    A novel counterbalanced implementation study design : methodological description and application to implementation research

    Background: Implementation research is increasingly being recognised for optimising the outcomes of clinical practice. Frequently, the benefits of new evidence are not implemented due to the difficulties applying traditional research methodologies to implementation settings. Randomised controlled trials are not always practical for the implementation phase of knowledge transfer, as differences between individual and organisational readiness for change combined with small sample sizes can lead to imbalances in factors that impede or facilitate change between intervention and control groups. Within-cluster repeated measure designs could control for variance between intervention and control groups by allowing the same clusters to receive a sequence of conditions. Although in implementation settings, they can contaminate the intervention and control groups after the initial exposure to interventions. We propose the novel application of counterbalanced design to implementation research where repeated measures are employed through crossover, but contamination is averted by counterbalancing different health contexts in which to test the implementation strategy. Methods: In a counterbalanced implementation study, the implementation strategy (independent variable) has two or more levels evaluated across an equivalent number of health contexts (e.g. community-acquired pneumonia and nutrition for critically ill patients) using the same outcome (dependent variable). This design limits each cluster to one distinct strategy related to one specific context, and therefore does not overburden any cluster to more than one focussed implementation strategy for a particular outcome, and provides a ready-made control comparison, holding fixed. The different levels of the independent variable can be delivered concurrently because each level uses a different health context within each cluster to avoid the effect of treatment contamination from exposure to the intervention or control condition. Results: An example application of the counterbalanced implementation design is presented in a hypothetical study to demonstrate the comparison of 'video-based' and 'written-based' evidence summary research implementation strategies for changing clinical practice in community-acquired pneumonia and nutrition in critically ill patient health contexts.     Defining early steps in mRNA transport: mutant mRNA in myotonic dystrophy type I is blocked at entry into SC-35 domains

    In myotonic dystrophy type 1 (DM1), triplet repeat expansion in the 3′ untranslated region of dystrophia myotonica protein kinase (DMPK) causes the nuclear retention of mutant messenger RNA (mRNA). Although the DMPK gene locus positions precisely at the outer edge of a factor-rich SC-35 domain, the normal mRNA consistently accumulates within the domain, and this RNA is depleted upon transcriptional inhibition. In DM1, mutant transcripts detach from the gene but accumulate in granules that abut but do not enter SC-35 domains, suggesting that RNA entry into the domain is blocked. Despite their exclusion from these compartments, mutant transcripts are spliced. MBNL1 (muscleblind-like protein 1) is an alternative splicing factor that becomes highly concentrated with mutant RNA foci. Small interfering RNA–mediated knockdown of MBNL1 promotes the accumulation or entry of newly synthesized mutant transcripts in the SC-35 domain. Collectively, these data suggest that an initial step in the intranuclear path of some mRNAs is passage from the gene into an SC-35 domain and implicate these structures in postsplicing steps before export

    A Multi-Proxy Approach to Archaeobotanical Research: Archaic and Fremont Diets, Utah

    New analytical techniques in archaeobotany allow researchers to examine human plant use by developing interrelated, yet independent lines of evidence. Here we outline the results of a two-method archaeobotanical approach to investigate Archaic and Fremont Great Basin diets. We conducted both macro- and microbotanical (starch granule) analyses at nine archaeological sites located in central and southwestern Utah. Our results show that in contexts where macrobotanical remains are poorly preserved, the application of microbotanical methods can produce additional sets of information, thus improving interpretations about past human diets. In this study, macrobotanical remains represented seed-based dietary contributions, while microbotanical remains came primarily from geophytes. Results suggest largely overlapping diets for Archaic and Fremont residents of Utah

    Planting Prosperity and Harvesting Health: Trade-offs and Sustainability in the Oregon-Washington Regional Food System

    This assessment reveals food system sustainability trends in Oregon and Washington, focusing specifically on the producers in both states and the consumers in the Portland- Vancouver region. We began the assessment by asking a group of food system stakeholders from Oregon and Washington to define broadly supported goals for a sustainable food system. They also helped us identify the data necessary to understand trends in the food system. This information can be used in the future to establish benchmarks and to assess future progress toward food system sustainability goals. Framed by stakeholder concerns, this report will assist program and policy decision makers in prioritizing efforts to shape and strengthen the regional food system. This information is also a foundation for building new and unique partnerships among organizations in food system planning
