    Radiation pneumonitis following Yttrium-90 radioembolization: A Korean multicenter study

    ObjectiveTo report the incidence of radiation pneumonitis after radioembolization.MethodsIn this retrospective study, from May 2009 to July 2021, 782 consecutive patients underwent radioembolization in two institutes. Medical internal radiation dose dosimetry and partition dosimetry were used for glass and resin Yttrium-90-labeled microspheres (90Y-microspheres), respectively. Medical records and radiological findings were retrospectively evaluated with emphasis on the symptomatic radiation pneumonitis.ResultsOf the 732 patients with lung shunt study and follow-up, 13 (1.8%) had symptomatic radiation pneumonitis and six patients died due to radiation pneumonitis. Of the 721 patients whose lung doses were calculated, 10 patients who were treated with glass (n = 5) and resin (n = 5) 90Y-microspheres had radiation pneumonitis. No significant statistical difference between glass and resin 90Y-microspheres (p = 0.304) was noted in terms of radiation pneumonitis incidence. Among the patients with radiation pneumonitis, all five patients treated with glass 90Y-microspheres had estimated lung doses > 29 Gy, whereas five patients treated with resin 90Y-microspheres had relatively wide range of lung dose reaching much lower value (13.21Gy).ConclusionThe present study suggests that radiation pneumonitis after radioembolization may occur even though the manufacturer’s instructions are followed

    AbstractThe present report describes a case of successful embolization of the hepatic artery pseudoaneurysm in a 61-year-old female patient with celiac axis occlusion. Because of celiac artery occlusion, the hepatic artery had to be catheterized through the pancreaticoduodenal arcades and the gastroduodenal artery (GDA) from the superior mesenteric artery (SMA). Despite coaxial catheterization using a torque guide wire, the proper hepatic artery (PHA) could not be catheterized because of the acute angle between the GDA and the PHA. The use of the microcatheter looping technique facilitated catheterization of the PHA and subsequent embolization of the right hepatic artery pseudoaneurysm

    Numerical Sensitivity Tests of Volatile Organic Compounds Emission to PM2.5 Formation during Heat Wave Period in 2018 in Two Southeast Korean Cities

    A record-breaking severe heat wave was recorded in southeast Korea from 11 July to 15 August 2018, and the numerical sensitivity simulations of volatile organic compound (VOC) to secondarily generated particulate matter with diameter of less than 2.5 mu m (PM2.5) concentrations were studied in the Busan and Ulsan metropolitan areas in southeast Korea. A weather research and forecasting (WRF) model coupled with chemistry (WRF-Chem) was employed, and we carried out VOC emission sensitivity simulations to investigate variations in PM2.5 concentrations during the heat wave period that occurred from 11 July to 15 August 2018. In our study, when anthropogenic VOC emissions from the Comprehensive Regional Emissions Inventory for Atmospheric Transport Experiment-2015 (CREATE-2015) inventory were increased by approximately a factor of five in southeast Korea, a better agreement with observations of PM2.5 mass concentrations was simulated, implying an underestimation of anthropogenic VOC emissions over southeast Korea. The simulated secondary organic aerosol (SOA) fraction, in particular, showed greater dominance during high temperature periods such as 19-21 July, 2018, with the SOA fractions of 42.3% (in Busan) and 34.3% (in Ulsan) among a sub-total of seven inorganic and organic components. This is considerably higher than observed annual mean organic carbon (OC) fraction (28.4 +/- 4%) among seven components, indicating the enhancement of secondary organic aerosols induced by photochemical reactions during the heat wave period in both metropolitan areas. The PM2.5 to PM10 ratios were 0.69 and 0.74, on average, during the study period in the two cities. These were also significantly higher than the typical range in those cities, which was 0.5-0.6 in 2018. Our simulations implied that extremely high temperatures with no precipitation are significantly important to the secondary generation of PM2.5 with higher secondary organic aerosol fraction via photochemical reactions in southeastern Korean cities. Other possible relationships between anthropogenic VOC emissions and temperature during the heat wave episode are also discussed in this study

    Transcatheter Arterial Chemoembolization of Hepatocellular Carcinoma: Prevalence and Causative Factors of Extrahepatic Collateral Arteries in 479 Patients

    OBJECTIVE: We wanted to investigate the prevalence and causative factors of extrahepatic arterial blood supply to hepatocellular carcinoma (HCC) at its initial presentation and during chemoembolization. MATERIALS AND METHODS: Between February 1998 and April 2000, consecutive 479 patients with newly diagnosed HCC were prospectively enrolled into this study. A total of 1629 sessions of transcatheter arterial chemoembolization (TACE) were performed in these patients (range: 1-15 sessions; mean: 3.4 sessions) until April 2004. For each TACE procedure, we determined the potential extrahepatic collateral arteries (ExCAs) depending on the location of the tumor, and we performed selective angiography of all suspected collaterals that could supply the tumor. The prevalence of ExCAs and the causative factors were analyzed. RESULTS: At initial presentation, 82 (17%) of these 479 patients showed 108 ExCAs supplying tumors. Univariate analysis showed that tumor size (p or =5 cm) was significantly higher than that for those patients with a small tumor (< 5 cm) (p < 0.01). CONCLUSION: The presence of ExCAs supplying HCC is rather common, and the tumor size is a significant causative factor for the development of these collateral arteries.This study was supported by a grant (0620220-1) from the National R & D Program for Cancer Control, Ministry of Health & Welfare, Republic of Korea

    Genetic and Metabolic Characterization of Insomnia

    Insomnia is reported to chronically affect 10∼15% of the adult population. However, very little is known about the genetics and metabolism of insomnia. Here we surveyed 10,038 Korean subjects whose genotypes have been previously profiled on a genome-wide scale. About 16.5% reported insomnia and displayed distinct metabolic changes reflecting an increase in insulin secretion, a higher risk of diabetes, and disrupted calcium signaling. Insomnia-associated genotypic differences were highly concentrated within genes involved in neural function. The most significant SNPs resided in ROR1 and PLCB1, genes known to be involved in bipolar disorder and schizophrenia, respectively. Putative enhancers, as indicated by the histone mark H3K4me1, were discovered within both genes near the significant SNPs. In neuronal cells, the enhancers were bound by PAX6, a neural transcription factor that is essential for central nervous system development. Open chromatin signatures were found on the enhancers in human pancreas, a tissue where PAX6 is known to play a role in insulin secretion. In PLCB1, CTCF was found to bind downstream of the enhancer and interact with PAX6, suggesting that it can probably inhibit gene activation by PAX6. PLCB4, a circadian gene that is closely located downstream of PLCB1, was identified as a candidate target gene. Hence, dysregulation of ROR1, PLCB1, or PLCB4 by PAX6 and CTCF may be one mechanism that links neural and pancreatic dysfunction not only in insomnia but also in the relevant psychiatric disorders that are accompanied with circadian rhythm disruption and metabolic syndrome

    Follow-up CT Evaluation of the Mural Changes in Active Takayasu Arterities

    OBJECTIVE: We wanted to evaluate the mural changes by CT on the follow-up examination of patients with active Takayasu arteritis. MATERIALS AND METHODS: The study included 18 patients, (4 males and 14 females), with active Takayasu arteritis. A total of 44 CT examinations were done during the follow-up period (mean: 55.6 months). At the time of the last follow-up CT, the disease, on the basis of the erythrocyte sedimentation rate (ESR), was found to be inactive in five patients and the disease was active and persistent in 13 patients. The thickness and CT attenuation of the aortic wall on the precontrast, arterial and venous phases were measured on the initial and the follow-up CT examinations. The ratio of the mural attenuation over that of the back muscle on the initial CT was compared with the ratio found on the follow-up CT. RESULTS: The initial CT findings included high density and calcifications of the aortic wall in the precontrast images and a thickened wall with enhancements in the arterial and the venous phases. A low-attenuation ring was demonstrated in the venous phase in 15 patients (83%). On the follow-up evaluation, the mean mural thickness decreased significantly from 4.1 mm to 2.4 mm. The mean mural attenuation ratio in the venous phase decreased significantly from 1.9 to 1.3 (p = 0.001). The low attenuation ring was identified in seven patients (39%) who had only with active, persistent Takayasu arteritis. CONCLUSION: The mural changes demonstrated by the follow-up CT evaluations for the patients with active Takayasu arteritis included a decrease of the mural thickness and enhancement, disappearance of the low-attenuation ring on the venous phase, and an increase of the mural attenuation and calcification on the precontrast phase

    Local Corrosion of the Oxide Passivation Layer during Cu Chemical Mechanical Polishing

    In this article, we analyze the effect of complexing agents in Cu chemical mechanical polishing slurry on the formation of oxide and the evolution of stress. The passivation property and surface morphology of the oxide on the surface showed significant differences depending on the kind of complexing agent. Oxalic acid showed fast oxide formation with poor passivation performance, and this caused large tensile stress evolution over 250 MPa in the Cu film.
