Profile of Clients Seeking Consultation at Yoga Therapy Department: A Cross Sectional Study
It is estimated that between 10% and 30% of the general practice population are mentally or emotionally disturbed. Physical complaints, multiple aches and pains are construed as signs and symptoms of stress. Although yoga is considered as beneficial, any hidden psychiatric morbidity may go undetected and thus untreated. Thus the aim of this study was to obtain an estimate of hidden psychiatric morbidity in clients seeking consultation at the Department of Yoga. Fifty respondents were administered Life Satisfaction Scale and General Health Questionnaire-12. Majority were females, younger, graduates, married and also scored above the cut off on GHQ, suggesting minor psychiatric disorder/distress; reasons for consultation were related to being over weight, stress, aches and pains, poor concentration. A significant number of persons reported of some dissatisfaction in life. The perceived benefits were related to reducing symptoms, stress, weight, relaxation and improvement in concentration and memory
Hospital Related Stress Among Patients Admitted to a Psychiatric In-patient Unit in India
The psychiatric patient’s attitudes towards hospitalization have found an association between patient perceptions of the ward atmosphere and dissatisfaction. The aim of the study was to determine the aspects of stress related to hospitalization in inpatients admitted to a psychiatric facility. Fifty in-patients of both sexes admitted consecutively to a psychiatric unit in a General Hospital were asked to rate the importance of, and their satisfaction with, 38 different aspects of in-patient care and treatment. Results showed that the major sources of stress were related to having a violent patient near to his/her bed; being away from family; having to stay in closed wards; having to eat cold and tasteless food; losing income or job due to illness, being hospitalized away from home; not able to understand the jargons used by the clinical staff and not getting medication for sleep. A well-differentiated assessment of stress and satisfaction has implications for the evaluation of the quality of psychiatric care and for the improvement of in-patient psychiatric care
Frequency of Parental Psychopathology in Children with Attention Deficit Hyperactivity Disorder
Parental mental health concerns place children at a significantly greater risk of lower social, psychological and physical health than children in families not affected by mental illness. Several studies have examined the extent to which psychopathology in children are closely related to parental substance use disorders or to other mental disorders. This study intended to investigate the frequency of occurrence of parental psychopathology in children with Attention Deficit/Hyperactivity Disorder and compare the characteristics of children with and without parental psychopathology. Fifty children between 7-14 years of either sex fulfilling the ICD-10 criteria for ADHD comprised the sample of this study.It would be a chart review of in-patients and out-patients who have consulted at Kasturba Hospital, Department of Psychiatry, Manipal over a two year period. Parents diagnosed with Substance Use Disorders (SUD) and other psychiatric morbidity according to ICD-10 was compared with those parents without any psychiatric illness on various Sociodemographic, clinical characteristics. Most of the parents were in the second or third decade of their life. Of the fifty patients 41(82%) were males and 9 (18%) were females. In 36(87.0 %) of the fathers had a history of substance use disorder and 5(13.0 %) it was absent. Other morbidities were also present in both parents. The findings are discussed in light of the implications
EVALUATION OF ANTIDEPRESSANT ACTIVITY OF TAPENTADOL IN SWISS ALBINO MICE
Objective: The aim of this study is to evaluate the antidepressant activity of tapentadol using forced swimming test (FST) and tail suspension test (TST) experimental models.Methods: A total of 36 Swiss albino mice (18 for each experimental model) were divided into 3 groups of 6 animals each. In both the experimental models, Group I received normal saline – 10 ml/kg (Control group), Groups II and III given tapentadol 20 mg/kg and tapentadol 40 mg/kg, respectively, for 7 days, intraperitoneally. On day 7, the drugs were given 40 minutes before conducting the experiment. The duration of immobility was noted and compared among all the 3 groups. The observations were analyzed using analysis of variance and Tukey's post-hoc test.Results: The duration of immobility was significantly decreased in both the experimental models. Tapentadol groups when compared to control group showed statistically significant values, and better results were obtained with tapentadol 20 mg/kg groups in both the models. The mean duration of Immobility was 34.67 seconds in FST model and 101.00 seconds in TST model when treated with tapentadol 20 mg/kg compared to 102.33 seconds in FST control and 141 seconds in TST control groups. FST model demonstrates greater antidepressant efficacy of tapentadol (p<0.00) than with TST model (p<0.04).Conclusion: Tapentadol showed significant antidepressant activity at the dose of 20 mg/kg. The results should be further confirmed by animal studies with different experimental models for the evaluation of depression and by human clinical studies, and if found effective, tapentadol can be preferred for patients with chronic pain, such as cancer pain
The nosological significance of Folie à Deux: a review of the literature
BACKGROUND: Folie à Deux is a rare syndrome that has attracted much clinical attention. There is increasing doubt over the essence of the condition and the validity of the original description, such that it remains an elusive entity difficult to define. METHOD: We conducted a systematic review of the literature of all cases reporting the phenomenon of Folie à Deux, from the years 1993–2005. RESULTS: 64 cases were identified of which 42 met the inclusion criteria. The diagnoses in the primary and secondary were more heterogeneous than current diagnostic criteria suggest. There exists a high degree of similarity between the primary and secondary in terms of susceptibility to psychiatric illness, family and past psychiatric history, than previously thought. CONCLUSION: Folie à Deux can occur in many situations outside the confines of the current classification systems and is not as rare as believed, and should alert the clinician to unrecognized psychiatric problems in the secondary
Reversal of TGF-β1 stimulation of α-smooth muscle actin and extracellular matrix components by cyclic AMP in Dupuytren's - derived fibroblasts
<p>Abstract</p> <p>Background</p> <p>Myofibroblasts, a derived subset of fibroblasts especially important in scar formation and wound contraction, have been found at elevated levels in affected Dupuytren's tissues. Transformation of fibroblasts to myofibroblasts is characterized by expression of alpha- smooth muscle actin (α-SMA) and increased production of extracellular matrix (ECM) components, both events of relevance to connective tissue remodeling. We propose that increasing the activation of the cyclic AMP (cAMP)/protein kinase A signaling pathway will inhibit transforming growth factor-beta1 (TGF-β<sub>1</sub>)-induced ECM synthesis and myofibroblast formation and may provide a means to blunt fibrosis.</p> <p>Methods</p> <p>Fibroblasts derived from areas of Dupuytren's contracture cord (DC), from adjacent and phenotypically normal palmar fascia (PF), and from palmar fascia from patients undergoing carpal tunnel release (CTR; CT) were treated with TGF-β<sub>1 </sub>(2 ng/ml) and/or forskolin (10 μM) (a known stimulator of cAMP). Total RNA and protein extracted was subjected to real time RT-PCR and Western blot analysis.</p> <p>Results</p> <p>The basal mRNA expression levels of fibronectin- extra domain A (FN1-EDA), type I (COL1A2) and type III collagen (COL3A1), and connective tissue growth factor (CTGF) were all significantly increased in DC- and in PF-derived cells compared to CT-derived fibroblasts. The TGF-β<sub>1 </sub>stimulation of α-SMA, CTGF, COL1A2 and COL3A1 was greatly inhibited by concomitant treatment with forskolin, especially in DC-derived cells. In contrast, TGF-β<sub>1 </sub>stimulation of FN1-EDA showed similar levels of reduction with the addition of forskolin in all three cell types.</p> <p>Conclusion</p> <p>In sum, increasing cAMP levels show potential to inhibit the formation of myofibroblasts and accumulation of ECM components. Molecular agents that increase cAMP may therefore prove useful in mitigating DC progression or recurrence.</p
Suicidal behaviour among terminally ill cancer patients in India
BACKGROUND: Passive suicidal thoughts are relatively common in patients with terminal cancer. There is a need for more information about the factors that influence these patients to desire death. AIM: To examine the prevalence of suicidal ideation among terminally ill cancer patients. METHODS: Fifty-four terminally ill inpatients (27 men and 27 women) from the palliative care unit of the Oncology department of Kasturba Hospital, Manipal were evaluated on various rating scales for depression, hopelessness and suicidal ideation, and the correlation of suicidal ideation with medical symptoms such as pain, as well as awareness of the diagnosis and understanding of the illness. RESULTS: Most patients (79.7%) denied having suicidal thoughts or wishing for an early death; only 9.2% had severe suicidal ideation. Two patients (3.8%) with severe suicidal ideation had a past history of major depression. Factors such as the presence of pain, awareness of the diagnosis and understanding of the illness contributed to depressive mood states. CONCLUSION: Suicidal ideation and a desire for death appear to be linked exclusively to the presence of a mental disorder. In addition, poor pain control, and awareness of the diagnosis may also contribute to suicidal ideation
