    Infection control and the prevalence, management and outcomes of SARS-CoV-2 infections in mental health wards in London, UK: lessons learned from wave 1 to wave 2.

    BACKGROUND: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) disease (COVID-19) has high morbidity and mortality in older adults and people with dementia. Infection control and prevention measures potentially reduce transmission within hospitals. AIMS: We aimed to replicate our earlier study of London mental health in-patients to examine changes in clinical guidance and practice and associated COVID-19 prevalence and outcomes between COVID-19 waves 1 and 2 (1 March to 30 April 2020 and 14 December 2020 to 15 February 2021). METHOD: We collected the 2 month period prevalence of wave 2 of COVID-19 in older (≥65 years) in-patients and those with dementia, as well as patients' characteristics, management and outcomes, including vaccinations. We compared these results with those of our wave 1 study. RESULTS: Sites reported that routine testing and personal protective equipment were available, and routine patient isolation on admission occurred throughout wave 2. COVID-19 infection occurred in 91/358 (25%; 95% CI 21-30%) v. 131/344, (38%; 95% CI 33-43%) P < 0.001 in wave 1. Hospitals identified more asymptomatic carriers (26/91; 29% v. 16/130; 12%) and fewer deaths (12/91; 13% v. 19/131; 15%; odds ratio = 0.92; 0.37-1.81) compared with wave 1. The patient vaccination uptake rate was 49/58 (85%). CONCLUSIONS: Patients in psychiatric in-patient settings, mostly admitted without known SARS-CoV-2 infection, had a high risk of infection compared with people in the community but lower than that during wave 1. Availability of infection control measures in line with a policy of parity of esteem between mental and physical health appears to have lowered within-hospital COVID-19 infections and deaths. Cautious management of vulnerable patient groups including mental health patients may reduce the future impact of COVID-19

    Influence of right coronary artery motion, flow pulsatility and non-Newtonian rheology on wall shear stress metrics

    The patchy distribution of atherosclerosis within the arterial system is consistent with a controlling influence of hemodynamic wall shear stress (WSS). Patterns of low, oscillatory and transverse WSS have been invoked to explain the distribution of disease in the aorta. Disease of coronary arteries has greater clinical importance but blood flow in these vessels may be complicated by their movement during the cardiac cycle. Previous studies have shown that time average WSS is little affected by the dynamic geometry, and that oscillatory shear is influenced more. Here we additionally investigate effects on transverse WSS. We also investigate the influence of non-Newtonian blood rheologyas it can influence vortical structure, on which transverse WSS depends; Carreau-Yasuda models were used. WSS metrics were derived from numerical simulations of blood flow in a model of a moving right coronary artery which, together with a subject-specific inflow waveform, was obtained by MR imaging of a healthy human subject in a previous study. The results confirmed that time average WSS was little affected by dynamic motion, and that oscillatory WSS was more affected. They additionally showed that transverse WSS and its non-dimensional analogue, the Cross Flow Index, were affected still further. This appeared to reflect time-varying vortical structures caused by the changes in curvature. The influence of non-Newtonian rheology was significant with some physiologically realistic parameter values, and hence may be important in certain subjects. Dynamic geometry and non-Newtonian rheology should be incorporated into models designed to produce maps of transverse WSS in coronary arteries

    Wrist-worn actigraphy in agitated late-stage dementia patients:A feasibility study on digital inclusion

    BACKGROUND: Wrist-worn actigraphy can be an objective tool to assess sleep and other behavioral and psychological symptoms in dementia (BPSD). We investigated the feasibility of using wearable actigraphy in agitated late-stage dementia patients.METHODS: Agitated, late-stage Alzheimer's dementia care home residents in Greater London area (n = 29; 14 females, mean age ± SD: 80.8 ± 8.2; 93.1% White) were recruited to wear an actigraphy watch for 4 weeks. Wearing time was extracted to evaluate compliance, and factors influencing compliance were explored.RESULTS: A high watch-acceptance (96.6%) and compliance rate (88.0%) was noted. Non-compliance was not associated with age or BPSD symptomatology. However, participants with "better" cognitive function (R = 0.42, p = 0.022) and during nightshift (F 1.240, 33.475 = 8.075, p = 0.005) were less compliant. Female participants were also marginally less compliant (F 1, 26 = 3.790, p = 0.062). DISCUSSIONS: Wrist-worn actigraphy appears acceptable and feasible in late-stage agitated dementia patients. Accommodating the needs of both the patients and their carers may further improve compliance.</p
