    Perioperative dynamic alterations in peripheral regulatory T and B cells in patients with hepatocellular carcinoma

    <p>Abstract</p> <p>Background</p> <p>Intratumoral and circulating regulatory T cells (Tregs) have been shown to be critical in the pathogenesis of hepatocellular carcinoma (HCC). However there is limited knowledge on the alterations of regulatory B cells (Bregs). We here investigated perioperative dynamic alterations of peripheral circulating Tregs and Bregs in HCC patients to reveal the relationship between regulatory lymphocytes and its clinical implications.</p> <p>Methods</p> <p>36 patients with HCC, 6 with chronic hepatitis B infection and 10 healthy donors were enrolled for this study. Frequencies of peripheral Tregs and Bregs were measured by flow cytometry with antibodies against CD4, CD25, CD127, CD19 and IL-10 before, and after radical surgery. Then, clinical informatics of HCC patients was achieved through Digital Evaluation Score System (DESS) for the assessment of disease severity. Finally, we analysed correlations between digitalized clinical features and kinetics of circulating regulatory lymphocytes.</p> <p>Results</p> <p>Level of circulating CD4<sup>+</sup>CD25<sup>+</sup>CD127<sup>- </sup>Tregs in HCC patients was significantly lower than that in healthy donors and patients with chronic hepatitis B infection before surgery, but was increased after surgery. Preoperative level of CD19<sup>+ </sup>IL-10<sup>+ </sup>Bregs in HCC patients was also significantly lower than the other groups. However it dramatically was elevated right after surgery and remained elevated compared to controls (about 7 days after surgery, <it>P </it>= 0.04). Frequency of circulating Tregs was correlated with circulating leukocytes, ferritin, and clinical features suggesting tumor aggressiveness including portal vein thrombosis, hepatic vein involvement and advanced clinical stages. Frequency of circulating Bregs was associated with Hepatitis B e Antigen (HBeAg) and Hepatitis B virus (HBV) DNA copy number. In addition, DESS was significantly and positively correlated with other staging systems.</p> <p>Conclusion</p> <p>Frequencies of peripheral Tregs and Bregs in HCC patients increased after surgery. These results suggest that a postoperative combination of therapies against Tregs and Bregs may be beneficial for better outcome of HCC patients after resection.</p

    Robust estimation of bacterial cell count from optical density

    Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals &lt;1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data

    Spontaneous hepatic haemorrhage after caesarean section in a patient with uraemia and superimposed preeclampsia: a case report

    Perinatal spontaneous hepatic haemorrhage is a very rare disease affecting pregnant women, particularly those on long-term dialysis, that has a high maternal and infant mortality rate. Most patients experience preeclampsia with haemolysis, elevated liver enzymes and low platelets syndrome. Here, the case of a 35-year-old multigravida patient with known chronic kidney disease and chronic hypertension with uraemia, who developed spontaneous hepatic haemorrhage after caesarean section, is described. The patient experienced sudden massive circulatory failure, but hemodynamics were temporarily stabilized after emergency surgery. Following transfer to the intensive care unit for continued treatment, her blood pressure and haemoglobin level continued to drop. Selective hepatic artery embolization was performed on day 2 after delivery, and her vital signs gradually stabilized. On day 30 after delivery, the patient was discharged in a stable condition. The newborn recovered after therapy in neonatal intensive care for 2 months. The present case suggests that, for perinatal spontaneous hepatic haemorrhage, timely and accurate diagnosis, multidisciplinary management and determining the therapeutic approach according to clinical symptoms are essential

    Dynamic Inversion of Inland Aquaculture Water Quality Based on UAVs-WSN Spectral Analysis

    The inland aquaculture environment is an artificial ecosystem, where the water quality is a key factor which is closely related to the economic benefits of inland aquaculture and the quality of aquatic products. Compared with marine aquaculture, inland aquaculture is normally smaller and susceptible to pollution, with poor self-purification capacity. Considering its low cost and large-scale monitoring ability, many researches have developed spectrum sensor on-board satellite platforms to allow remote monitoring of inland water surface. However, there remain many problems, such as low image resolution, poor flexible data acquisition, and anti-interference. Apart from that, the conventional forecasting model is of weak generalization ability and low accuracy. In our study, we combine unmanned aerial vehicles system (UAVs) with the wireless sensor network (WSN) to design a new ground water quality parameter and drone spectrum information acquisition approach, and to propose a novel dynamic network surgery-deep neural networks (DNS-DNNs) model based on multi-source feature fusion to forecast the distribution of dissolved oxygen (DO) and turbidity (TUB) in inland aquaculture areas. The result of using fused features, including characteristic spectrum, Gray-level co-occurrence matrix (GLCM) texture feature, and convolutional neural network (CNN) texture feature to build a model is that the characteristic spectrum+ CNN texture fusion features were the best input items for DNS-DNNs when forecasting DO, with the determination coefficient R 2 of the vertical set arriving at 0.8741, while the characteristic spectrum+ GLCM texture+ CNN texture fusion features were the best for TUB, with the R 2 reaching 0.8531. Compared with a variety of conventional models, our model had a better performance in the inversion of DO and TUB, and there was a strong correlation between predicted and real values: R 2 reached 0.8042 and 0.8346, whereas the root mean square error (RMSE) were only 0.1907 and 0.1794, separately. Our study provides a new insight about using remote sensing to rapidly monitor water quality in inland aquaculture regions

    Comparison of Remimazolam-Flumazenil versus Propofol for Rigid Bronchoscopy: A Prospective Randomized Controlled Trial

    Background: Remimazolam is a novel ultrashort-acting intravenous benzodiazepine sedative–hypnotic that significantly reduces the times to sedation onset and recovery. This trial was conducted to confirm the recovery time from anesthesia of remimazolam-flumazenil versus propofol in patients undergoing endotracheal surgery under rigid bronchoscopy. Methods: Patients undergoing endotracheal tumor resection or stent implantation were randomly allocated into a remimazolam group (Group R) or a propofol group (Group P). The primary outcome was the recovery time from general anesthesia. The secondary outcomes were the time to loss of consciousness (LoC), hemodynamic fluctuations, and adverse events. Results: A total of 34 patients were screened, and 30 patients were enrolled in the study. The recovery time was significantly shorter for Group R (140 ± 52 s) than for Group P (374 ± 195 s) (p < 0.001). The times to LoC were 76 ± 40 s in Group R and 75 ± 25 s in Group P and were not significantly different. There were also no significant differences in hemodynamic fluctuations or adverse events between the two groups. Conclusions: The recovery time from general anesthesia in rigid bronchoscopy patients was shorter using remimazolam-flumazenil than with propofol, with no dramatic hemodynamic fluctuations and adverse events or differences between the agents.     Decoding the metastatic potential and optimal postoperative adjuvant therapy of melanoma based on metastasis score

    Abstract Metastasis is a formidable challenge in the prognosis of melanoma. Accurately predicting the metastatic potential of non-metastatic melanoma (NMM) and determining effective postoperative adjuvant treatments for inhibiting metastasis remain uncertain. In this study, we conducted comprehensive analyses of melanoma metastases using bulk and single-cell RNA sequencing data, enabling the construction of a metastasis score (MET score) through diverse machine-learning algorithms. The reliability and robustness of the MET score were validated using various in vitro assays and in vivo models. Our findings revealed a distinct molecular landscape in metastatic melanoma characterized by the enrichment of metastasis-related pathways, intricate cell–cell communication, and heightened infiltration of pro-angiogenic tumor-associated macrophages compared to NMM. Importantly, patients in the high MET score group exhibited poorer prognoses and an immunosuppressive microenvironment, featuring increased infiltration of regulatory T cells and decreased infiltration of CD8+ T cells, compared to the low MET score patient group. Expression of PD-1 was markedly higher in patients with low MET scores. Anti-PD-1 (aPD-1) therapy profoundly affected antitumor immunity activation and metastasis inhibition in these patients. In summary, our study demonstrates the effectiveness of the MET score in predicting melanoma metastatic potential. For patients with low MET scores, aPD-1 therapy may be a potential treatment strategy to inhibit metastasis. Patients with high MET scores may benefit from combination therapies

    Integrated Transcriptome Analysis of Long Noncoding RNA and mRNA in Developing and Aging Mouse Retina

    Abstract Mice have emerged as a widely employed model for investigating various retinal diseases. However, the availability of comprehensive datasets capturing the entire developmental and aging stages of the mouse retina, particularly during the elderly period, encompassing integrated lncRNA and mRNA expression profiles, is limited. In this study, we assembled a total of 18 retina samples from mice across 6 distinct stages of development and aging (5 days, 3 weeks, 6 weeks, 10 weeks, 6 months, and 15 months) to conduct integrated lncRNA and mRNA sequencing analysis. This invaluable dataset offers a comprehensive transcriptomic resource of mRNA and lncRNA expression profiles during the natural progression of retinal development and aging. The discoveries stemming from this investigation will significantly contribute to the elucidation of the underlying molecular mechanisms associated with various retinal diseases, such as congenital retinal dysplasia and retinal degenerative diseases

    Knowledge and Awareness of Stroke among residents in a Singapore housing estate

    Introduction Inadequate knowledge of warning symptoms and risk factors of stroke is associated with significant pre-hospital and treatment delays. This study aims to assess the awareness of warning symptoms and risk factors of stroke amongst Singaporeans and permanent residents, and to identify factors associated with awareness. Materials and Methods A cross-sectional study was done in a local public housing estate. Through multi-stage random sampling, 1522 household units were selected for a face-to-face, open-ended questionnaire interview, which was conducted by trained interviewers. Information collected included knowledge of stroke warning symptoms and risk factors, together with hypothetical scenarios to assess participants’ responses. Socio-demographic information, participant reports of health-related behaviours, and personal and family history of stroke risk-conferring conditions were obtained.   Results 52.4% of participants were unable to identify any key warning symptoms of stroke, with up to 12.4% of participants citing symptoms more closely associated with an acute myocardial infarction. 60.4% were unable to identify ≥ 2 risk factors for stroke. 4 out of 10 participants were unable to provide the correct number to dial for an emergency ambulance in Singapore. No increased level of awareness was noted amongst participants with stroke risk-conferring medical conditions, nor among the elderly.   Conclusion There was a general lack of awareness of stroke. Participants at a higher risk of stroke were ill-informed about the condition. Broad-based educational efforts targeted at the general population to improve awareness of stroke symptoms are needed. In addition, education should be aimed at individuals with greater stroke risk
