    Characterization of antimicrobial resistance mechanisms of multidrug resistant Gram negative bacterial wound infections and their clinical epidemiology from a tertiary care hospital in Karnataka, India

    Background: Extended spectrum beta lactamases, AmpC and Metallo-betalactamases in GNB isolates are a common occurrence in most Indian hospitals. The presence of these antimicrobial resistance mechanisms contributes to prolonged hospital stay, poor quality of life, increased morbidity and mortality among patients with these infections. The aim of the study was to analyse the antimicrobial resistance mechanisms of multidrug resistant Gram negative bacterial wound infection and their clinical epidemiology.Methods: A prospective study was conducted for one year among 100 patients of Kasturba Medical College, Manipal admitted with MDR GNB wound infections. The antibiogram and phenotypic resistance mechanisms of the bacterial isolate from these infections were identified using phenyl boronic acid and ethyl diacetate.┬а The empirical therapy, specific therapy and clinical outcome of the patients were also analyzed.Results: Out of 100 study patients, 152 MDR GNB isolates were obtained.┬а 73% patients were admitted in the surgical wards. 43% patients had diabetes. Ulcers (27%) and abscess (25%) were the most common diagnosis. Escherichia coli (39%), Klebsiella pneumoniae (24%) and Pseudomonas aeruginosa (19%) were the most common isolates. Maximum number of ESBL was seen among Enteric Gram negative bacilli (36%), MBL was seen among Pseudomonas aeruginosa and Acinetobacter species (55% each), AmpC was seen among enteric GNB (10%) and Acinetobacter species (18%). Cefaperazone sulbactam, amikacin and meropenem were the most common antibiotics given as specific therapy. Clinical response was observed among 93% patients.Conclusions: The determination of the antimicrobial resistance mechanisms of GNB isolates from wound infections plays a major role in establishing an antibiotic policy for the treatment of these infections

    Objective: Lantana camaraL (L. c) has been known for its anti-bacterial activity since quite some time in its various extracts and pharmacological preparations. In view of this, we plan to study and compare the wound healing property of tincture of L. cin surgically induced wound in animal models with tincture of iodine (I2).Methods: An herbal tincture was developed from stems of L. cin concentrations of 20%, 40%, alcohol in tincture was followed as mentioned in IP, tested against tincture I2 in same aged different groups of rats under incision and excision models, also with commonly wound infecting bacteria like S. aureus, E. coli, E. fecalis, P. aeruginosa[a1]├В┬а. Wounds were induced surgically under anaesthesia. Excised rats were observed for wound contraction and epithelialisation. On 7th day, incised rats were subjected to wound dehiscence test. Tissue biopsy took on day 5, 14 and21 for histopathological studies.Results: Both test groups were found statistically significant against standard with P<0.05 and with mean epithelialisation day value 11├В┬▒1.02 in the excision model and P<0.05 between the groups in incision model depicting 20% tincture as better with a mean value of 366.67├В┬▒47.2. Histologically 40% tincture was found non-inferior against standard. Tincture was active against mentioned microbes in various dilutions.Conclusion: Tincture L. c was non-inferior to iodine. Further studies with concentrated extracts of L. cmay reveal its accelerated healing and better anti-microbial competency so that it can be developed for clinical practice.├В

    Evolution of the N=20 and 28 Shell Gaps and 2-particle-2-hole states in the FSU Interaction

    The FSU spsdfpspsdfp cross-shell interaction for the shell model was successfully fitted to a wide range of mostly intruder negative parity states of the sdsd shell nuclei. This paper reports the application of the FSU interaction to systematically trace out the relative positions of the effective single-particle energies of the 0f7/20f_{7/2} and 1p3/21p_{3/2} orbitals, the evolution from normally ordered low-lying states to the "Island of Inversion" (IoI), and the behavior of a wide range of excited states with a 0f7/20f_{7/2} proton and neutron coupled to maximum spin of 7тДП7 \hbar. Above a proton number of about 13 the 0f7/20f_{7/2} orbital lies below that of 1p3/21p_{3/2}, which is considered normal ordering, but systematically at Z=10Z = 10 to 1212 the orbitals cross. The calculations reproduce well the 2p2h - 0p0h inversion in the configurations of nuclei inside the IoI, they reproduce the absolute binding energies and the transition to normal ordering as the proton number approaches that of the neutrons. The important role of 1p3/21p_{3/2} neutron pairs in the IoI is also demonstrated. The calculations account well for the energies of the fully aligned states with 0, 1, or 2 individual sdsd nucleon aligned in spin with the aligned ╧А0f7/2\pi 0f_{7/2} - ╬╜0f7/2\nu 0f_{7/2} pair and reproduce well their systematic variation with AA and number of aligned sdsd nucleons. The results presented in this paper give hope for the predictive power of the FSU interaction for more exotic nuclei to be explored in near future

    Multimodal Treatment Eliminates Cancer Stem Cells and Leads to Long-Term Survival in Primary Human Pancreatic Cancer Tissue Xenografts.

We and others have previously identified a subpopulation of pancreatic cancer stem cells within the tumor as a critical therapeutic target and additionally shown that the tumor stroma represents not only a restrictive barrier for successful drug delivery, but also serves as a paracrine niche for cancer stem cells. Therefore, we embarked on a large-scale investigation on the effects of combining chemotherapy, hedgehog pathway inhibition, and mTOR inhibition in a preclinical mouse model of pancreatic cancer. EXPERIMENTAL DESIGN: Prospective and randomized testing in a set of almost 200 subcutaneous and orthotopic implanted whole-tissue primary human tumor xenografts. RESULTS: The combined targeting of highly chemoresistant cancer stem cells as well as their more differentiated progenies, together with abrogation of the tumor microenvironment by targeting the stroma and enhancing tissue penetration of the chemotherapeutic agent translated into significantly prolonged survival in preclinical models of human pancreatic cancer. Most pronounced therapeutic effects were observed in gemcitabine-resistant patient-derived tumors. Intriguingly, the proposed triple therapy approach could be further enhanced by using a PEGylated formulation of gemcitabine, which significantly increased its bioavailability and tissue penetration, resulting in a further improved overall outcome. CONCLUSIONS: This multimodal therapeutic strategy should be further explored in the clinical setting as its success may eventually improve the poor prognosis of patients with pancreatic ductal adenocarcinoma
