    Perception-reality bias: the differences in government trust across income groups

    This paper first measures and compares the size of middle-income groups in China based on the subjective income evaluation method and the objective criteria. Second, it empirically investigates the differences in government trust of different income groups defined by the subjective evaluation method and the objective criteria. It is found that there is a significant difference between the results of the subjective evaluation of income and objective criteria. Compared with individuals in the middle-income group, individuals in the low-income group have a significantly worse overall evaluation of local government and a considerably lower trust in local government officials. On the other hand, individuals in the high-income group have a substantially better assessment of local government and a significantly higher trust in local government officials. However, the differences in trust in government across income groups defined by objective criteria are insignificant overall. In terms of policy insights, the effect of targeting low-income groups determined by subjective evaluation may be more effective in improving peopleā€™s trust in the government

    SMAD3 deficiency promotes vessel wall remodeling, collagen fiber reorganization and leukocyte infiltration in an inflammatory abdominal aortic aneurysm mouse model

    TGF-beta signaling plays critical roles in the pathogenesis of aneurysms; however, it is still unclear whether its role is protective or destructive. In this study, we investigate the role of SMAD3 in the pathogenesis of calcium chloride (CaCl2)-induced abdominal aortic aneurysms (AAA) in Smad3(-/-), Smad3(+/-) and Smad3(+/+) mice. We find that loss of SMAD3 drastically increases wall thickening of the abdominal aorta. Histological analyses show significant vessel wall remodeling with elastic fiber fragmentation. Remarkably, under polarized light, collagen fibers in the hyperplastic adventitia of Smad3(-/-) mice show extensive reorganization accompanied by loosely packed thin and radial collagen fibers. The expressions of matrix metalloproteinases including MMP2, MMP9, and MMP12 and infiltration of macrophage/T cells are drastically enhanced in the vascular wall of Smad3(-/-) mice. We also observe marked increase of NF-kappaB and ERK1/2 signaling as well as the expression of nuclear Smad2, Smad4 and TGF-beta1 in the vessel wall of Smad3(-/-) mice. In addition, we find that SMAD3 expression is reduced in the dedifferentiated medial smooth muscle-like cells of human AAA patients. These findings provide direct in vivo evidence to support the essential roles of SMAD3 in protecting vessel wall integrity and suppressing inflammation in the pathogenesis of AAAs

    YY1 directly interacts with myocardin to repress the triad myocardin/SRF/CArG box-mediated smooth muscle gene transcription during smooth muscle phenotypic modulation

    Yin Yang 1 (YY1) regulates gene transcription in a variety of biological processes. In this study, we aim to determine the role of YY1 in vascular smooth muscle cell (VSMC) phenotypic modulation both in vivo and in vitro. Here we show that vascular injury in rodent carotid arteries induces YY1 expression along with reduced expression of smooth muscle differentiation markers in the carotids. Consistent with this finding, YY1 expression is induced in differentiated VSMCs in response to serum stimulation. To determine the underlying molecular mechanisms, we found that YY1 suppresses the transcription of CArG box-dependent SMC-specific genes including SM22Ī±, SMĪ±-actin and SMMHC. Interestingly, YY1 suppresses the transcriptional activity of the SM22Ī± promoter by hindering the binding of serum response factor (SRF) to the proximal CArG box. YY1 also suppresses the transcription and the transactivation of myocardin (MYOCD), a master regulator for SMC-specific gene transcription by binding to SRF to form the MYOCD/SRF/CArG box triad (known as the ternary complex). Mechanistically, YY1 directly interacts with MYOCD to competitively displace MYOCD from SRF. This is the first evidence showing that YY1 inhibits SMC differentiation by directly targeting MYOCD. These findings provide new mechanistic insights into the regulatory mechanisms that govern SMC phenotypic modulation in the pathogenesis of vascular diseases

    Learning curves and long-term outcome of simulation-based thoracentesis training for medical students

    <p>Abstract</p> <p>Background</p> <p>Simulation-based medical education has been widely used in medical skills training; however, the effectiveness and long-term outcome of simulation-based training in thoracentesis requires further investigation. The purpose of this study was to assess the learning curve of simulation-based thoracentesis training, study skills retention and transfer of knowledge to a clinical setting following simulation-based education intervention in thoracentesis procedures.</p> <p>Methods</p> <p>Fifty-two medical students were enrolled in this study. Each participant performed five supervised trials on the simulator. Participant's performance was assessed by performance score (PS), procedure time (PT), and participant's confidence (PC). Learning curves for each variable were generated. Long-term outcome of the training was measured by the retesting and clinical performance evaluation 6 months and 1 year, respectively, after initial training on the simulator.</p> <p>Results</p> <p>Significant improvements in PS, PT, and PC were noted among the first 3 to 4 test trials (p < 0.05). A plateau for PS, PT, and PC in the learning curves occurred in trial 4. Retesting 6 months after training yielded similar scores to trial 5 (p > 0.05). Clinical competency in thoracentesis was improved in participants who received simulation training relative to that of first year medical residents without such experience (p < 0.05).</p> <p>Conclusions</p> <p>This study demonstrates that simulation-based thoracentesis training can significantly improve an individual's performance. The saturation of learning from the simulator can be achieved after four practice sessions. Simulation-based training can assist in long-term retention of skills and can be partially transferred to clinical practice.</p

    Modified Restriction Enzyme-based Detection of EGFR Mutations in Lung Cancer

    Background and objective Epidermal growth factor receptor (EGFR) gene mutation in lung cancer is a reliable predictor of the efficacy of tyrosine kinase inhibitors, so detection of gene mutation has very important clinical significance. The aim of this study is to optimize conditions for restriction enzyme-based mutant enriched detection of EGFR mutation in lung cancer specimens. Methods EGFR somatic mutations identified in DNA specimens from 251 lung adenocarcinomas with modified mutant enriched method and direct sequencing were compared. The sensitivity of the modified method was determined by serial dilution of mutant-carrying lung cancer cells in wild-type cells. Results 46 deletion mutations in exon 19 and 26 point mutations in exon 21 were found by sequencing, while modified method identified another 78 deletions in exon 19 and 57 substitutions in exon 21 with a mutation rate of 53.8%. Sensitivity of this modified method was 0.5% (mutant/wild type) in serial diluted cells. Conclusion The modified restriction enzyme-based method is convenient for clinical EGFR mutation screening in non-small cell lung cancer for its simpleness, cost-saving and high sensitivity

    Ecological Security Pattern Construction in Rural Settlements Based on Importance and Vulnerability of Ecosystem Services: A Case Study of the Southeast Region of Chongqing, China

    With the development of urbanization, a large number of village settlements have disappeared. As important carriers of ethnic and cultural heritage, village settlements are important for the continuation of folk culture and farming spirit. Building a regional ecological security pattern helps to protect the integrity of village settlements and promote the sustainable development of the Chinese nation. In this work, the importance of ecosystem services was first obtained by using the Integrated Valuation of Ecosystem Services and Tradeoffs model (In-VEST) and the revised universal soil loss equation model (RUSLE) to evaluate the regional ecosystem services in the southeastern region of Chongqing (SRC). Then, the ecological importance evaluation results were combined with the sensory evaluation results to derive ecological conservation importance areas and identify ecological source points in their high-value areas. Finally, the ecological corridors were obtained by the minimum cumulative resistance model (MCR). The regional ecological security pattern was constructed, i.e., the triangular protection area strategy of ā€œone screen, one belt, and three coresā€. The results show the following: the total area of ecological source sites larger than 20 km2 in SRC is 436.02 km2, accounting for 2.28% of the area of SRC. The largest ecological patch is 69.93 km2, accounting for 16.04% of the total area of ecological source sites. Thirteen ecological source sites were identified as follows: four in Youyang County, three in Shizhu County, three in Wulong County, and one in Pengshui County. There are 78 ecological corridors with a total length of 4832.82 km, including 32 important ecological corridors with a length of about 1544.53 km and 46 potential ecological corridors with a length of about 3288.29 km. Based on the minimum cumulative model constructed by eight resistance factors, the spatial variation of the ecological resistance surface was analyzed, showing a trend of being high in the west and central part and low in the surrounding area. There are one high-value area of the resistance surface of Wulong County Nature Reserve and one high-value area of ecological sensitivity that overlap to the extreme, and which should be given sufficient protection attention. The core protected areas consist of three mountains including the Qiyao Mountain Range, the Wuling Mountain Range, and the Wulong County Nature Reserve. The ecological security pattern in SRC based on the mode of ā€œsourceā€“ecological corridorā€“ecological nodeā€ can identify important ecological function areas, providing scientific guidance for sustainable development and ecological security protection in the ethnic village settlements in China

    Association between Serum Adiponectin and Atrial Fibrillation: A Case-Control Study Stratified by Age and Gender

    Background. Circulating adiponectin has been suggested to be associated with atrial fibrillation (AF). However, whether the association differs by age and gender remains unknown. We performed a case-control study to evaluate the above association. Methods. AF patients who underwent 24-hour long-range 12-channel electrocardiogram examination at our center were included in this study, and people with normal sinus rhythm (NSR) were included as controls. All participants underwent echocardiography and heart rate variability tests. Biochemical parameters and adiponectin levels were also evaluated. Receiver operating characteristic (ROC) analyses were used to determine the predictive efficacy of adiponectin for AF, and multivariate logistic regression analysis was performed to evaluate the potential independent predictors of AF. Results. Overall, 84 patients with AF and 84 people with NSR were included. Serum adiponectin was significantly higher in AF patients compared to that in controls (P6.098ā€‰Ī¼g/mL) had predictive efficacy for AF, with an area under the curve of 0.660 (95% confidence interval [CI]: 577ā€“0.742). The results of multivariate logistic regression analysis showed that higher adiponectin was an independent predictor of AF in the overall participants (odds ratio [OR] 1.224, 95% CI 1.018ā€“1.471, P=0.032). Subgroup analysis showed that higher adiponectin was independently associated with AF in women (OR 1.893, 95% CI 1.160ā€“3.089, P=0.011) and in patients agedā€‰<ā€‰65 years (OR 1.453, 95% CI 1.023ā€“2.064, P=0.037), but not in men or those agedā€‰ā‰„ā€‰65 years. Conclusions. Higher serum adiponectin level was independently associated with higher odds for AF in women and in participants <65 years old, but not in men or those aged ā‰„65 years

    HDAC2 overexpression correlates with aggressive clinicopathological features and DNA-damage response pathway of breast cancer

    There are 18 lysine deacetylases, also known as histone deacetylases (HDACs), that remove acetyl groups from histone and non-histone proteins, thereby playing critical roles in numerous biological processes. In many human cancers, HDACs are dysregulated through mutation, altered expression, or inappropriate recruitment to certain loci. However, knowledge of the genomic and transcriptomic alterations and the clinical significance of most HDACs in breast cancer remain incomplete. We used TCGA and METABRIC datasets to perform comprehensive, integrated genomic and transcriptomic analyses of 18 HDAC genes in approximately 3000 primary breast cancers and identified associations among recurrent copy number alteration, gene expression, clinicopathological features, and patient survival.
