    Socioeconomic Differences in Antenatal Care between the United States and Scandinavia

    Despite their analogous status as economically developed nations, the United States and Scandinavian countries have marked differences in their healthcare systems. In particular both areas discernibly differ in the antenatal treatment provided for expecting women and their babies. Sweden and Denmark’s healthcare systems are universal, run primarily on taxpayer dollars, and provide equal antenatal care regardless of socioeconomic status. The United States’ healthcare system is run on a combination of private and government run insurance, in which socioeconomic status often determines insurance coverage. This variability in insurance coverage often results in differing levels of antenatal care. An overarching question remains as to how women of low socioeconomic status receive differing antenatal care in the United States and Scandinavia. Antenatal care discrepancies between the two systems emanate a difference in patient outcomes and patient satisfaction of their treatment. Analyzing the differences in these outcomes can better point to which health care system provides more effective antenatal care. Women of lower socioeconomic status in Sweden and Denmark receive superior antenatal care than women of a comparable socioeconomic status in the United States. [excerpt

    Evolution of the Antarctic polar vortex in spring: Response of a GCM to a prescribed Antarctic ozone hole

    The possible effect of the Antartic ozone hole on the evolution of the polar vortex during late winter and spring using a general circulation model (GCM) is examined. The GCM is a version of the NCAR Community Climate Model whose domain extends from the surface to the mesosphere and is similar to that described on Boville and Randel (1986). Ozone is not a predicted variable in the model. A zonally averaged ozone distribution is specified as a function of latitude, pressure and month for the radiation parameterization. Rather that explicitly address reasons for the formation of the ozone hole, researchers postulate its existence and ask what effect it has on the subsequent evolution of the vortex.     Nitrogen Dynamics from Decomposing Litter of \u3ci\u3ePanicum maximum\u3c/i\u3e with Different Nitrogen and Phosphorus Content in Brazilian Alfissol

    The objective of this study was to measure the dynamics (immobilization and release) of N and to evaluate the effect of the initial chemical composition of four Panicum maximum cultivars grown in a Alfisol and fertilized with different levels of nitrogen (0, 80 e 160 kg ha-1 de N) and phosphorus (0 e 200 kg ha-1), on the release of the N from the litter using the litterbags technique. There was an increase in the litter initial concentration of N with time of decomposition. The Aruana and Vencedor cultivars released about 70 and 60% of N; respectively, during the decomposition of the litter from 0 (zero) to 336 days; the TobiatĂŁ and TanzĂąnia cultivars released about 30 and \u3e30% of N from the concentration of the initial litter respectively. Nitrogen fertization increased the N release, up to 20% in the highest N level tested (160 kg ha-1 of N)

    Modular Organization of Functional Network Connectivity in Healthy Controls and Patients with Schizophrenia during the Resting State

    Neuroimaging studies have shown that functional brain networks composed from select regions of interest have a modular community structure. However, the organization of functional network connectivity (FNC), comprising a purely data-driven network built from spatially independent brain components, is not yet clear. The aim of this study is to explore the modular organization of FNC in both healthy controls (HCs) and patients with schizophrenia (SZs). Resting state functional magnetic resonance imaging data of HCs and SZs were decomposed into independent components (ICs) by group independent component analysis (ICA). Then weighted brain networks (in which nodes are brain components) were built based on correlations between ICA time courses. Clustering coefficients and connectivity strength of the networks were computed. A dynamic branch cutting algorithm was used to identify modules of the FNC in HCs and SZs. Results show stronger connectivity strength and higher clustering coefficient in HCs with more and smaller modules in SZs. In addition, HCs and SZs had some different hubs. Our findings demonstrate altered modular architecture of the FNC in schizophrenia and provide insights into abnormal topological organization of intrinsic brain networks in this mental illness

    Standardized and quality-assured video-recorded examination in undergraduate education: informed consent prior to surgery

    AbstractBackgroundCommunication skills combined with specialized knowledge are fundamental to the doctor–patient relationship in surgery. During a single-station video-recorded objective structured clinical examination (VOSCE), students were tasked with obtaining informed consent. Our aim was to develop a standardized and quality-assured assessment method in undergraduate education.MethodsOne hundred fifty-five students in their fifth year of medical school (78 videos) participated in a summative VOSCE within the framework of the teaching module “Operative Medicine.” They prepared for three clinical scenarios and the surgical procedures involved. The examination comprised participants having to obtain informed consent from simulated patients, video recording their performance. Students were assessed by two independent raters, the background of one of whom was nonsurgical. Results were statistically tested using SPSS.ResultsStudents' scores were all beyond the pass mark of 70%, averaging 91.0% (±4.0%), 88.4% (±4.4%), and 87.0% (±4.7%) for the appendectomy, cholecystectomy, and inguinal hernia repair checklist, respectively. Most items (68%–89% of the checklists) were found to have fair to excellent discrimination values. Cronbach's α values ranged between 0.565 and 0.605 for the individual checklists. Interrater agreement was strong (Pearson correlation coefficient = 0.80, P < 0.01; intraclass correlation coefficient 2.1 = 0.78).ConclusionsThe VOSCE is both feasible and reliable as a method of assessing student communication skills and the application of clinical knowledge while obtaining informed consent in surgery. This method is efficient (flexible rating outside normal working hours possible with reductions in administrative load) and may be used for high-stakes evaluation of student performance

    Regular cannabis and alcohol use is associated with resting-state time course power spectra in incarcerated adolescents

    Cannabis and alcohol are believed to have widespread effects on the brain. Although adolescents are at increased risk for substance use, the adolescent brain may also be particularly vulnerable to the effects of drug exposure due to its rapid maturation. Here, we examined the association between cannabis and alcohol use duration and resting-state functional connectivity in a large sample of male juvenile delinquents.The present sample was drawn from the Southwest Advanced Neuroimaging Cohort, Youth sample, and from a youth detention facility in Wisconsin. All participants were scanned at the maximum-security facilities using The Mind Research Network’s 1.5T Avanto SQ Mobile MRI scanner. Information on cannabis and alcohol regular use duration was collected using self-report. Resting-state networks were computed using group independent component analysis in 201 participants. Associations with cannabis and alcohol use were assessed using Mancova analyses controlling for age, IQ, smoking and psychopathy scores in the complete case sample of 180 male juvenile delinquents.No associations between alcohol or cannabis use and network spatial maps were found. Longer cannabis use was associated with decreased low frequency power of the default mode network, the executive control networks (ECNs), and several sensory networks, and with decreased functional network connectivity. Duration of alcohol use was associated with decreased low frequency power of the right frontoparietal network, salience network, dorsal attention network, and several sensory networks.Our findings suggest that adolescent cannabis and alcohol use are associated with widespread differences in resting-state time course power spectra, which may persist even after abstinence.Development Psychopathology in context: famil

    A major shift to the retention approach for forestry can help resolve some global forest sustainability issues

    Approximately 85% of the global forest estate is neither formally protected nor in areas dedicated to intensive wood production (e.g., plantations). Given the spatial extent of unprotected forests, finding management approaches that will sustain their multiple environmental, economic, and cultural values and prevent their conversion to other uses is imperative. The major global challenge of native forest management is further demonstrated by ongoing steep declines in forest biodiversity and carbon stocks. Here, we suggest that an essential part of such management—supplementing the protection of large reserves and sensitive areas within forest landscapes (e.g., aquatic features)—is the adoption of the retention approach in forests where logging occurs. This ecological approach to harvesting provides for permanent retention of important selected structures (e.g., trees and decayed logs) to provide for continuity of ecosystem structure, function, and species composition in the postharvest forest. The retention approach supports the integration of environmental, economic, and cultural values and is broadly applicable to tropical, temperate, and boreal forests, adaptable to different management objectives, and appropriate in different societal settings.     Correspondence between structure and function in the human brain at rest

    To further understanding of basic and complex cognitive functions, previous connectome research has identified functional and structural connections of the human brain. Functional connectivity is often measured by using resting-state functional magnetic resonance imaging (rs-fMRI) and is generally interpreted as an indirect measure of neuronal activity. Gray matter (GM) primarily consists of neuronal and glia cell bodies; therefore, it is surprising that the majority of connectome research has excluded GM measures. Therefore, we propose that by exploring where GM corresponds to function would aid in the understanding of both structural and functional connectivity and in turn the human connectome. A cohort of 603 healthy participants underwent structural and functional scanning on the same 3 T scanner at the Mind Research Network. To investigate the spatial correspondence between structure and function, spatial independent component analysis (ICA) was applied separately to both GM density (GMD) maps and to rs-fMRI data. ICA of GM delineates structural components based on the covariation of GMD regions among subjects. For the rs-fMRI data, ICA identified spatial patterns with common temporal features. These decomposed structural and functional components were then compared by spatial correlation. Basal ganglia components exhibited the highest structural to resting-state functional spatial correlation (r = 0.59). Cortical components generally show correspondence between a single structural component and several resting-state functional components. We also studied relationships between the weights of different structural components and identified the precuneus as a hub in GMD structural network correlations. In addition, we analyzed relationships between component weights, age, and gender; concluding that age has a significant effect on structural components

    Higher media multi-tasking activity is associated with smaller gray-matter density in the anterior cingulate cortex

    Media multitasking, or the concurrent consumption of multiple media forms, is increasingly prevalent in today’s society and has been associated with negative psychosocial and cognitive impacts. Individuals who engage in heavier media-multitasking are found to perform worse on cognitive control tasks and exhibit more socio-emotional difficulties. However, the neural processes associated with media multi-tasking remain unexplored. The present study investigated relationships between media multitasking activity and brain structure. Research has demonstrated that brain structure can be altered upon prolonged exposure to novel environments and experience. Thus, we expected differential engagements in media multitasking to correlate with brain structure variability. This was confirmed via Voxel-Based Morphometry (VBM) analyses: Individuals with higher Media Multitasking Index (MMI) scores had smaller gray matter density in the anterior cingulate cortex (ACC). Functional connectivity between this ACC region and the precuneus was negatively associated with MMI. Our findings suggest a possible structural correlate for the observed decreased cognitive control performance and socio-emotional regulation in heavy media-multitaskers. While the cross-sectional nature of our study does not allow us to specify the direction of causality, our results brought to light novel associations between individual media multitasking behaviors and ACC structure differences
