    Emerging trends of traumatic brain injury in Western Uttar Pradesh, India: diagnosis and rehabilitation

    Background: To study the various advance technology for the diagnosis of Traumatic Brain Injury (TBI) and find out Emerging trends occur in TBI (Traumatic Brain Injury) patients.Methods: The present study was conducted with 500 patients, age between 02 year to 70 years mean age (36 Years) presenting to emergency department of Uttar Pradesh University of Medical Sciences, Saifai, Etawah, with a history of acute head trauma from January 2016 to December 2018. All patients were examined using 64 slices MDCT and 1.5T MRI Scanner also.Results: Traumatic brain injury caused by various reasons like 62.1% road traffic accidents (RTA) and 25.1% fall from height (FFH) being and 11.83% Assault/hit by hard object and 0.88% are gunshot injury. Loss of consciousness was the most common complaint of the 59.1% TBI patients followed by 17.75% Vomiting and headache, 11.83% facial injury and 11.24% scalp injury. All TBI patients were diagnosed by MDCT 64 Slices Somatom Sensation Scanner who was observed 41.42% skull fractures, 29.28% extra dural hematoma, 27.21% sub dural hematoma, 23.96% sub archnoid haemorrhage, 13% intra cerebral hematoma, 30.17% brain contusions and 24.26% diffuse cerebral edema.Conclusions: Road traffic accidents remain the leading cause of trauma in our country. MRI and MDCT are well recognized method to know the extent and various types of hemorrhages and skull fractures in TBI patients. The present study data is indicated 62.1% majority of TBI patients are suffered by Road traffic accidents mainly young males with alcoholism

    Successfully treated synchronous double malignancy of the breast and esophagus: a case report

    <p>Abstract</p> <p>Introduction</p> <p>The incidence of multiple primary cancers is reported to be between 0.3% and 4.3%. The second primary lesion is identified either simultaneously with the primary lesion (synchronous) or after a period of time (metachronous). Few cases of metastasis of breast carcinoma to the esophagus and vice versa have been reported in the past.</p> <p>Case presentation</p> <p>We report an extremely rare case of a 55-year-old Indian woman who had carcinomas in both the esophagus and the breast simultaneously. She was treated successfully using combined modalities of surgery, chemotherapy and radiation therapy.</p> <p>Conclusion</p> <p>Cases of synchronous double malignancies can be treated by dealing with the malignancy in the two sites as independent carcinomas. We have to take into consideration the total dose of radiation to a critical organ as well as the effect of the total dose of toxic chemotherapeutic drugs on our patient.</p

    The estimation of minimum effective volume of 0.5% ropivacaine in ultrasound-guided interscalene brachial plexus nerve block: A clinical trial

    Background and Aims: Interscalene brachial plexus block (ISB) is the most commonly used mode of anesthesia for upper limb surgeries. Higher volume of local anesthetic used in ISB is associated with increased incidence of side effects, particularly phrenic nerve palsy. The aim of this study was to determine the minimum effective volume of 0.5% ropivacaine in 90% patients (MEV90) in ISB. Material and Methods: With target of 45 successful cases, phase 1 clinical trial was conducted based on the principles of biased coin design up-and-down method. After obtaining Ethical Committee's approval and patient's consent, patients with American Society of Anesthesiologist physical status (ASA PS) I and II, aged 18–60 years of either sex, undergoing upper arm surgery were recruited into the study until 45 successful cases. A 7 ml of 0.5% ropivacaine was used as starting dose, with patients receiving a higher or lower dose depending on previous patient's response. R package, SPSS 23, and Microsoft Excel were used for statistical analysis. Results: MEV90 of 0.5% ropivacaine for ISB was determined as 8.64 ml [confidence interval (CI) 95%, 8.28–9.02 ml]. Time for onset of sensory block and motor block was 5 min (5–15 min) and 10 min (5–20 min), respectively, while duration of analgesia was observed as 8.2 (4.8–12.5) h. Conclusions: This study observes that surgical anesthesia can be accomplished with 8.64 ml (95% CI: 8.28–9.02 ml) of 0.5% ropivacaine with ultrasound-guided ISB with multiple injection technique, without clinical deterioration in block onset and duration of analgesia

    Introduction: Repeated use of social network is said to cause addiction. Attention deficit hyperactivity disorder (ADHD) is disorder found in childhood. Studies have shown that such children continue to have the symptoms of ADHD as adults. The aim of this study was to find the prevalence of social networking addiction and Internet addiction and their relationship with ADHD. Also, to assess the relationship between perceived stress and sociodemographic factors with social networking and internet addiction. Material and Methods: A cross-sectional study was conducted among 150 undergraduate and postgraduate students of BMCRI after ethical clearance. Medical students who gave consent were included through stratified random sampling. Those who were taking treatment for psychiatric illness were excluded. Scales like the Young Internet addiction test (IAT); Bergen's Facebook Reporting Scale, ADHD Self-Reporting Scale, and Wender Utah Brief Rating Scale for ADHD, and Cohen Perceived Stress Scale (PSS) for perceived stress have been used. Results: The mean age was found to be 21.56 ± 3 years. 62.6% were males and 37.33% of were females. The prevalence of Internet addiction was found to be 23.3% (n = 35), with 2% (n = 3) were having severe addiction, 6.6% (n = 10) were having moderate Internet and 14.6% (n = 22) having mild internet addiction according to modified Young's criteria (2011). The prevalence of social networking addiction as per the Bergen Facebook Addiction Scale (BFAS) score was 4.66% and adult ADHD was 15%. There was a strong positive correlation (0.76) between Internet Addiction and Facebook addiction scores and between BFAS score and IAT score. A moderate positive correlation (0.46 and 0.47) between Internet addiction and Facebook addiction with perceived stress and ADHD and IAT score with PSS score and ASRS score, weak positive correlation with PSS score and ASRS score was observed. Conclusion: A significant proportion of medical students develop Internet addiction and minority develops social network addiction. Furthermore, perceived stress and ADHD scores are positively correlated with internet and social networking addiction

    A novel device to measure tactile reaction time

    Introduction: Reaction Time has been widely used as one of the measures of mental chronometry since very long. Most of the work has been done with visual and auditory reaction time. Till now reaction time has never been measured by giving tactile stimuli. Hence we have designed a new device to provide different tactile stimuli and to simultaneously measure the time taken by the subject to respond to it. Probably for the first time in the history of mental chronometry Tactile Choice Reaction Time has been compared between males and females. Methods and Materials: The study was conducted among 53 male and 34 female healthy subjects who were between the age group of 18- 24 years. A new instrument has been designed by the author of this article to deliver tactile stimuli on the skin of the subject and to measure the reaction time for that particular sensory modality. Result: All the reaction times were found to be significantly faster in males than in females except for fine touch. For fine touch although the RT is faster in males but the difference is not statistically significant. Conclusion: Present study has reconfirmed the gender differences in case of tactile choice reaction time also. It is going to be really interesting with the use of this new device to find out the relationship of various Tactile Choice Reaction Times with these conditions

    Board meeting, promoter CEO and firm performance: Evidence from India

    AbstractThe study examined the relationship between board characteristics and firm performance and the moderating effects of firm size, the board size, and firm age between board characteristics and firm performance. This study considers the legal reforms implemented after the Indian Companies Act 2013. Data from 113 firms with 904 observations from 2012–13 to 2019–20 were analyzed using the fixed panel data estimation approach. A subsample analysis is employed, dividing the data by firm size, the board size, and firm age to test the robustness of the results. The results show that the board size, female director, Promoter CEOs, meeting frequencies, and attendance rate positively affect firm performance. At the same time, the impact of the independent directors and busy CEO has a negative impact on performance. Male CEOs are beneficial for firm performance. The study adds to the literature by identifying critical board characteristics in light of ongoing regulatory reform in emerging economies like India. It has implications for regulators and policymakers who are entrusted with the framing of corporate governance policies
