    Comparison of Intravitreal Bevacizumab Upload Followed by a Dexamethasone Implant versus Dexamethasone Implant Monotherapy for Retinal Vein Occlusion with Macular Edema

    Purpose: To compare the efficacy and safety of three intravitreal bevacizumab upload injections followed by a dexamethasone implant versus dexamethasone implant monotherapy in eyes with macular edema due to retinal vein occlusion. Methods: Sixty-four eyes of 64 patients were included in this prospective, consecutive, nonrandomized case series: group 1 consisted of 38 patients (22 with central retinal vein occlusion, CRVO, 16 with branch retinal vein occlusion, BRVO) treated using a dexamethasone implant (Ozurdex) alone; group 2 consisted of 26 patients (14 CRVO, 12 BRVO) treated with three consecutive intravitreal bevacizumab injections at monthly intervals followed by a dexamethasone implant. In case of recurrence, both cohorts received further dexamethasone implants. Preoperatively and monthly best corrected visual acuity (BCVA, ETDRS), central retinal thickness (Spectralis-OCT), intraocular pressure, and wide-angle fundus photodocumentation (Optomap) were performed. The primary clinical endpoint was BCVA at 6 months after initiation of therapy. Secondary endpoints were central retinal thickness and safety of the therapy applied. Results: In group 1, an increase in BCVA of 2.5 (+/- 1.6) letters in the CRVO and of 13.0 (+/- 3.2) letters in BRVO patients was seen after 6 months, in group 2 of 5.9 (+/- 0.4) letters (CRVO) and 3.8 (+/- 2.4) letters (BRVO), which was not statistically significant. When comparing the two treatment groups with respect to the type of vein occlusion, there was a significant advantage for BRVO patients for the dexamethasone implant monotherapy (BRVO patients in group 1, p = 0.005). Central retinal thickness showed a significant reduction after 6 months only in patients of group 1, both for CRVO (p = 0.01) and BRVO (p = 0.003). First recurrence after the first dexamethasone implant injection occurred after 3.8 months (mean) in CRVO and 3.5 months in BRVO patients (group 1), versus 3.2 and 3.7 months, respectively, in group 2. In group 1, 63.6% with CRVO and 50% with BRVO showed an increased intraocular pressure after treatment; in group 2, 57.1% with CRVO and 50.0% with BRVO, respectively. Conclusion: In CRVO, there was no difference between the two treatment strategies investigated. However, in BRVO, dexamethasone implant monotherapy was associated with better functional outcome.     The Design and Evaluation of Microphone Arrays for the Visualization of Noise Sources on Moving Vehicles

    The present work was directed towards the design of a sideline microphone array specifically adapted to the visualization of automotive noise sources in the 500 Hz to 2000 Hz range. The particular design philosophy followed here involved the minimization of the array redundancy: i.e., the minimization of the number of pairs of microphones that are separated by the same distance in the same directions. The performance of sixty-four element microphone arrays designed according to this principle will be illustrated through the use of simulated motor vehicle passbys. In addition, their performance will be compared with more conventional array designs: e.g., elliptical, and spiral arrays

    The Application of Flame Image Velocimetry to After-injection Effects on Flow Fields in a Small-Bore Diesel Engine

    This study implements Flame Image Velocimetry (FIV), a diagnostic technique based on post-processing of high-speed soot luminosity images, to show the in-flame flow field development impacted by after-injection in a single-cylinder, small-bore optical diesel engine. Two after-injection cases with different dwell times between the main injection and after-injection, namely, close-coupled and long-dwell, as well as a main-injection-only case are compared regarding flow fields, flow vector magnitude, and turbulence intensity distribution. For each case, high-speed soot luminosity movies from 100 individual combustion cycles are recorded at a high frame rate of 45 kHz for FIV processing. The Reynolds decomposition using a spatial filtering method is applied to the obtained flow vectors so that bulk flow structures and turbulence intensity distributions can be discussed. The results show significant after-injection-induced flow structures as a group of vectors travelling back toward the center of the combustion chamber upon the jet impingement on the piston bowl wall and then jet-to-jet collision. Despite higher cyclic variations caused by the after-injection, increased bulk flow magnitude and turbulence intensity upon the after-injection event suggests locally enhanced mixing. Compared to the long-dwell after-injection, the close-coupled after-injection shows more significant turbulence enhancements

    Intravitreal bevacizumab for the treatment of macular oedema secondary to branch retinal vein occlusion

    Purpose: To evaluate the effect of intravitreal bevacizumab (Avastin) injections on visual acuity (VA) and foveal retinal thickness in patients with macular oedema secondary to branch retinal vein occlusion.Methods: A prospective, non-comparative, consecutive, interventional case series of 34 patients. Patients received repeated intravitreal injections of 1.25 mg bevacizumab. Main outcome measures were VA (Snellen charts and ETDRS) and retinal thickness (optical coherence tomography measurements) in a follow-up period of 6 months.Results: Patients presented at a mean age of 69 years (range 44--86). Mean duration of symptoms was 40 weeks (range 1--300). Mean (SD) VA at baseline was 0.79 (0.39) logMAR, improving to 0.51 (0.34) logMAR at 6 months (p = 0.009). Mean number of letters on the ETDRS chart at baseline was 45.3 (19.0), improving to 60.6 (19.9) at 6 months (p = 0.003). Mean (SD) retinal thickness at baseline was 474 (120) \textgreekmm, declining to 316 (41) \textgreekmm at 6 months.Conclusion: Intravitreal injection of 1.25 mg bevacizumb appears to be an effective treatment option for branch retinal vein occlusion

    Strange stars in low-mass binary pulsar systems

    Based on observational facts and a variety of theoretical arguments we discuss in this work the possibility that pulsars in Low-Mass Binary Pulsar systems could be strange stars rather than neutron stars. It is shown that, although subject to reasonable uncertainties, the consideration of the physics of the SQM core and thin normal crusts leads to the prediction of several observed features of the magnetic field history of these systems whitin this working hypothesis.Comment: 6 pages, no figures, PlainTex file submitted to IJMP

    Serially connected forward osmosis membrane elements of pressure-assisted forward osmosis-reverse osmosis hybrid system: Process performance and economic analysis

Due to the improved dilution of draw streams, employing pressure-assisted forward osmosis (PAFO) to the hybrid system of forward osmosis (FO) followed by reverse osmosis (RO) for seawater desalination has been expected to reduce the overall economics. However, replacing FO with PAFO causes an additional energy cost in the seawater dilution step which inevitably leads to a question that PAFO-RO hybrid is truly an economically beneficial option. More importantly, though serial connection of FO elements improves the dilution of initial draw water, this economic benefit is also compensated with the additional membrane. To rationalize its overall performance and economic benefit, thorough performance and economic evaluations were conducted based on actual pilot-scale PAFO operations for serial connection of up to three 8040 FO elements. The results showed the FO-RO hybrid is not an economically feasible option unless a significant unit FO element cost cut-down is guaranteed. Meanwhile, PAFO-RO showed benefits with regards to target RO recovery and unit FO element cost, particularly when two FO elements are serially connected (SE2). It was found that PAFO-RO, indeed, has higher economic potential than FO-RO. A graphical overlapping method suggested in this work can help determine optimal serial configuration and operating conditions of PAFO-RO
