Advancing Care Excellence for Seniors: Reflections on a John A. Hartford Foundation-funded Nursing Education Program

This brief will describe the work and assess the impact of the NLN Advancing Care Excellence for Seniors initiative, or ACE.S, which the John A. Hartford Foundation supported from 2009 to 2012. ACE.S was the first national effort to prepare students in all pre-licensure nursing programs, including associate degree programs, to deliver high-quality care to older adults in a variety of settings, by providing new educational opportunities, teaching tools, and other resources to help pre-licensure nursing faculty offer geriatric content and understanding to their students

Our Parents, Ourselves: Health Care for an Aging Population; A Report of the Dartmouth Atlas Project

The new Dartmouth Atlas, funded by The John A. Hartford Foundation, is a report card that analyzes Medicare data to show us where the United States is making progress in patient-centered, evidence-based care for Medicare beneficiaries and where improvement is still needed. It also offers insight into regional variations in care.Filling in the gaps in our knowledge about the state of care across the country will help health care providers, health systems, and patients and families work together to improve care for all older adults.This Dartmouth Atlas report looks at a number of measures from Medicare data, including:The number of days older adults spend in contact with the health care system;Use of high-risk medications;Cancer screening rates (and how they compare with recommendations);30-day hospital readmission rates;Annual Wellness Visit (AWV) rates;Late hospice referral; andThe number of days spent in intensive care.The report also offers a historical look at key practices, comparing data from 2003-05 and 2012

The Effectiveness of a Geriatrics Curriculum

With support from the AAMC/John A. Hartford Foundation the University of Massachusetts Medical School developed a Geriatrics curriculum and faculty development that would be integrated across all four years. Beginning in Fall 2001, these were implemented over the next two academic years in both preclinical and clinical areas and are highlighted on the timeline above. Would implementation of this new Geriatrics curriculum and faculty development impact students’ ratings of instruction time in Geriatrics? The Fierce Urgency of Now: The John A. Hartford Foundation 2020 Annual Report

We have lived through a transformative year. Vaccinations are showing promise in controlling the COVID-19 pandemic that consumed much of 2020, and it is vitally important to take stock of what we have learned. From the harrowing experiences of frontline health care providers, to the pain felt by families who lost loved ones and were kept apart due to the virus, to the insidious structural racism and disparities continuing to harm our society, it has been a challenging time for all of us. Older adults and their families have disproportionately suffered the consequences, especially older adults of color and those living in nursing homes. As we reflect on the past year, our commitment to rapidly improving the care of older adults has only grown deeper and stronger
