    Mapping Unexploded Ordnance in Syria: Harnessing the Power of Open-Source

    For over a decade, the widespread use of explosive weapons by all sides in the Syrian conflict has been well documented by a litany of public sources. Many of these explosive munitions fail to detonate as intended, thereby becoming unexploded ordnance (UXO) that threaten post-conflict recovery. To begin the process of clearing these explosive remnants of war (ERW), desk studies/non-technical studies can be utilized to initially assess the concentration and distribution of explosive weapons across a conflict zone, which in turn suggest the risk of UXO in an area. Traditional methods in non-technical surveys (NTS) focus on unweighted conflict intensity scores (counting the number of events) or after-the-fact munition detonations to determine current contamination.The authors propose a novel, nuanced approach to counting the number of munitions per event, not just the number of events. This new open-source weighted estimate (OSWE) method contains higher-fidelity data for analysis with more specific coverage across a larger geographic area than prior models. Using crucial and corroborated open-source investigation workflows, the authors created a nationwide assessment paradigm. In comparison with older models, we anticipate that the OSWE method of estimating UXO concentration is more useful across a greater range of geographic scopes through its leverage of big data, weighted nature, and data selection for events likely to generate UXO. The OSWE method also produces an estimate for UXO in Syria (a minimum of 100,000 nationwide). These are important findings, as more accurate estimates can be replicated across contexts, including in Ukraine

    Prognostic Communication Between Oncologists and Parents of Children With Advanced Cancer

    BACKGROUND AND OBJECTIVES: Parents of children with cancer perceive deficits in quality of prognostic communication. How oncologists disclose information about disease progression and incurability and how prognostic communication impacts parental understanding of prognosis are poorly understood. In this study, we aimed to (1) characterize communication strategies used by pediatric oncologists to share prognostic information across a child\u27s advancing illness course and (2) explore relationships between different communication approaches and concordance of oncologist-parent prognostic understanding. METHODS: In this prospective, longitudinal, mixed-methods study, serial disease reevaluation conversations were audio recorded across an advancing illness course for children with cancer and their families. Surveys and interviews also were conducted with oncologists and caregivers at specific time points targeting disease progression. RESULTS: Seventeen children experienced advancing illness on study, resulting in 141 recordings (40 hours). Fewer than 4% of recorded dialogue constituted prognostic communication, with most codes (77%) occurring during discussions about frank disease progression. Most recordings at study entry contained little or no prognosis communication dialogue, and oncologists rated curability lower than parents across all dyads. Parent-oncologist discordance typically was preceded by conversations without incurability statements; ultimately, concordance was achieved in most cases after the oncologist made direct statements about incurability. Content analysis revealed 3 distinct patterns (absent, deferred, and seed planting) describing the provision of prognostic communication across an advancing pediatric cancer course. CONCLUSIONS: When oncologists provided direct statements about incurability, prognostic understanding appeared to improve. Further research is needed to determine optimal timing for prognostic disclosure in alignment with patient and family preferences

    Creation and pilot testing of cases for case-based learning: A pedagogical approach for pathology cancer diagnosis

    Background: Case-based learning (CBL) is an established pedagogical active learning method used in various disciplines and defined based on the field of study and type of case. The utility of CBL for teaching specific aspects of cancer diagnosis to practising pathologists has not been previously studied in sub-Saharan Africa. Objectives: We aimed to pilot test standardised cancer cases on a group of practising pathologists in sub-Saharan Africa to evaluate case content, clarity of questions and delivery of content. Methods: Expert faculty created cases for the four most commonly diagnosed cancers. The format included mini-cases and bullet cases which were all open-ended. The questions dealt with interpretation of clinical information, gross specimen examination, morphologic characteristics of tumours, ancillary testing, reporting and appropriate communication to clinicians. Results: Cases on breast, cervical, prostate and colorectal cancers were tested on seven practising pathologists. Each case took an average of 45–90 min to complete. Questions that were particularly challenging to testers were on: •  Specimens they should have been but for some reason were not exposed to in routine practice. •  Ancillary testing and appropriate tumour staging. New knowledge gained included tumour grading and assessment of radial margins. Revisions to cases were made based on testers’ feedback, which included rewording of questions to reduce ambiguity and adding of tables to clarify concepts. Conclusion: Cases were created for CBL in Kenya, but these are applicable elsewhere in Africa and beyond to teach cancer diagnosis. The pilot testing of cases prepared faculty for the actual CBL course and feedback provided by the testers assisted in improving the questions and impact on day-to-day practice

    #InSituPathologists: how the #USCAP2015 meeting went viral on Twitter and founded the social media movement for the United States and Canadian Academy of Pathology

    Professional medical conferences over the past five years have seen an enormous increase in the use of Twitter in real-time, also known as live-tweeting . At the United States and Canadian Academy of Pathology (USCAP) 2015 annual meeting, 24 attendees (the authors) volunteered to participate in a live-tweet group, the #InSituPathologists. This group, along with other attendees, kept the world updated via Twitter about the happenings at the annual meeting. There were 6,524 #USCAP2015 tweets made by 662 individual Twitter users; these generated 5,869,323 unique impressions (potential tweet-views) over a 13-day time span encompassing the dates of the annual meeting. Herein we document the successful implementation of the first official USCAP annual meeting live-tweet group, including the pros/cons of live-tweeting and other experiences of the original #InSituPathologists group members. No prior peer-reviewed publications to our knowledge have described in depth the use of an organized group to live-tweet a pathology meeting. We believe our group to be the first of its kind in the field of pathology
