    Environmental risk factors of pregnancy outcomes: A summary of recent meta-analyses of epidemiological studies.

    Background Various epidemiological studies have suggested associations between environmental exposures and pregnancy outcomes. Some studies have tempted to combine information from various epidemiological studies using meta-analysis. We aimed to describe the methodologies used in these recent meta-analyses of environmental exposures and pregnancy outcomes. Furthermore, we aimed to report their main findings. Methods We conducted a bibliographic search with relevant search terms. We obtained and evaluated 16 recent meta-analyses. Results The number of studies included in each reported meta-analysis varied greatly, with the largest number of studies available for environmental tobacco smoke. Only a small number of the studies reported having followed meta-analysis guidelines or having used a quality rating system. Generally they tested for heterogeneity and publication bias. Publication bias did not occur frequently. The meta-analyses found statistically significant negative associations between environmental tobacco smoke and stillbirth, birth weight and any congenital anomalies; PM2.5 and preterm birth; outdoor air pollution and some congenital anomalies; indoor air pollution from solid fuel use and stillbirth and birth weight; polychlorinated biphenyls (PCB) exposure and birth weight; disinfection by-products in water and stillbirth, small for gestational age and some congenital anomalies; occupational exposure to pesticides and solvents and some congenital anomalies; and agent orange and some congenital anomalies. Conclusions The number of meta-analyses of environmental exposures and pregnancy outcomes is small and they vary in methodology. They reported statistically significant associations between environmental exposures such as environmental tobacco smoke, air pollution and chemicals and pregnancy outcomes

    Disordered Microbial Communities in the Upper Respiratory Tract of Cigarette Smokers

    Cigarette smokers have an increased risk of infectious diseases involving the respiratory tract. Some effects of smoking on specific respiratory tract bacteria have been described, but the consequences for global airway microbial community composition have not been determined. Here, we used culture-independent high-density sequencing to analyze the microbiota from the right and left nasopharynx and oropharynx of 29 smoking and 33 nonsmoking healthy asymptomatic adults to assess microbial composition and effects of cigarette smoking. Bacterial communities were profiled using 454 pyrosequencing of 16S sequence tags (803,391 total reads), aligned to 16S rRNA databases, and communities compared using the UniFrac distance metric. A Random Forest machine-learning algorithm was used to predict smoking status and identify taxa that best distinguished between smokers and nonsmokers. Community composition was primarily determined by airway site, with individuals exhibiting minimal side-of-body or temporal variation. Within airway habitats, microbiota from smokers were significantly more diverse than nonsmokers and clustered separately. The distributions of several genera were systematically altered by smoking in both the oro- and nasopharynx, and there was an enrichment of anaerobic lineages associated with periodontal disease in the oropharynx. These results indicate that distinct regions of the human upper respiratory tract contain characteristic microbial communities that exhibit disordered patterns in cigarette smokers, both in individual components and global structure, which may contribute to the prevalence of respiratory tract complications in this population

    Yak are adapted to the extreme cold, low oxygen, and high solar radiation of the Himalaya. Traditionally, they are kept at high altitude pastures during summer, moving lower in the winter. This system is highly susceptible to climate change, which has increased ambient temperatures, altered rainfall patterns and increased the occurrence of natural disasters. Changes in temperature and precipitation reduced the yield and productivity of alpine pastures, principally because the native plant species are being replaced by less useful shrubs and weeds. The impact of climate change on yak is likely to be mediated through heat stress, increased contact with other species, especially domestic cattle, and alterations in feed availability. Yak have a very low temperature humidity index (52 vs. 72 for cattle) and a narrow thermoneutral range (5-13 °C), so climate change has potentially exposed yak to heat stress in summer and winter. Heat stress is likely to affect both reproductive performance and milk production, but we lack the data to quantify such effects. Increased contact with other species, especially domestic cattle, is likely to increase disease risk. This is likely to be exacerbated by other climate-change-associated factors, such as increases in vector-borne disease, because of increases in vector ranges, and overcrowding associated with reduced pasture availability. However, lack of baseline yak disease data means it is difficult to quantify these changes in disease risk and the few papers claiming to have identified such increases do not provide robust evidence of increased diseases. The reduction in feed availability in traditional pastures may be thought to be the most obvious impact of climate change on yak; however, it is clear that such a reduction is not solely due to climate change, with socio-economic factors likely being more important. This review has highlighted the large potential negative impact of climate change on yak, and the lack of data quantifying that impact. More research on the impact of climate change in yak is needed. Attention also needs to be paid to developing mitigating strategies, which may include changes in the traditional system such as providing shelter and supplementary feed and, in marginal areas, increased use of yak-cattle hybrids
