    Postpartum behaviour as predictor of weight change from before pregnancy to one year postpartum

    <p>Abstract</p> <p>Background</p> <p>Postpartum weight retention affects many women and increases the risk of becoming overweight. The research objective was to study modifiable factors contributing to weight change at one year postpartum.</p> <p>Methods</p> <p>In this prospective cohort, postpartum behavior, such as physical activity, sedentary behavior, sleep, and intake of total energy, total fat and saturated fatty acids of 118 Dutch women were assessed in 2003/2004 by self-report at 6 weeks, 6 and 12 months postpartum. Mean postpartum scores were computed for the behavioral measures. In linear regression models it was determined which factors were associated with average weight change from before pregnancy to one year postpartum. Furthermore, factors associated with substantial postpartum weight retention (≥ 5 kg) were also studied in logistic regression models.</p> <p>Results</p> <p>At one year postpartum, the average weight of participants had increased by 0.9 kg (SD 4.4). Moreover, 20% of the women retained ≥ 5 kg. Women who perceived themselves more physically active than others were almost ten times less likely to retain ≥ 5 kg than women who perceived themselves equally active (OR = 0.11, 95%CI: 0.02 - 0.66). Exceeding the guideline for saturated fatty acid intake (OR = 3.40, 95%CI: 1.04 - 11.11), total gestational weight gain (OR = 1.14/kg, 95%CI: 1.01 - 1.27), and not having completed post high school education (OR = 5.13, 95%CI: 1.66 - 15.90) increased the odds of retaining ≥ 5 kg.</p> <p>Conclusions</p> <p>Since one in five women had substantial weight retention postpartum, effective interventions for the prevention of weight retention are much needed. Future studies should evaluate whether interventions focusing on the identified modifiable postpartum factors are successful in reducing weight retention after childbirth.</p

    MALDI imaging mass spectrometry for direct tissue analysis: a new frontier for molecular histology

    Matrix-assisted laser desorption/ionization (MALDI) imaging mass spectrometry (IMS) is a powerful tool for investigating the distribution of proteins and small molecules within biological systems through the in situ analysis of tissue sections. MALDI-IMS can determine the distribution of hundreds of unknown compounds in a single measurement and enables the acquisition of cellular expression profiles while maintaining the cellular and molecular integrity. In recent years, a great many advances in the practice of imaging mass spectrometry have taken place, making the technique more sensitive, robust, and ultimately useful. In this review, we focus on the current state of the art of MALDI-IMS, describe basic technological developments for MALDI-IMS of animal and human tissues, and discuss some recent applications in basic research and in clinical settings

    Prozessinnovation in der Praxis : die Anwendung der Lean-Methode auf Arztpraxen

    Als Methode zur kontinuierlichen Qualitätsverbesserung ist Lean mittlerweile im Gesundheitswesen, insbesondere in Krankenhäusern, etabliert. Bisher gibt es nur wenige Erkenntnisse zur Anwendung von Lean in Arztpraxen. Dieser Beitrag beleuchtet das Innovationspotenzial von Lean in Arztpraxen und zeigt praktische Umsetzungsmöglichkeiten auf. Bisher durchgeführte Studien zeigen, dass Arztpraxen langfristig primär das „Kaizen-Prinzip“ befolgen und in kurzen intensiveren Phasen Projekte umsetzen. Übergeordnetes Ziel der Lean-Initiativen ist meist eine Verbesserung der Effizienz und Produktivität bei gleichbleibender (medizinischer) Qualität und gleichbleibendem oder reduziertem Ressourceneinsatz. Die Akzeptanz der Lean-Implementierung wird stark von der vorherrschenden Kultur und dem Führungsstil beeinflusst und davon, welche Art der Implementierung gewählt wurde (Bottom-up vs. Top-down). Aus der Literatur können für den Praxistransfer drei Handlungsempfehlungen aus den Bereichen Mitarbeitereinbindung, Erfolgsevidenz und patientenzentrierte Outcomes extrahiert werden

    The airbag problem-a potential culprit for bench-to-bedside translational efforts: relevance for Alzheimer's disease

    For the last 20 years, the "amyloid cascade hypothesis" has dominated research aimed at understanding, preventing, and curing Alzheimer's disease (AD). During that time researchers have acquired an enormous amount of data and have been successful, more than 300 times, in curing the disease in animal model systems by treatments aimed at clearing amyloid deposits. However, to date similar strategies have not been successful in human AD patients. Hence, before rushing into further clinical trials with compounds that aim at lowering amyloid-beta (Aβ) levels in increasingly younger people, it would be of highest priority to re-assess the initial assumption that accumulation of Aβ in the brain is the primary pathological event driving AD. Here we question this assumption by highlighting experimental evidence in support of the alternative hypothesis suggesting that APP and Aβ are part of a neuronal stress/injury system, which is up-regulated to counteract inflammation/oxidative stress-associated neurodegeneration that could be triggered by a brain injury, chronic infections, or a systemic disease. In AD, this protective program may be overridden by genetic and other risk factors, or its maintenance may become dysregulated during aging. Here, we provide a hypothetical example of a hypothesis-driven correlation between car accidents and airbag release in analogy to the evolution of the amyloid focus and as a way to offer a potential explanation for the failure of the AD field to translate the success of amyloid-related therapeutic strategies in experimental models to the clinic
