    Recent Advances in Epilepsy Surgery

The modern practice of epilepsy surgery requires multiple modalities of presurgical investigations such as video-EEG, intracranial EEG, high-resolution imaging, advanced functional imaging, and clinical analysis. A multidisciplinary approach is essential, including close collaboration between neurosurgeons, neurologists, neurophysiologists, neuropsychologists, neuropsychiatrists, and neuroradiologists. Candidates for epilepsy surgery require a history of seizures that are refractory to appropriate medical therapy. A meticulous selection of candidates will lead to a better chance of freedom from seizure. Epilepsy surgery includes a variety of surgical procedures including resective surgery for focal refractory seizure, which offers a significant chance of seizure freedom in temporal and extratemporal lobe epilepsy. Palliative treatment for patients who are not candidates for resective surgery, such as vagal nerve stimulation, deep stimulation, and callosotomy, offers further options. We reviewed and analyzed the recent scientific literature and forthcoming advances that will impact on the future of epilepsy surgery. This chapter on recent advances in epilepsy surgery emphasizes improved methods of assessment, a better understanding of seizures, the development of new surgical techniques, and the outcome of epilepsy surgery

Purpose: Medical students are the cornerstone for future public health awareness; this study aimed to assess e-cigarettes perceptions and use among medical students. Methods: This is a cross-sectional, questionnaire-based national study. The study included all five medical schools in the country and addressed students in their clinical years. Data were collected through a self-administered and structured paper-based questionnaire between Dec 1, 2018, and Mar 30, 2019. Data obtained were analyzed using IBM SPSS Statistics for Windows and univariate inferential tests were employed to examine differences between groups. Results: A total sample of 1819 (97.4%) students returned their questionnaires; with a declaration of their smoking habits from the five medical schools, 47% are males, and 53% are females. Only 13 students 0.7% of all students or 2.1% of all smokers used e-cigarettes solely, but 177 students 9.5% of all students or 29% of all smokers used e-cigarettes combined with other types of smoking. Most medical students (86%) were aware of e-cigarettes. The main motive behind using e-cigarettes was the rare and different flavors provided by these cigarettes. The general perception among smokers and non-smokers is that e-cigarettes are less harmful and helpful in quitting smoking. Conclusion: Most medical students are aware of e-cigarettes. While their use as the only form of smoking is rare, e-cigarettes are frequently used in combination with other types of smoking

18F-fluorodeoxyglucose positron emission tomography-positive sarcoidosis after chemoradiotherapy for Hodgkin’s disease: a case report

<p>Abstract</p> <p>Introduction</p> <p>The occurrence of granulomatous disease in the setting of Hodgkin's disease is rare; however, when it occurs it can pose significant clinical and diagnostic challenges for physicians treating these patients.</p> <p>Case presentation</p> <p>We report the case of a 33-year-old Caucasian woman of Mediterranean descent with newly diagnosed 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET)/computed tomography (CT) scan-positive, early-stage Hodgkin's disease involving the cervical nodes who, despite having an excellent clinical response to chemotherapy, had a persistent 18F-FDG PET scan-positive study, which was suggestive of residual or progressive disease. A subsequent biopsy of her post-chemotherapy PET-positive nodes demonstrated sarcoidosis with no evidence of Hodgkin's disease.</p> <p>Conclusion</p> <p>This case highlights the fact that abnormalities observed on posttherapy PET/CT scans in patients with Hodgkin's disease are not always due to residual or progressive disease. An association between Hodgkin's disease and/or its treatment with an increased incidence of granulomatous disease appears to exist. Certain patterns of 18F-FDG uptake observed on PET/CT scans may suggest other pathologies, such as granulomatous inflammation, and because of the significant differences in prognosis and management, clinicians should maintain a low threshold of confidence for basing their diagnosis on histopathological evaluations when PET/CT results appear to be incongruent with the patient's clinical response.</p
