    Economic effects analysis of public investment in road improvement works in Hokkaido. Simulation analysis based on a macro-econometric model of Hokkaido

    The objective of this study is to clarify how public investment in road improvement projects over a given analytical period of time has affected Hokkaido`s economic structure on the whole in relation to the industrial economy, prefectural income, household consumption, and commodity prices, through a simulation analysis based on a macro-econometric model. More specifically, our goal is to model both the direct effects achieved through the use of improved roads including the reduction of time-distance coefficients, the reduction of transportation costs and market expansion, and the indirect effects such as enhancement of lifestyles and convenience and influence on other public projects including living area improvement and promotion of regional areas, and to identify these effects quantitatively. Taking data availability into consideration, this study covers a 21-year analysis period covering the years 1976 through 1996. In constructing a quantitative model, the effect flow to be modeled was examined from two perspectives: 1) an effect flow showing the effects of road improvement works on production efficiency and market efficiency; and 2) an effect flow showing the effects of road improvement works on living standards considering convenience and lifestyle improvement. Then we attempted building a model that could indicate the occurrence of these effects in both Flow and Stock contexts. As a result of the simulation analysis, it was clarified that application of road improvement works would bring about pronounced positive economic benefits in tertiary industries, particularly in the transportation-service and wholesale/retail sectors, and greatly expand the prefectural net product on the whole. It was also revealed that these expansion effects would stimulate an increase in the prefectural income and in private final consumption expenditure. Furthermore, a simulation analysis on the economic effects that the expansion of the express-highway network would have on Hokkaido`s entire economy revealed that there would be a large effect particularly on investment and production within the transportation/communication industry and also on the commercial output of the wholesale/retail industry.

    Higher solubility and lower onset temperature of protein denaturation increase the osteoconductive capacity of collagen membranes: A preclinical in vivo study.

    OBJECTIVES Collagen membranes are extensively used for guided bone regeneration procedures, primarily for horizontal bone augmentation. More recently, it has been demonstrated that collagen membranes promote bone regeneration. Present study aimed at assessing if structural modifications of collagen membranes may enhance their osteoconductive capacity. METHODS Twenty-four adult Wistar rats were used. Bilateral calvaria defects with a diameter of 5 mm were prepared and covered with prototypes of collagen membranes (P1 or P2). The P1 membrane (positive control) presented a lower onset temperature of protein denaturation and a higher solubility than the P2 membrane (test). The contralateral defects were left uncovered (NC). After 1 and 4 weeks, the animals were euthanized. A microcomputed tomography analysis of the harvested samples was performed within and above the bony defect. Undecalcified ground sections were subjected to light microscopy and morphometric analysis. RESULTS Bone formation was observed starting from the circumferential borders of the defects in all groups at 1-week of healing. The foci of ossification were observed at the periosteal and dura mater sites, with signs of collagen membrane mineralization. However, there was no statistically significant difference between the groups. At 4 weeks, remnants of the collagen fibers were embedded in the newly formed bone. In the P2 group, significantly more bone volume, more new bone, and marrow spaces compared to the NC group were observed. Furthermore, the P2 group showed more bone volume ectocranially then the P1 group. CONCLUSIONS Bone formation subjacent to a P2 membrane was superior than subjacent to the P1 membrane and significantly better compared to the control. Modifications of the physico-chemical properties may enhance the osteoconductive competence of collagen membranes, supporting bone formation outside the bony defects

    Long-lasting adverse effects of short-term stress during the suckling–mastication transition period on masticatory function and intraoral sensation in rats

The present study assessed the hypothesis that short-term early-life stress during infancy before weaning, a period for the maturation of mastication and sleep, poses long-lasting adverse effects on masticatory function and intraoral sensations later in life. Rat pups were exposed to either maternal separation (MS) or intermittent hypoxia (IH-Infancy) for 6 h/day in the light/sleep phase from postnatal day (P)17 to P20 to generate “neglect” and “pediatric obstructive sleep apnea” models, respectively. The remaining rats were exposed to IH during P45–P48 (IH-Adult). Masticatory ability was evaluated based on the rats’ ability to chew pellets and bite pasta throughout the growth period (P21–P70). Intraoral chemical and mechanical sensitivities were assessed using two-bottle preference drinking tests, and hind paw pain thresholds were measured in adulthood (after P60). No differences were found in body weight, grip force, and hind paw sensitivity in MS, IH-Infancy, and IH-Adult rats compared with naïve rats. Masticatory ability was lower in MS and IH-Infancy rats from P28 to P70 than in naïve rats. MS and IH-Infancy rats exhibited intraoral hypersensitivity to capsaicin and mechanical stimulations in adulthood. The IH-Adult rats did not display inferior masticatory ability or intraoral hypersensitivity. In conclusion, short-term early-life stress during the suckling–mastication transition period potentially causes a persistent decrease in masticatory ability and intraoral hypersensitivity in adulthood. The period is a “critical window” for the maturation of oral motor and sensory functions

    Addition of Synthetic Biomaterials to Deproteinized Bovine Bone Mineral (DBBM) for Bone Augmentation-A Preclinical In Vivo Study.

    (1) Aim: To investigate the effect of synthetic bone substitutes, α-tricalcium phosphate (α-TCP) or bi-layered biphasic calcium-phosphate (BBCP) combined with deproteinized bovine bone mineral (DBBM), on bone formation. (2) Methods: Thirty critical size defects were randomly treated with the following five different treatment modalities: (1) negative control (NC, empty), (2) DBBM, (3) α-TCP + DBBM (1:1), (4) BBCP 3%HA/97%α-TCP + DBBM (1:1), and (5) BBCP 6%HA/94%α-TCP + DBBM (1:1). The samples, at four weeks post-surgery, were investigated by micro-CT and histological analysis. (3) Results: A similar level of new bone formation was demonstrated in the DBBM with α-TCP bone substitute groups when compared to the negative control by histomorphometry. DBBM alone showed significantly lower new bone area than the negative control (p = 0.0252). In contrast to DBBM, the micro-CT analysis revealed resorption of the α-TCP + DBBM, BBCP 3%HA/97%α-TCP + DBBM and BBCP 6%HA/94%α-TCP + DBBM, as evidenced by a decrease of material density (p = 0.0083, p = 0.0050 and p = 0.0191, respectively), without changing their volume. (4) Conclusions: New bone formation was evident in all defects augmented with biomaterials, proving the osteoconductive properties of the tested material combinations. There was little impact of the HA coating degree on α-TCP in bone augmentation potential and material resorption for four weeks when mixed with DBBM

    In Vitro Comparison of Macrophage Polarization and Osteoblast Differentiation Potentials between Granules and Block Forms of Deproteinized Bovine Bone Mineral.

    Deproteinized bovine bone mineral (DBBM) bone grafts are commonly utilized for guided bone regeneration (GBR) techniques in regenerative dentistry. It has been hypothesized that different forms (blocks versus particulates) might demonstrate the varying properties of cell behavior during the regenerative process. Therefore, the aim of the present study was to investigate DBBM granules and blocks for their effects on osteoblasts and macrophages (Mφs). DBBM granules and blocks were filled to the same size (φ6.4 mm in diameter × 2.0 mm in height) in cell culture wells and assessed for cell viability and cell differentiation of human osteoblast-like Saos-2 cells, and Mφs derived from human monocyte THP-1 cells. The two groups were first characterized by micro-CT analysis, which demonstrated that DBBM granules had a two-fold greater material volume and a four-fold larger surface area than the blocks. DBBM blocks showed superior viability for both osteoblasts and Mφs. Osteoblast experiments were then utilized to better characterize the influence of DBBM shapes/forms on osteoblast differentiation. Alkaline phosphatase (ALP) staining on the undecalcified frozen sections was observed throughout the DBBM granule surface, yet this staining was only observed on the upper portion of the DBBM blocks. Furthermore, DBBM blocks showed M1-Mφ polarization trends with higher IL-1 and IL-6 mRNA expression in Mφs, while the conditioned media from Mφs cultured on DBBM granules promoted osteoblast differentiation with higher mRNA levels of Runx 2, ALP and osteocalcin. In conclusion, the DBBM granules showed more regenerative effects, lower M1 marker expression, and higher osteoblast differentiation potential when compared with the blocks, which might be related to the larger material volume, higher surface area and greater ability for the cells to penetrate through the scaffold

    Background : Various musculoskeletal screening and functional performance tests are used to evaluate physical condition. However, validated analysis tools that can identify gaps in pain knowledge during athletes’ daily training are lacking. This study aimed to investigate the relationship between pain intensity in athletes during their daily training and the KOJI AWARENESSℱ test in order to determine whether body dysfunction is related to pain among athletes. Methods : This cross-sectional study was conducted in a fitness center at the authors’ affiliated institution. Thirty-five athletes (17 women and 18 men) aged 20-40 years were selected for study participation. KOJI AWARENESSℱ self-evaluated test scores and pain intensity during daily training, as assessed on the numerical rating scale (NRS), were recorded. Results : The KOJI AWARENESSℱ score showed a strong negative correlation with the NRS score for pain intensity during daily training (r = −0.640, P < 0.001). There was a significant negative correlation between KOJI AWARENESSℱ and NRS scores, even when body mass index, sex, and age were entered as control variables. Conclusions : KOJI AWARENESSℱ was highly accurate in detecting pain in athletes during their training

    Background : The KOJI AWARENESSℱ screening test is a self-administered screening tool for assessment of mobility, stability, and strength. It provides corrective exercises corresponding to the individual’s answers to the KOJI AWARENESSℱ test questions. However, there is no evidence of the effectiveness of corrective exercises in improving KOJI AWARENESSℱ screening test scores and activity-related pain. Methods : Twenty- six healthy subjects (11 female and 15 male ; age 20–50 years) were selected for participation. In a controlled laboratory setting, subjects were administered the KOJI AWARENESSℱ test and were provided with individualized exercise programs based on the results of this first test. KOJI AWARENESSℱ results were checked on the first day immediately after the first exercise session, and then again after the two-week program. Pain intensity was also assessed during daily training using a numerical rating scale. Results : Compared to pre-intervention, KOJI AWARENESSℱ scores were significantly higher immediately after the first corrective exercise session and after 2 weeks of intervention (p < 0.001). Moreover, pain intensity was significantly lower after 2 weeks of intervention (p < 0.001). Conclusions : Individualized KOJI AWARENESSℱ corrective exercises were effective immediately on the first day and also after the two-week program in improving the KOJI AWARENESSℱ score and reducing pain intensity during daily training

    HbA1c and telemedicine during COVID-19

    Aims/Introduction: To investigate whether the COVID-19 pandemic affected behavioral changes and glycemic control in patients with diabetes and to conduct a survey of telemedicine during the pandemic. Materials and Methods: In this retrospective study, a total of 2,348 patients were included from 15 medical facilities. Patients were surveyed about their lifestyle changes and attitudes toward telemedicine. Hemoglobin A1c (HbA1c) levels were compared among before (from June 1 to August 31, 2019) and in the first (from June 1 to August 31, 2020) and in the second (from June 1 to August 31, 2021) year of the pandemic. A survey of physician attitudes toward telemedicine was also conducted. Results: The HbA1c levels were comparable between 2019 (7.27 ± 0.97%), 2020 (7.28 ± 0.92%), and 2021 (7.25 ± 0.94%) without statistical difference between each of those 3 years. Prescriptions for diabetes medications increased during the period. The frequency of eating out was drastically reduced (51.7% in 2019; 30.1% in 2020), and physical activity decreased during the pandemic (48.1% in 2019; 41.4% in 2020; 43.3% in 2021). Both patients and physicians cited increased convenience and reduced risk of infection as their expectations for telemedicine, while the lack of physician–patient interaction and the impossibility of consultation and examination were cited as sources of concern. Conclusions: Our data suggest that glycemic control did not deteriorate during the COVID-19 pandemic with appropriate intensification of diabetes treatment in patients with diabetes who continued to attend specialized diabetes care facilities, and that patients and physicians shared the same expectations and concerns about telemedicine

In contrast, fast ignition creates the hot spot through external heating. This difference allows the fast ignition approach to use a solid sphere as the fusion fuel shape. The implosion of a solid sphere is one form of laser-direct-drive slow implosion. Solid sphere fuel exhibits tolerance to hydrodynamic instability and can be mass-produced relatively easily, offering significant advantages for developing inertial fusion energy. Achieving high fuel peak and areal densities of with a solid sphere requires quasi-isentropic compression, which involves multiple shock waves. Our results show the critical role of uniform laser irradiation in initiating weak shock waves in the early phase, which is essential for forming a uniform and dense fuel core with solid spheres. Furthermore, dynamically adjusting the laser spot diameter could be crucial in optimizing the effectiveness of laser-direct-drive and fast ignition techniques when using solid sphere fuel
