    Dermoscopic characterization and image study of a Sister Mary Joseph nodule in a patient with esophageal cancer

    AbstractA Sister Mary Joseph nodule (SMJN) is rare cutaneous metastasis spreading to the umbilicus. It originates mostly from an intra-abdominal or pelvic malignancy and is an ominous sign. In this paper, we report a case of a SMJN in a 64-year-old man with esophageal cancer. We recorded his clinical progression, radiologic and nuclear scintigraphic images, and histopathologic and dermoscopic findings. The image study provided information on the localization of the SMJN and dermoscopy showed a polymorphous vascular pattern. The patient received palliative therapy because of terminal disease

    A novel regulatory event-based gene set analysis method for exploring global functional changes in heterogeneous genomic data sets

    <p>Abstract</p> <p>Background</p> <p>Analyzing gene expression data by assessing the significance of pre-defined gene sets, rather than individual genes, has become a main approach in microarray data analysis and this has promisingly derive new biological interpretations of microarray data. However, the detection power of conventional gene list or gene set-based approaches is limited on highly heterogeneous samples, such as tumors.</p> <p>Results</p> <p>We developed a novel method, the regulatory <b>e</b>vent-based <b>G</b>ene <b>S</b>et <b>A</b>nalysis (eGSA), which considers not only the consistently changed genes but also every gene regulation (event) of each sample to overcome the detection limit. In comparison with conventional methods, eGSA can detect functional changes in heterogeneous samples more precisely and robustly. Furthermore, by utilizing eGSA, we successfully revealed novel functional characteristics and potential mechanisms of very early hepatocellular carcinoma (HCC).</p> <p>Conclusion</p> <p>Our study creates a novel scheme to directly target the major cellular functional changes in heterogeneous samples. All potential regulatory routines of a functional change can be further analyzed by the regulatory event frequency. We also provide a case study on early HCCs and reveal a novel insight at the initial stage of hepatocarcinogenesis. eGSA therefore accelerates and refines the interpretation of heterogeneous genomic data sets in the absence of gene-phenotype correlations.</p

    Safety and Efficacy of Tien-Hsien Liquid Practical in Patients with Refractory Metastatic Breast Cancer: A Randomized, Double-Blind, Placebo-Controlled, Parallel-Group, Phase IIa Trial

    To evaluate the safety and efficacy of Tien-Hsien Liquid Practical (THL-P), a Chinese herbal mixture, in patients with refractory metastatic breast cancer, we performed a randomized, double-blind, placebo-controlled, parallel-group, phase IIa pilot trial. Patients were randomly assigned to either receive THL-P or matching placebo and followed up every 4 weeks for 24 weeks. The primary endpoint was changes in the global health status/quality of life (GHS/QOL) scale. The secondary endpoints were changes in functional and symptom scales, immunomodulating effects, and adverse events. Sixty-three patients were enrolled between June 2009 and June 2011. The intent-to-treat population included 28 patients in the THL-P group and 11 patients in the placebo group. Compared to the placebo group, the THL-P group had significant improvement from baseline to last visit in GHS/QOL (41.7 versus −33.3; P < 0.05), CD3, CD4/CD8, CD19, CD16+56 positive cells (P < 0.05), and higher levels of physical, role, emotional, and cognitive functioning, as well as decreased fatigue and systemic side effects. Treatment-related adverse events were mild constipation and localized itching, and no serious adverse events were reported. THL-P appears to be a safe alternative adjuvant treatment for patients with refractory metastatic breast cancer, as it effectively improves QOL and palliates cancer-related symptoms

    Plasmonic Circular Nanostructure for Enhanced Light Absorption in Organic Solar Cells

    This study attempts to enhance broadband absorption in advanced plasmonic circular nanostructures (PCN). Experimental results indicate that the concentric circular metallic gratings can enhance broadband optical absorption, due to the structure geometry and the excitation of surface plasmon mode. The interaction between plasmonic enhancement and the absorption characteristics of the organic materials (P3HT:PCBM and PEDOT:PSS) are also examined. According to those results, the organic material's overall optical absorption can be significantly enhanced by up to ~51% over that of a planar device. Additionally, organic materials are enhanced to a maximum of 65% for PCN grating pitch = 800 nm. As a result of the PCN's enhancement in optical absorption, incorporation of the PCN into P3HT:PCBM-based organic solar cells (OSCs) significantly improved the performance of the solar cells: short-circuit current increased from 10.125 to 12.249 and power conversion efficiency from 3.2% to 4.99%. Furthermore, optimizing the OSCs architectures further improves the performance of the absorption and PCE enhancement

    Fabrication of Nanostructured PLGA Scaffolds Using Anodic Aluminum Oxide Templates

    PLGA (poly(lactic-co-glycolic acid)) is one of the most used biodegradable and biocompatible materials. Nanostructured PLGA even has great application potentials in tissue engineering. In this research, a fabrication technique for nanostructured PLGA membrane was investigated and developed. In this novel fabrication approach, an anodic aluminum oxide (AAO) film was use as the template ; the PLGA solution was then cast on it ; the vacuum air-extraction process was applied to transfer the nano porous pattern from the AAO membrane to the PLGA membrane and form nanostures on it. The cell culture experiments of the bovine endothelial cells demonstrated that the nanostructured PLGA membrane can double the cell growing rate. Compared to the conventional chemical-etching process, the physical fabrication method proposed in this research not only is simpler but also does not alter the characteristics of the PLGA. Although the market scale of global e-commerce market has declined, global online shopping continues to grow. Many previous researches focused on the effect of website design characteristics on online impulsive buying behavior, and few have explored such behavior from consumer individual internal factor perspectives. This paper aims to explore and integrate individual internal factors influencing consumer online buying impulsiveness, and further to recognize the relationships among these factors. The results showed as follows: (1) hedonic consumption needs, impulsive buying tendency, positive affect and normative evaluations positively influence buying impulsiveness, respectively; (2) hedonic consumption needs positively influence positive affect; (3) impulsive buying tendency positively influences normative evaluations; (4) normative evaluations positively influence positive affect

    Association between health examination items and body mass index among school children in Hualien, Taiwan

    BACKGROUND: To assess the prevalence of obesity and major physical examination items including dental caries, myopia, pinworm, hematuria, and proteinuria among school children in Hualien, Taiwan. In addition, the health status differences between gender, grader, levels of residence urbanization, and body mass index (BMI) were examined. METHODS: Cross-sectional studies with a total of 11,080 students (age, 7–14 years) in grades 1, 4, and 7 were evaluated for weight, height, routine physical examination, and urine analysis during the 2010 Student Health Examination in Hualien. Frequencies, Chi-square test, and logistic regression were conducted using SPSS. RESULTS: Of the 11,080 students evaluated, 1357 (12.2%) were overweight, and 1421 (12.8%) were obese. There were significant differences in overweight/obese prevalence by gender, by grader, and by levels of residence urbanization. Dental caries, myopia, and obesity were the most prevalent health problems among these students (75.6%, 33.0%, and 12.8%, respectively). In crude and adjusted analyses, research results showed that there were significant differences in the prevalence of major physical examination items between different gender, grader, levels of residence urbanization, and BMI groups. Girls had a higher prevalence of dental caries, myopia, and hematuria than boys (all p < 0.01), whereas boys had a higher prevalence of pinworm than girls (p = 0.02). Students in higher grades had significantly higher prevalence of myopia, hematuria, and proteinuria (all p < 0.01), whereas students in lower grades had higher prevalence of dental caries and pinworm (p < 0.01). Students with abnormal BMI had lower prevalence of pinworm (p < 0.01). Students residing in suburban and rural areas had higher prevalence of dental caries, pinworm, and hematuria (all p < 0.01), and lower prevalence of myopia than students residing in urban areas (all p < 0.01). CONCLUSION: Routine health examination provides an important way to detect students’ health problems. Our study elucidated major health problems among school children in Hualien, Taiwan. In addition, the results also indicated that the prevalence of health problems had a significant relationship with gender, grader, levels of residence urbanization, and BMI. It is suggested that school health interventions should consider students’ health profiles along with their risk factors status in planning
