    Stroke and the Younger Adult: Recommendations for Occupational Therapy

    Occupational therapy (OT) is an integral component of the rehabilitation process for individuals who have sustained stroke. As the number of individuals sustaining and surviving stroke in the United States has consistently risen over the past decades (Bartels, 2011), it is anticipated that stroke prevalence from 2010 will have a 25% increase by the year 2030 (American Heart Association (AHA), 2011). In addition, a current trend in research has been found, indicating an increase in the incidence of stroke within the young and middle-aged, or younger, adult population between the ages of 18 and 55 years (AHA, 2011). An extensive review of the literature was conducted with a focus on statistics, risk factors, and the impact of stroke to examine the unique effect of stroke on younger adults. Although stroke survivors at every age experience limitations in a variety of areas of occupation, the literature revealed that younger stroke survivors experience stroke differently than older survivors do (Stone, 2005b; Wolf, Baum, & Connor, 2009). As research supports the rising rates of stroke in the younger adult population and the unique impact of stroke on younger survivors, it is evident that occupational therapists will be providing more services to younger stroke survivors and must be adequately prepared to do so. Therefore, the purpose of this scholarly project is to increase occupational therapists’ awareness of this trend and provide evidence-based recommendations that are unique to younger stroke survivors through the submission of the article, Stroke and the Younger Adult – Recommendations for Occupational Therapy. The development of the content and structure of Stroke and the Younger Adult – Recommendations for Occupational Therapy was based off of the significant findings from the literature and guided by the Canadian Model of Occupational Performance and Engagement (CMOP-E). Stroke and the Younger Adult – Recommendations for Occupational Therapy includes a brief literature review with supporting evidence and recommendations unique to younger survivors for occupational therapists to use as a guide throughout the acute, inpatient, and outpatient phases of rehabilitation. In addition, the article concludes with a case study to highlight the differing areas of occupation that are impacted by stroke in younger survivors. Through the submission of Stroke and the Younger Adult – Recommendations for Occupational Therapy, it is hoped that awareness of this upcoming trend will increase and occupational therapists will be enabled to maintain a client-centered approach to therapy when working to meet the unique needs of the younger stroke survivor population

    Wellness and Professional Quality of Life in Counselor-in-Training Interns: Assessment of Wellness and Non-Wellness-Infused Supervision

    Introduction: Counselors-in-training (CITs) commonly encounter issues of burnout, compassion fatigue, and/or vicarious traumatization due to the nature of their jobs in the helping profession. Wellness infused supervision may help CITs foster personal wellness and mitigate deleterious effects of helping. This investigation examined connections related to counselor-in-training wellness and professional quality of life during an internship-level supervision course across a wellness and control section. Methods: A quasi-experimental design was piloted, comparing a wellness-focused supervision internship section with a non-wellness control group supervision internship section during one academic semester. Participants included 15 clinical mental health CITs (9 experimental; 6 control), who were randomly assigned into the wellness or control internship section. Internship classes consisted of two-hour meetings across a 16 week semester, with participants working towards 300 clock hours of experience. All participants who were offered inclusion into internship accepted, and were randomly assigned into the wellness-infused or control internship sections, which took place within a large, Council for Accreditation in Counseling and Related Educational Program (CACREP) accredited program. Results: Results indicated decreased wellness scores in both internship sections from pre-to-post assessment, no differences between wellness-based internship and the control group in wellness or professional quality of life, and an increase in compassion satisfaction in the wellness-based internship group. Conclusion: Although counselors are vulnerable to compromised wellness due to the nature of their work, training CITs to work from a wellness paradigm in their personal and professional lives may facilitate well-being, and mitigate the effects of burnout and fatigue. Results from this study shed light on how CITs are viewing their personal wellness and how supervisors can utilize assessments to facilitate reflective conversations with supervisees about their wellness and quality of life

    SDSS J094604.90+183541.8: A Gravitationally Lensed Quasar at z=4.8

    We report the discovery of a gravitationally lensed quasar identified serendipitously in the Sloan Digital Sky Survey (SDSS). The object, SDSS J094604.90+183541.8, was initially targeted for spectroscopy as a luminous red galaxy, but the SDSS spectrum has the features of both a z=0.388 galaxy and a z=4.8 quasar. We have obtained additional imaging that resolves the system into two quasar images separated by 3.06 arcsec and a bright galaxy that is strongly blended with one of the quasar images. We confirm spectroscopically that the two quasar images represent a single lensed source at z=4.8 with a total magnification of 3.2, and we derive a model for the lensing galaxy. This is the highest redshift lensed quasar currently known.     Using reporters of different misfolded proteins reveals differential strategies in processing protein aggregates

    The accumulation of misfolded proteins is a hallmark of aging and many neurodegenerative diseases, making it important to understand how the cellular machinery recognizes and processes such proteins. A key question in this respect is whether misfolded proteins are handled in a similar way regardless of their genetic origin. To approach this question, we compared how three different misfolded proteins, guk1-7, gus1-3, and pro3-1, are handled by the cell. We show that all three are nontoxic, even though highly overexpressed, highlighting their usefulness in analyzing the cellular response to misfolding in the absence of severe stress. We found significant differences between the aggregation and disaggregation behavior of the misfolded proteins. Specifically, gus1-3 formed some aggregates that did not efficiently recruit the protein disaggregase Hsp104 and did not colocalize with the other misfolded reporter proteins. Strikingly, while all three misfolded proteins generally coaggregated and colocalized to specific sites in the cell, disaggregation was notably different; the rate of aggregate clearance of pro3-1 was faster than that of the other misfolded proteins, and its clearance rate was not hindered when pro3-1 colocalized with a slowly resolved misfolded protein. Finally, we observed using super-resolution light microscopy as well as immunogold labeling EM in which both showed an even distribution of the different misfolded proteins within an inclusion, suggesting that misfolding characteristics and remodeling, rather than spatial compartmentalization, allows for differential clearance of these misfolding reporters residing in the same inclusion.     "Scaling up" effective health services is high on the policy agendas of many countries and international agencies. The current concern has been driven by growing recognition both of the challenges of achieving the health-related Millennium Development Goals (MDGs) in many countries, and of the need to ensure that the increased resources for health channelled through disease-specific health initiatives are able generate health gain at scale. Effective and cost-effective interventions exist to address many of the major causes of disease burden in the developing world, but coverage of many of these services remains low. There is a substantial gap between what could be achieved and what is actually being achieved in terms of health improvement in low- and middle-income countries

    The Sloan Digital Sky Survey Reverberation Mapping Project: Technical Overview

    The Sloan Digital Sky Survey Reverberation Mapping project (SDSS-RM) is a dedicated multi-object RM experiment that has spectroscopically monitored a sample of 849 broad-line quasars in a single 7 deg2^2 field with the SDSS-III BOSS spectrograph. The RM quasar sample is flux-limited to i_psf=21.7 mag, and covers a redshift range of 0.1<z<4.5. Optical spectroscopy was performed during 2014 Jan-Jul dark/grey time, with an average cadence of ~4 days, totaling more than 30 epochs. Supporting photometric monitoring in the g and i bands was conducted at multiple facilities including the CFHT and the Steward Observatory Bok telescopes in 2014, with a cadence of ~2 days and covering all lunar phases. The RM field (RA, DEC=14:14:49.00, +53:05:00.0) lies within the CFHT-LS W3 field, and coincides with the Pan-STARRS 1 (PS1) Medium Deep Field MD07, with three prior years of multi-band PS1 light curves. The SDSS-RM 6-month baseline program aims to detect time lags between the quasar continuum and broad line region (BLR) variability on timescales of up to several months (in the observed frame) for ~10% of the sample, and to anchor the time baseline for continued monitoring in the future to detect lags on longer timescales and at higher redshift. SDSS-RM is the first major program to systematically explore the potential of RM for broad-line quasars at z>0.3, and will investigate the prospects of RM with all major broad lines covered in optical spectroscopy. SDSS-RM will provide guidance on future multi-object RM campaigns on larger scales, and is aiming to deliver more than tens of BLR lag detections for a homogeneous sample of quasars. The bacterial FASII pathway differs significantly from human FAS pathways and is a long-standing target for antibiotic development against Gram-negative bacteria due to differences from the human FAS, and several existing antibacterial agents are known to inhibit FASII enzymes. N-acetylcysteamine (NAC) fatty acid thioesters have been used as mimics of the natural acyl carrier protein (ACP) pathway intermediates to assay FASII enzymes, and we now report an assay of FabV from Pseudomonas aeruginosa using (E)-2-decenoyl-NAC. In addition, we have converted an existing UV absorbance assay for FabA, the bifunctional dehydration/epimerization enzyme and key target in the FAS II pathway, into a high throughput enzyme coupled fluorescence assay that has been employed to screen a library of diverse small molecules. With this approach, N-(4-chlorobenzyl)-3-(2-furyl)-1H-1,2,4-triazol-5-amine (N42FTA) was found to competitively inhibit (pIC50 = 5.7 ± 0.2) the processing of 3-hydroxydecanoyl-NAC by P. aeruginosa FabA. N42FTA was shown to be potent in blocking crosslinking of E. coli ACP and FabA, a direct mimic of the biological process. The co-complex structure of N42FTA with P. aeruginosa FabA protein rationalizes affinity and suggests future design opportunities. Employing NAC fatty acid mimics to developing further high throughput assays for individual enzymes in the FASII pathway should aid in the discovery of new antimicrobials.Publisher PDFPeer reviewe
