Y chromosome (MSY) has accumulated a higher density of human endogenous retroviruses (HERVs) and related sequences when compared with other regions of the human genome. Here, we focused on one HERV family, HERV-K14C that seemed to integrate preferentially into the Y chromosome in humans. To identify every copies of HERV-K14C in the human genome, we applied computational screening to map precisely the locus of individual HERV-K14C copies. Interestingly, 29 of all 146 copies were located in Y chromosome, and these 29 copies were mostly dispersed in the palindromic region. Three distinct HERV-K14C-related transcripts were found and were exclusively expressed in human testis tissue. Based on our phylogenetic analysis of the solitary LTRs derived from HERV-K14C on the Y chromosome we suggested that these sequences were generated as pairs of identical sequences. Specifically, analysis of HERV-K14C-related sequences in the palindromic region demonstrated that the Y chromosomal amplicons existed in our common ancestors and the duplicated pairs arose after divergence of great apes approximately 8-10 million years ago. Taken together, our observation suggested that HERV-K14C-related sequences contributed to genomic diversification of Y chromosome during speciation of great ape lineage. © 2010 The Japan Society of Human Genetics All rights reserved

    Enhanced Nogo-P3 amplitudes of mothers compared with non-mother women during an emotional Go/Nogo task

    Background: It is known that emotion regulatory responses of humans are changed by the experiences they have, but in particular, they are changed by becoming a mother. A recent study has found how a woman's emotion regulatory response to a child's crying changes after becoming a mother. However, mothers' emotion regulatory responses other than those to children and the association between emotion regulatory response and parental stress are still unknown. Methods: Eighteen healthy Japanese females (nine mothers and nine non-mothers) participated in the experiment. They performed an emotional Go/Nogo task, with facial expressions of others (angry, happy, and neutral faces) used as emotional stimuli. The percentage of correct responses, response time, and event-related potentials (ERPs) during the task was measured. Results: This comparison revealed that the mother group had a larger P3 (Nogo-P3) amplitude than the non-mother group when Nogo trials were held. This indicates that in mothers, there was greater activation of the behavioral inhibition-related brain areas than in non-mother women when they inhibited inappropriate behavior following recognition of facial expressions of others. In addition, in the mother group, there was a negative correlation between parental stress levels and Nogo-P3 amplitudes evoked by angry faces. This suggests that there is a relation between the level of parental stress of mothers and their emotion regulatory responses to angry faces. Conclusions: Our results demonstrate that mothers' emotion regulatory processes may differ from those of non-mothers in response, not only to a child's crying but also to expressions of emotions by others, and also suggest that the inhibitory recognition activity of mothers can be affected by parental stres

    Molecular Characteristics of Extended-Spectrum Beta-Lactamases and qnr Determinants in Enterobacter Species from Japan

    The incidence of extended-spectrum β-lactamases (ESBLs) has been increasing worldwide, but screening criteria for detection of ESBLs are not standardized for AmpC-producing Enterobacteriaceae such as Enterobacter species. In this study, we investigated the prevalence of ESBLs and/or AmpC β-lactamases in Japanese clinical isolates of Enterobacter spp. and the association of plasmid-mediated quinolone resistance (PMQR) determinants with ESBL producers. A total of 364 clinical isolates of Enterobacter spp. collected throughout Japan between November 2009 and January 2010 were studied. ESBL-producing strains were assessed by the CLSI confirmatory test and the boronic acid disk test. PCR and sequencing were performed to detect CTX-M, TEM, and SHV type ESBLs and PMQR determinants. For ESBL-producing Enterobacter spp., pulsed-field gel electrophoresis (PFGE) was performed using XbaI restriction enzyme. Of the 364 isolates, 22 (6.0%) were ESBL producers. Seven isolates of Enterobacter cloacae produced CTX-M-3, followed by two isolates producing SHV-12. Two isolates of Enterobacter aerogenes produced CTX-M-2. Of the 22 ESBL producers, 21 had the AmpC enzyme, and six met the criteria for ESBL production in the boronic acid test. We found a significant association of qnrS with CTX-M-3-producing E. cloacae. The 11 ESBL-producing Enterobacter spp. possessing blaCTX-M, blaSHV, or blaTEM were divided into six unique PFGE types. This is the first report about the prevalence of qnr determinants among ESBL-producing Enterobacter spp. from Japan. Our results suggest that ESBL-producing Enterobacter spp. with qnr determinants are spreading in Japan
