    Developing a model of mental health self-care support for children and young people through an integrated evaluation of available types of provision involving systematic review, meta-analysis and case study

    Background The mental health of children and young people (CYP) is a major UK public health concern. Recent policy reviews have identified that service provision for CYP with mental health needs is not as effective, responsive, accessible or child-centred as it could be. Following on from a previous National Institute for Health Research (NIHR) study into self-care support for CYP with long-term physical health needs, this study explored self-care support’s potential in CYP’s mental health. Objectives To identify and evaluate the types of mental health self-care support used by, and available to, CYP and their parents, and to establish how such support interfaces with statutory and non-statutory service provision. Design Two inter-related systematic literature reviews (an effectiveness review with meta-analysis and a perceptions review), together with a service mapping exercise and case study. Setting Global (systematic reviews); England and Wales (mapping exercise and case study). Participants (case study) Fifty-two individuals (17 CYP, 16 family members and 19 staff) were interviewed across six sites. Main outcome measures (meta-analysis) A measure of CYP’s mental health symptomatology. Data sources (literature reviews) MEDLINE, Cumulative Index to Nursing and Allied Health Literature (CINAHL), PsycINFO, All Evidence-Based Medicine (EBM) Reviews, Applied Social Sciences Index and Abstracts (ASSIA) and Education Resources Information Center (ERIC). Review methods Titles and abstracts of papers were screened for relevance then grouped into studies. Two independent reviewers extracted data from studies meeting the inclusion criteria. A descriptive analysis and meta-analysis were conducted for the effectiveness review; descriptive analyses were conducted for the perceptions review. These analyses were integrated to elicit a mixed-methods review. Results Sixty-five of 71 included studies were meta-analysable. These 65 studies elicited 71 comparisons which, when meta-analysed, suggested that self-care support interventions were effective at 6-month [standardised mean difference (SMD) = −0.20; 95% confidence interval (CI) −0.28 to −0.11] and 12-month (SMD = −0.12; 95% CI −0.17 to −0.06) follow-ups. However, judged against Cochrane criteria, the studies were mostly low quality. Key elements of self-care support identified in the perceptions review were the acquisition of knowledge and skills, peer support and the relationship with the self-care support agent; CYP also had different perceptions from adults about what is important in self-care support. The mapping exercise identified 27 providers of 33 self-care support services. According to the case study data, effective self-care support services are predicated on flexibility; straightforward access; non-judgemental, welcoming organisations and staff; the provision of time and attention; opportunities to learn and practise skills relevant to self-care; and systems of peer support. Conclusions Mental health self-care support interventions for CYP are modestly effective in the short to medium term. Self-care support can be conceptualised as a process which has overlap with ‘recovery’. CYP and their families want choice and flexibility in the provision of such interventions and a continued relationship with services after the nominal therapy period. Those delivering self-care support need to have specific child-centred attributes. Future work Future work should focus on under-represented conditions (e.g. psychosis, eating disorders, self-harm); the role of technology, leadership and readiness in self-care support; satisfaction in self-care support; the conceptualisation of self-care support in CYP’s mental health; and efficacy and cost-effectiveness

    Disequilibrium, adaptation and the Norse settlement of Greenland

    This research was supported by the University of Edinburgh ExEDE Doctoral Training Studentship and NSF grant numbers 1202692 and 1140106.There is increasing evidence to suggest that arctic cultures and ecosystems have followed non-linear responses to climate change. Norse Scandinavian farmers introduced agriculture to sub-arctic Greenland in the late tenth century, creating synanthropic landscapes and utilising seasonally abundant marine and terrestrial resources. Using a niche-construction framework and data from recent survey work, studies of diet, and regional-scale climate proxies we examine the potential mismatch between this imported agricultural niche and the constraints of the environment from the tenth to the fifteenth centuries. We argue that landscape modification conformed the Norse to a Scandinavian style of agriculture throughout settlement, structuring and limiting the efficacy of seasonal hunting strategies. Recent climate data provide evidence of sustained cooling from the mid thirteenth century and climate variation from the early fifteenth century. Archaeological evidence suggests that the Norse made incremental adjustments to the changing sub-arctic environment, but were limited by cultural adaptations made in past environments.Publisher PDFPeer reviewe

    The James Webb Space Telescope Mission

    Twenty-six years ago a small committee report, building on earlier studies, expounded a compelling and poetic vision for the future of astronomy, calling for an infrared-optimized space telescope with an aperture of at least 4m4m. With the support of their governments in the US, Europe, and Canada, 20,000 people realized that vision as the 6.5m6.5m James Webb Space Telescope. A generation of astronomers will celebrate their accomplishments for the life of the mission, potentially as long as 20 years, and beyond. This report and the scientific discoveries that follow are extended thank-you notes to the 20,000 team members. The telescope is working perfectly, with much better image quality than expected. In this and accompanying papers, we give a brief history, describe the observatory, outline its objectives and current observing program, and discuss the inventions and people who made it possible.     Nicotine Stimulates Biliary Proliferation and Fibrogenesis and Potentiates Cholangiocarcinoma Growth

    Head of Department: Alejandro ArroligaNicotine is a major addictive substance in cigarette smoke, and has powerful biological properties throughout the body. Studies have demonstrated that nicotine promotes the progression of a number of disease processes, including cancer and fibrosis. Recent evidence suggests that nicotine plays a role in liver fibrosis. Cholangiocytes are the epithelial cells that line the intrahepatic and extrahepatic bile ducts. During biliary injury that occurs with diseases such as primary sclerosing cholangitis (PSC), cholangiocytes undergo a reactive process termed "ductular reaction," and display a neuroendocrine phenotype that may contribute to the fibrotic pathologies, and eventual development of cholangiocarcinoma. Epidemiological data indicate that smoking contributes to biliary fibrosis and cholangiocarcinoma. Due to this reason and the prevalence of nicotine usage, it was important to understand the role that nicotine plays in the progression of biliary fibrosis, and the regulators of this process. We first examined whether nicotine has pro-proliferative effects on the biliary system. We demonstrated in vitro that nicotine increases the proliferation of the normal cholangiocyte cell line (NRIC) by MTS assay and PCNA expression by western blot. We also demonstrated that nicotine works through a calcium/inositol triphosphate/phosphorylated extracellular regulated kinase (Ca2+/IP3/p-ERK) intracellular signaling pathway in NRIC. In vivo, rats were treated with nicotine via osmotic minipump infusion for two weeks. In the nicotine-treated group, we found an increase in the proliferation of cholangiocytes identified by cytokeratin (CK-19, a cholangiocyte specific marker) staining, and proliferating cell nuclear antigen (PCNA) expression. Increased expression of fibrotic genes/proteins was seen in liver tissue, and cholangiocytes isolated from rodent livers. Next, we sought to determine the effect of nicotine on cholangiocarcinoma proliferation. In vitro, nicotine stimulated the proliferation of cholangiocarcinoma growth, in the MzChA-1, CCLP1, and HuCC-Tl cell lines. In a nude mouse xenograft model, nicotine treatment caused an increase in tumor volume and PCNA expression. These data support the current literature suggesting that nicotine may induce fibrogenesis, and contributes to the liver pathology observed in smokers. They also support the notion that nicotine stimulates the growth of biliary cancer and call for further investigation of the subject

    Stress relaxation insensitive designs for metal compliant mechanism threshold accelerometers

    We present two designs for metal compliant mechanisms for use as threshold accelerometers which require zero external power. Both designs rely on long, thin flexures positioned orthogonally to a flat body. The first design involves cutting or stamping a thin spring-steel sheet and then bending elements to form the necessary thin flexors. The second design uses precut spring-steel flexure elements mounted into a mold which is then filled with molten tin to form a bimetallic device. Accelerations necessary to switch the devices between bistable states were measured using a centrifuge. Both designs showed very little variation in threshold acceleration due to stress relaxation over a period of several weeks. Relatively large variations in threshold acceleration were observed for devices of the same design, most likely due to variations in the angle of the flexor elements relative to the main body of the devices.     Using a bioaerosol personal sampler in combination with real-time PCR analysis for rapid detection of airborne viruses

    We have recently developed a new personal sampler and demonstrated its feasibility for detection of viable airborne microorganisms including bacteria, fungi and viruses. To accelerate the time-consuming analytical procedure involving 2-5 days of biological testing, we employed a real-time PCR protocol in conjunction with the personal sampler for collection of airborne viruses. The advantage of this approach is that if the presence of a particular pathogen in the air is detected by the PCR, the remaining collecting liquid can be further analysed by more time-consuming biological methods to estimate the number of airborne infectious/live microorganisms. As sampling of bioaerosols in natural environments is likely to be associated with substantial contamination by a range of microorganisms commonly existing in an ambient air, an investigation of the specificity of detection by targeted PCR analysis is required. Here we present the results of the study on the detection of Influenza virus in the ambient air contaminated with high concentrations of bacteria and fungi using real-time PCR protocol. The combined sampling PCR detection method was found to be fully feasible for the rapid (similar to 2.5 h) and highly specific (no cross-reactivity) identification of the labile airborne virus in the air containing elevated concentrations of other microorganisms

    Impact of Daily Electronic Laboratory Alerting on Early Detection and Clinical Documentation of Acute Kidney Injury in Hospital Settings

    Acute kidney injury, especially early-stage disease, is a common hospital comorbidity requiring timely recognition and treatment. We investigated the effect of daily laboratory alerting of patients at risk for acute kidney injury as measured by documented International Classification of Diseases diagnoses. A quasi-experimental study was conducted at 8 New York hospitals between January 1, 2014, and June 30, 2017. Education of clinical documentation improvement specialists, physicians, and nurses was conducted from July 1, 2014, to December 31, 2014, prior to initiating daily hospital-wide laboratory acute kidney injury alerting on January 1, 2015. Incidence based on documented International Classification of Diseases diagnosis of acute kidney injury and acute tubular necrosis during the intervention periods (3 periods of 6 months each: January 1 to June 30 of 2015, 2016, and 2017) were compared to one preintervention period (January 1, 2014, to June 30, 2014). The sample consisted of 269 607 adult hospital discharges, among which there were 39 071 episodes based on laboratory estimates and 27 660 episodes of documented International Classification of Diseases diagnoses of acute kidney injury or acute tubular necrosis. Documented incidence improved significantly from the 2014 preintervention period (5.70%; 95% confidence interval: 5.52%-5.88%) to intervention periods in 2015 (9.89%; 95% confidence interval, 9.66%-10.12%; risk ratio = 1.73, P < .001), 2016 (12.76%; 95% confidence interval, 12.51%-13.01%; risk ratio = 2.24, P < .001), and 2017 (12.49%; 95% confidence interval, 12.24%-12.74%; risk ratio = 2.19, P < .001). A multifactorial intervention comprising daily laboratory alerting and education of physicians, nurses, and clinical documentation improvement specialists led to increased recognition and clinical documentation of acute kidney injury
