    From database to normbase

    After the database concept, we are ready for the normbase concept. The object is to decouple organizational and technical knowledge that are now mixed inextricably together in the application programs we write today. The underlying principle is to find a way of specifying a social system as a system of norms. Our existing languages, developed to handle machine-like structures, do not enable us to embed our technical systems comfortably into the far more subtle human systems they should be serving.\ud \ud The present approach to the design of formal systems incorporates a philosophical position that obstructs one's thinking about the relationship between social systems and technical systems. The existing specification languages embody a view of the world as an objective reality. But social systems are constructed by their participants, so we propose a language that treats the world as essentially subjective. Our way of doing so introduces two fundamental postulates: (1) there is no reality without an agent, and (2) the agent only knows the world through actions. Based on these postulates, a formalism has been created that enables us to represent systems of social norms.\ud \ud In this system, meaning is regarded as a relationship between sign and behaviour (more strictly, invariants in the flux of behaviour). Semantic analysis is an essential prelude to norm analysis. A new prototype implementation of this normbase is under construction. The goal envisaged is explained in terms of an illustration of how the normbase would be used to develop a system. At every stage of design, the specification is turned immediately into a default version of a working system. The application programmer can then concentrate on tuning this system or developing another that can perform the business functions in the most efficient technical manner. Experiments have already shown the viability of the concepts and methods incorporated in this normbase

    First Observation of Quantum Oscillations in the Ferromagnetic Superconductor UCoGe

    We succeeded in growing high quality single crystals of the ferromagnetic superconductor UCoGe and measured the magnetoresistance at fields up to 34T. The Shubnikov-de Haas signal was observed for the first time in a U-111 system (UTGe, UTSi, T: transition metal). A small pocket Fermi surface (F~1kT) with large cyclotron effective mass 25m0 was detected at high fields above 22T, implying that UCoGe is a low carrier system accompanyed with heavy quasi-particles. The observed frequency decreases with increasing fields, indicating that the volume of detected Fermi surface changes nonlinearly with field. The cyclotron mass also decreases, which is consistent with the decrease of the A coefficient of resistivity.Comment: 5 pages, 5 figures, accepted for publication in J. Phys. Soc. Jp

    A prognostic index for operable, node-negative breast cancer

    Clinical data and samples from patients diagnosed, more than 10 years previously, with operable node-negative breast cancer (participants in the Scottish Adjuvant Tamoxifen trial), were revisited, Cases with two distinct categories of outcome were selected; more than 10 years disease-free survival ('good outcome') or distant relapse within 6 years of diagnosis ('poor outcome'). An initial set of cases was analysed for a range of putative prognostic markers and a prognostic index, distinguishing the two outcome categories, was calculated. This index was then validated by testing its predictive power on a second, independent set of cases. A combination of histological grade plus immunochemical staining for BCL-2, p27 and Cyclin D 1, generated a useful prognostic index for tamoxifen-treated patients but not for those treated by surgery alone, The value of the index was confirmed in a second set of tamoxifen-treated, early stage breast cancers. Over-all, it correctly predicted good and poor outcome in 79 and 74% of cases, respectively (odds ratio 11.0). Other markers assessed added little to prediction of outcome. In the case of molecular assays, sensitivity and reliability were compromised by the age of the tissue specimens and the variability of fixation protocols. In selecting patients for adjuvant systemic chemotherapy, the proposed index improves considerably on current international guidelines and matches the performance reported for 'gene-expression signature' analysis.     Adrenocortical oncocytic carcinoma with recurrent metastases: a case report and review of the literature

    <p>Abstract</p> <p>Background</p> <p>Adrenal cortex oncocytic carcinoma (AOC) represents an exceptional pathological entity, since only 22 cases have been documented in the literature so far.</p> <p>Case presentation</p> <p>Our case concerns a 54-year-old man with past medical history of right adrenal excision with partial hepatectomy, due to an adrenocortical carcinoma. The patient was admitted in our hospital to undergo surgical resection of a left lung mass newly detected on chest Computed Tomography scan. The histological and immunohistochemical study revealed a metastatic AOC. Although the patient was given mitotane orally in adjuvant basis, he experienced relapse with multiple metastases in the thorax twice in the next year and was treated with consecutive resections. Two and a half years later, a right hip joint metastasis was found and concurrent chemoradiation was given. Finally, approximately five years post disease onset, the patient died due to massive metastatic disease. A thorough review of AOC and particularly all diagnostic difficulties are extensively stated.</p> <p>Conclusion</p> <p>Histological classification of adrenocortical oncocytic tumours has been so far a matter of debate. There is no officially established histological scoring system regarding these rare neoplasms and therefore many diagnostic difficulties occur for pathologists.</p
